Dear Sir,

I/We have been engaged as Structural Design Consultant for the structural part of the
development undertaken by

Name and Address of the Developer/Builder on property bearing

as per architectural plans prepared by Name and address of the Architect under the
supervision of Name and Address of the PMC/ Site Supervising agency.

The construction work was carried out by Name and Address of the contractor

OR

The construction work was carried out by the Owner/ Developer/ Builder departmentally by
purchasing materials and engaging labour at his cost.

Based on completion certificates issued by:

1] Name of the Owner/Developer/ Builder

2] Name of the Contractor and
3] Name of the PMC/Site Supervising Agency
and based on the reports, memos and periodic site inspection carried out by me/ us through
my staff from time to time, I/We certify that to the best of my/ our knowledge and belief, the
structural work is carried out as per the structural design & drawings prepared by me/ us and
memos & reports issued by me/ us.

I/We further certify that to the best of my/ our knowledge and belief, the structural work is
carried out as per prescribed applicable standards and codes, rules, regulation and stipulations
and that the materials used and the workmanship and methodology employed for construction
work are of accepted standards, and the work is fit for occupation for its intended use.
This certificate is issued on the clear understanding that my/ our overall responsibility for the
structural stability of the building and its proper structural performance will cease the moment
any additions or alterations to the structural frame by accident or due to tampering by the
users/ occupants for any reasons whatsoever. My/ our responsibility will also cease in the event
of overloading or lack of paper maintenance of structure or any such act, which is detrimental
to the structure

Two sets of final structural drawings executed along with the design data are enclosed
herewith.
