xFusion Technologies Inc. (xFusion) is pleased to provide our offer to the California Department of
Health Care Services (CDPH) in response to RFO# 23-10831- IT Consulting Services.
We are confident that we have a thorough understanding to provide support and assistance for their
CID reorganization effort by applying PAL best practices to facilitate administrative working groups for
the CID Administrative Working Group Team members, Transition Management Task Force, Transition
Task Force Working Groups, Transition Management Task Force Steering Committee. We shall
provide the CDPH newly launched Center for Laboratory Sciences (CLS) support, facilitation, and
coordination with the Director’s Office using and applying PM best practices to establish processes,
and documentation for the new Center. We will support the Phase II of its re-organization effort to
reassess the new and current CLS organization for the identification and implementation of efficiencies
in administrative and laboratory areas and provide services to partner with the Director’s Office and
CLS leadership to support schedule management, track issues, risks, and decisions, and provide
overall implementation oversight.

Our proposed candidate Priyanka Das is an accomplished Business Solutions Analyst with a rich
background in the software engineering industry, specializing in projects for the State of California.
With extensive experience in the Software Development Life Cycle (SDLC), including Agile (Scrum)
and Waterfall methodologies, Priyanka has contributed significantly to various projects, such as
those for the Department of Water Resources, Franchise Tax Board, and the Department of Motor
Vehicles. With a solid educational foundation, including a Bachelor of Technology in Computer
Science & Engineering and a master’s in software engineering, Priyanka has also earned Scrum
Web Developer Certification from the Scrum Institute.

A detailed proposal addressing the specified requirements is enclosed with this response. We have

2. For services satisfactorily rendered, and upon receipt and approval of the invoices, the State agrees to
compensate the Contractor for actual services performed and accepted in accordance with the Classification rates
specified. Under no circumstance shall the Contractor be entitled to payment for preparatory expenses and any
anticipated future needs.
3. Personnel resources expended on task accomplishment in excess of authorized cost will be at no cost to the
State.
4. The Contractor further understands that the hourly rates must be fully loaded, including but not limited to,
operating expenses, labor, equipment costs, supplies, overhead, annual inflation costs/rate adjustments, profit
margin, taxes, shipping, and etc.

5. No travel will be reimbursed under this contract.


6. Subcontractor may be utilized for this project.

The Offeror hereby certifies that the materials submitted in response to this RFO, and the hourly rate(s) offered on
this Cost Worksheet are true and accurate to the best of its knowledge and shall remain in effect throughout the
term. Amendment rates, if allowed for time and/or money, must be consistent with the original rates in the offer at
the time of assessment.
