CASE SCENARIO: A 56-year-old patient came to ER with chief complaints of sudden onset of left-
sided weakness at midnight followed by dizziness, vertigo, slurred speech, and 6 to 8 episodes of
vomiting on the way to the hospital, initially taken to RIC hospital from there he referred to SIH. on
the arrival in ER GCS was 14/15 and then dropped to 7/15 and bp at that time was 160 \110 mm\hg.
CT showed that he has a right Thalamic bleed with upstream hydrocephalus. So, the admitting
diagnosis of the patient was hemorrhagic stroke (right intra-cerebral bleed).
Past medical history: the patient had hypertension from the past 7 years, cardiovascular arrest 8 to
10 months back, and complaint of left-sided chest pain once in the last 2 weeks.
Family history: hypertension and cardiac diseases were also positive in a family of patients.
Social history: smoker.
