Delivery Driver Agreement

To become and remain a delivery driver for Chaac Pizza Midwest, LLC, I understand and agree
that while on duty:

*This Agreement applies to all forms of transportation: cars, trucks, SUVs, motorbikes and
bicycles.

I WILL:

• Maintain auto insurance liability coverage as required by my state and Chaac Pizza
Midwest, LLC. I will supply a copy of my insurance information to my RGM.
• Always obey all traffic regulations and wear my seatbelt.
• Notify my RGM immediately if I or my vehicle is involved in an accident, receive any
moving violations while on or off duty, or if a change occurs in my driving privileges
or auto insurance coverage.
• When on duty, display Pizza Hut signage on my vehicle.
• Keep my vehicle clean and in safe and good repair.
• Have a current and valid Vehicle Safety Inspection and Motor Vehicle Registration
throughout my employment.
• Report all of my tips when filing income taxes as required by the IRS.

I WILL NOT:

• Operate my vehicle while I am under the influence of alcohol, marijuana, or any other
unlawful use of a legal drug or substance; or store these prohibited substances in my
vehicle.
• At no time will I operate my vehicle if I am physically or mentally impaired from
operating it safely regardless if the impairment is caused by legally prescribed drugs.
• Carry any passengers (other than fellow Team Members who are training or on duty).
• Carry firearms or other weapons unless permitted by law.
• Smoke, eat, drink, talk or text (including reading text messages) on the cell phone, wear
headphones or ear buds while driving or otherwise engage in any activity that may
distract me while driving.
• Use a radar detector while on duty.
• Resist or chase a robber, thief, or any other person.
• Display any offensive/obscene inflammatory flags, signs, or decals on my vehicle.
• Make personal stops (i.e., gas station, home, friend’s house, etc.) while on the clock.

I UNDERSTAND THAT:

• I will be reimbursed based on the type of car I drive for delivery tips and multiply the
total miles calculated by the mapping program from drivers’ deliveries during the shift
by the applicable Reimbursement Rate. (Ask your Manager for the current rate and to
 review the driver reimbursement policy). I understand that my reimbursement may be
subject to income tax withholding in the event it exceeds federal limits.
• I may be terminated or reassigned to non-driver duties if I do not maintain an
acceptable driving record.
• It is my responsibility to check with my auto insurance to ensure that my policy covers
bodily injury and property damage to others, in the event of an accident that I am
involved in while on duty.
• The Company does NOT provide insurance for any damage to my vehicle.
• GPS tracking may be used to monitor the safety of my driving behavior while working.
