Overview

It’s the 1960s. Susanna Kaysen, 19 years old, recently took an overdose of aspirin and
vodka, although she insists it was not a suicide attempt. She has been sent to Claymoore
for an extended hospital stay (common for this time period).

At Claymoore, Susanna meets other women on the ward with various psychiatric
problems. Lisa Rowe is one person Susanna gets to know well during her stay. Lisa is
erratic, acts tough, and doesn’t seem to care about anything.

You are a nurse working with both Susanna and Lisa.

What is the chief complaint? (In the client’s words, and report of others)

Response:Susanna reports feeling “empty” She suffers from headaches, is fatigued,


and believes she needs “rest.” She has bruises on her wrists suggestive of self harm
and suicidal thoughts that she portrayed as “half-hearted attempt” to kill herself by
taking a bottle of aspirin with vodka. She struggled with self identity issues as she
experienced increasing pressure from her family to be normal and later described
herself as ambivalent in an effort to suggest that she just doesn’t care and is going to
do what she wants.

Based on the above information and a close viewing of the movie, what questions would
arise during history taking? What are some possible answers? You might base your
questions on:

Response:When did these feelings start and what caused them? How do these
situations make you feel?
- She first notices her feeling in high school when she realizes she is different from
others. Her general disinterest and apathy is portrayed by her falling asleep at her
graduation ceremony. She continues to questions her identity and wether things
are real when she is mocked about her future plans to become a writer. When a
school administrator points out that she is the only one not attending attending
college she questions if she is crazy. She has had fantasies about suicide and
discusses death with a friend.

How is your relationship with your family? With peers? How does this make you
feel?

- Susanna displayed an intense and unstable relationships pattern with family,


friends, and loved ones, often swinging from extreme closeness and love to
extreme dislike or anger. The relationship Susanna has with her parents stems
from abuse, neglect and constant pressure to be normal. Susanna’s parents are
troubled because she isn’t going to college and embarrassed by her behavior
suggesting poor family dynamics. Susanna states she doesn’t have any close
girlfriends outside of the hospital and any relationship she has with men is sexual
in nature.

What observations do you have about the client’s behavior?

- She has the inability to understand that she has a probelm.
- Some behaviors of the disorder intensify briefly while she is
admitted such as poor
impulse control, ambivalent feelings, labile mood, taking reckless chances and breaking
rules at the facility.
- Susanna experienced extreme shifts in behavior and mood. In certain situations, she
appeared to be normal and could go on with her day to day life. Other times she acted
impulsively and did not care at all.
- Lisa has a very strong presence on Susanna suggesting Susanna conforms to people
around her
- Day to day consequences seem to have little effect on Susanna. She only responds to an
situation or major crises.
