1. Obtain and describe the form of organizational structure of the nursing division
on which you interview / study. Discuss changes that could be made to make it
more functional.
Mendero Medical Center (MMC) is the only tertiary hospital in northern
Cebu operating in full swing with its complete range of healthcare facilities and a
line of experienced medical experts. Each department of the hospital uses
different forms of organizational structure according to the function of the
department and the needs of the staff members.
The Infection Prevention and Control Department of MMC uses the
staffing organizational structure for HESU (Hospital Epidemiology Surveillance
Unit). It is an organizational arrangement where tasks and responsibilities are
grouped based on specialized functions or roles. In this structure, employees are
organized according to their specific expertise or skill set, and they report to a
common manager who oversees that particular function.
Based on observation and according to the nurse's response during the
interview, there is no need to change the organizational structure of the IPC
department because, in every ward and special area, there is always a
designated staff nurse who's responsible for informing the head nurse or charge
nurse of the unit when there are patients with notifiable diseases. Instead of
changing the structure, it would be best to continuously make improvements to
ensure that the functionalities in the hospital run smoothly.

2. Obtain and describe the organizational chart of nursing from any department. Is
it centralized or decentralized?
The hospital's IPC department uses a centralized approach in its
organizational design and staffing. Centralization is the process where activities
involving planning and decision-making within an organization are concentrated
in a specific leader or location. The decision-making powers are retained in the
head office, and all other units are directed from this chief office, resulting in
communication flowing in a top-down manner.
In this regard, the IPC department's head or team leader decides the
terms of scheduling and division of work among the staff members. The staff
members find it advantageous because it makes communication flow easier.
They also find it organized as it makes daily delivery of reports to the
Department of Health (DOH) faster.
3. Discuss the functioning of the nursing delivery system (s) in the health care
organization and does it make the nursing delivery system more efficient or
effective? How do you improve it?
The hospital's IPC division's nursing care delivery system utilizes the
functional nursing model to optimize patient care delivery. The Functional
Nursing Care Delivery System is a model where tasks and responsibilities are
divided and assigned based on the specific functions or skills of nursing staff. In
the context of the IPC unit, implementing a functional nursing care delivery
system involves organizing and assigning tasks to address infection prevention
and control measures efficiently.
The functional nursing delivery care system is indeed helpful as it helps
grasp the specific skills and expertise of nursing staff to enhance infection
prevention and control measures, ultimately contributing to a safer healthcare
environment. Without the proper organizational structure and nursing care
delivery plan, the system can become chaotic, and the reporting of diseases may
not be organized. Regular training, enhanced communication, and evaluation are
essential to improve the current care delivery system of the unit and ensure
patient safety and faster recovery from the disease.
