This questionnaire is about your experiences with your parent (mum or dad) or caregiver regarding
physical activities. You are doing a physical activity when you are moving your muscles and using
energy more than when you are resting. This questionnaire is about the types of physical activities you
already do or would like to do in your free time, like playing ‘chasey’ or a game of football.

Your responses are private – ONLY the researcher will be able to see your answers unless another adult is
helping you with this questionnaire.
