21/03/2024 01  Poor stacking of rebar resulting in rusting.
 There are no CWP’s/ MS’s at site with the QA/QC team for works like excavation
works, Engineering backfilling works, Blinding works etc that have already begun at
site.
 GFC / client stamped drawings are not available at site.
 Documentation of works at site is sub- standard.
 Concrete slurry being used to protect rebar from rusting claimed as a standard
practise instructed by CPWD. Kindly advise on the bonding capacity of old concrete
(slurry on rebar) & new concrete (poured while concreting).
 Site supervisor was not available at site at 9.00am.
 No Safety engineer at site.
 No P&M engineer at site.
 No Time- Keeper at site.
 Plant and equipment are not screened/ maybe not fit for use. Pls ensure
compliance with all necessary documents.
02  There are no GFC drawings at site. Drawings without client sign are being used
at site.
 Site engineering claims that CPWD is yet to provide GFC/ Stamped drawings to
the Sub-Con.
 These are internally approved drawings by the third party of the Sub/Con.

0  Workers are working without PPE.
 No handrail for edge protection (observed depth of more than 1.0m)
 No working access for workers.
 No Safety engineer till date at site. Site engineering team claims that the HSE
officer is on leave but there is no replacement at site.
 Workers are not screened.
 No record of daily Pep- talks at site.

0 No Cover at one side of the footing. Site engineering team admits doing open ended
concreting without shuttering. MS’s are not available at site for the same.

07 Site engineering team claims this to be the DB for electrical supply to site equipment like
drill machines and handheld cutter machine. This was placed inside one of the rooms of the
labour shed.

08 Inadequate labour welfare facilities- ceiling fans are missing in all rooms. The ambient
temperature is 36 degrees already and the site team admits that they are about to purchase
fans now.

09 No water facility in toilets.
These toilets are not being cleaned regularly.

10 No fans in operators’ rooms

11 No ETP system- effluent water is being left to open ground. Basic environmental practises
and hygiene measures are missing.
