1) Applicable to the Enhanced Survey requirements

2) C = Coating, A = Anodes, SC = Soft Coating, NP = No Protection

3) Hard coating condition is described according to the following standard:

GOOD Condition with only minor spot rusting.

FAIR Condition with local breakdown at edges of stiffeners and weld connections and/or light rusting over 20% or
more of areas under consideration, but less than as defined for POOR condition.

POOR Condition with general breakdown of coating over 20% or more of areas or hard scale at
10% or more of areas under consideration.

If coating condition LESS THAN GOOD for tankers (oil and chemical) or POOR for bulk carriers is given, other than double bottom ballast tanks,
extended annual surveys are to be introduced. This is noted in part 8B.

4) Substantial Corrosion, i.e. >75% of acceptable corrosion margins wasted.

5) NOTE: For BULK CARRIERS over 15 years of age: (7.3.2 / 1.1.10(b))

i) Ballast tanks, other than double bottom ballast tanks, in way of spaces designated for the carriage of cargo, where FAIR coating conditions
were identified at the previous Intermediate Survey or Special Periodical Survey are to have a min. of three (3) so identified tanks internally
examined (one forward / one midship / one aft).
ii) Peak Tanks reported with FAIR coating condition are to be internally examined at each annual survey.

9. Evaluation of the ship's longitudinal strength:

(For oil tankers of 130 m in length and upwards and of over 10 years of age - reference 7-A-4/33.3 of the SVR)
(Of sections A, B, C, below, only one applicable section is to be completed)

A. This section applies to ships regardless of the date of construction: Transverse sectional areas of deck flange
(deck plating and deck longitudinals) and bottom flange (bottom shell plating and bottom longitudinals) of the
ship's hull girder have been calculated by using the thickness measured, renewed or reinforced, as appropriate,
during the special survey most recently conducted after the ship reached 10 years of age, and found that the
diminution of the transverse sectional area does not exceed 10% of the as-built area, as shown in the following
table:

B. This section applies to ships constructed on or after 1 July 2002: Section moduli of transverse section of the
ship's hull girder have been calculated by using the thickness of structural members measured, renewed or
reinforced, as appropriate, during the special survey most recently conducted after the ship reached 10 years
of age in accordance with the provisions of paragraph 2.2.1.1 of Annex III and are found to be within their
diminution limits determined by the Classification Society*, as shown in the following table:

· Carried out and found satisfactory as per ABS Technical letter/fax dated: N/A

Notes *
The actual transverse section modulus of the hull girder of oil tankers calculated under paragraph 2.2.1.1 of
Annex III to UR Z10.1 is not to be less than 90% of the required section modulus for new buildings
specified in IACS Unified Requirements S7* or S11, whichever is the greater.

· C = 1.0 cn is to be used for the purpose of this calculation.

*1. Zact means the actual section moduli of the transverse section of the ship's hull girder calculated
by using the thickness of structural members measured, renewed or reinforced, as appropriate,
during the special survey, in accordance with the provisions of paragraph 2.2.1.1 of Annex III.
*2. Zreq means diminution limit of the longitudinal bending strength of ships. As calculated in
accordance with the provisions of paragraph 2.2.1.1 of Annex III.

· The calculation sheets for Zact are to be attached to this report.

Document No.: 14250070-6325593-435

C. This section applies to ships constructed before 1 July 2002: Section moduli of transverse section of the ship's
hull girder have been calculated by using the thickness of structural members measured, renewed or reinforced,
as appropriate, during the special survey most recently conducted after the ship reached 10 years of age in
accordance with the provisions of paragraph 2.2.1.2 of Annex III and are found to meet the criteria required by
the Classification Society and that Zact is not less than Z mc (defined in * 2 below) as specified in appendix 2 to
Annex III, as shown in the following table:
• Describe the criteria for acceptance of the minimum section moduli of the ship's hull girder for ships in service
required by the Classification Society.

*2. Zmc means the diminution limit of minimum section modulus calculated in accordance with
provisions of paragraph 2.2.1.2 of Annex III.

10. Conclusion
Evaluation of survey results indicates that vessel is fit for its intended purpose for the next five year period subject to
proper maintenance and operation and to periodic surveys being carried out at the due dates.
