SERVICE AGREEMENT FOR TEACHERS

This is to signify my intention and willingness to serve the school for the school
year 20___ - 20___. To start on _________ to ________, 20____. With teaching load in
this school, total no. of hours rendered including all assignments per day.

I hereby agree to be faithful to the duties and responsibilities of the position and
to abide by the terms and conditions of employment prescribed in the GNSSAI including
the following:

a) To enter in the attendance logbook each working day exact time of arrival and
departure every morning/afternoon to indicate presence and/or tardiness so will
mean absent for that particular period.

b) To be present and prompt in attendance in all curricular, co-curricular, extra-

curricular, and other school activities as the exigencies of the service require.
Corollary to this, to be prompt in reporting to work station and diligent in
overseeing entrance to and exit from the classroom by pupils so as to maintain
order and quite thereat; also to avoid lingering outside the classroom conscious
that without the presence of teacher in the classroom at this time, pupils are
without supervision usually resulting in unruly, rowdy, naughty, unbecoming and
disorderly behavior.

c) To attend and participate in the regular monthly meetings and other special
meetings that may be called from time to time.

d) To accept other work, assignments in additional to my teaching

supervisory/administrative responsibilities as exigencies of the service require.

e) To give achievement and periodical test on the appointed day and hour as
specified in the school calendar, to immediately check test paper and records
score, compute grades and accomplished grading sheets together with Class
Record 138 and submit to the office together with duly corrected test papers not
later than ten days before the next periodical exams, and for the final grades, not
later than three days before the graduation day.

f) To observe the Speak-English-Tagalog-Mother tongue within the school

premises and take leadership role/or cooperate in its enforcement in all areas in
the classroom especially in the class.
 g) To observe the Uniform Drive of the school corporately and take leadership role
in enforcement among the school population.

h) To help discipline pupils, particularly those in my area of responsibility.

i) To perform such other function as may assign as the exigency of the service
require.

j) If absent, a salary deduction may be done. And the salary that is deducted will be
given to the teacher who will take over the class.

k) I agree that if within the contract period I leave the service, the school shall have
the right not to release/give me Certification, documents, credentials or subject to
a Fine of Php______.

This document is a formal contract between the school administration and the
