Introductory
Transfers of patient care are a crucial but
sometimes overlooked component of patient care. A crucial part of patient
care is patient care transfers, which need for strict adherence to clinical
guidelines. Appropriate transfers are predicated on the idea of preserving
continuity of care throughout and following the transfer.
Transportation equipment
Ambulance is a medically-
equipped vehicle used to transport patients to treatment facilities,
such as hospitals. Typically, out-of-hospital medical care

Steps/Procedure:
Knock the door of the patient
1.Knock 3x on patient’s door
2. Introduce your name to the patient
3. Ask patient name.
4. Inform the patients of the doctors advise that he will
be transported
5. Place the wheelchair 45 degrees from the patient’s bed
6. Assist the patient to seat at the edge of his bed
7. Put a transfer belt to patient’s waist area
8. Place the weak side arm of the patient to a transfer
belt
9. Place the weak toe and knee of the patient to your toe
and knee
10. Put the strong arm of the patient to your arm
11. Hold the transfer belt of a patient and patient
will stand in a count of three
12. The Strong side foot is the nearest to the
wheelchair
13. The patients strong side arm will hold the
wheelchair to support his weight.
14. In the count of three enduring will seat on the
wheelchair and
15. Assist the patient to scoot to seat comfortably
16. Put the patients’ feet on the foot rest then put
his sleeper
17. Unlock the wheelchair
18. Arrange first the patient’s bed
19. In transferring always, the health care trainee
will be the first to go out to protect the patient
20. In gong inside the patient’s room, Health care
trainee will be the first to enter and park the patient
into the foot part of his bed
21. Unlock the wheelchair then arrange the knee of
the patient then toe to toe and knee to knee
22. Inform the patient that in the count of three he
will stand while his strong arm is place in arms of
health care trainee
23. The weak side foot of the patient will be place on
top of the toe of the trainee
24. The weak toe of the patient is near in the bed
that will be the first to move going to bed
25. In a count of three patient will seat back into his
bed and assist the patient to scoot for him to be seated
in the center of his bed
26. Then untie the transfer belt of the patient
27. Back to the lying position put the strong hand of
the patient’s bed corner
28. Insert the arm to the leg of the patient then
pivot.
29. From high fowler to simi fowler to a lying
position
30. then put pillow on the patient and make a
