AFFIDAVIT OF LOSS

1. That I am a License Real Estate Broker holding a Real Estate Broker identification
number / /valid until / / and issued by Professional Regulation Commission;
2. That I lost the abovementioned professional identification card sometime in May
when I was at SMDC head office in Pasay City. I went back to retrieve the same,
however, I was told by the office staff that they could not find it;
3. That I tried to search for the said identification card but the attempt was futile
despite the diligent effort exerted in locating the same;
4. That I hereby declare that the same is lost and beyond recovery;
5. That I am executing this affidavit to attest to the veracity of the foregoing facts for
whatever legal purposes it may serve.
