Jocelyn Uy Po to process and claim the PSA Birth Certificate of my child, Patrick Paul P.
Radam, my PSA Marriage Certificate, and the PSA Death Certificate of Joyce P. Radam.

I understand that Jocelyn Uy Po will be acting on my behalf for the aforementioned task, and I
authorize her to sign any necessary documents, provide information, and undertake any actions
required in connection with the processing and claiming of the said documents.

I trust that Jocelyn Uy Po will handle this matter with utmost responsibility and in accordance
with all relevant regulations. I also acknowledge that any decisions made or actions taken by
Jocelyn Uy Po in connection with this authorization letter are legally binding on my behalf.
