AFFIDAVIT OF CONSENT

I, SATOSHI ISHIHARA, a minor, Japanese and with local resident of

267 Sampaguita St., Dalig, Antipolo City, after having been duly sworn to in
accordance with law hereby voluntarily depose and state that:

1. That I am one of the legitimate child of petitioner TAMOTSU

ISHIHARA AND SHIRLEY G. ISHIHARA (SPOUSES) who was born on
20 January 2009 in Japan;

2. That I have been under the care and custody of petitioners Tamotsu
Ishihara and Shirley G. Ishihara since birth;

3. That the petitioners desire to adopt SHIRLYN S. GOCELA as their

daughter;
4. That her adoption by the said petitioners is for her best interest;
5. That I hereby give my consent to this adoption by petitioners Tamotsu
Ishihara and Shirley G. Ishihara; and

6. That this affidavit is executed in order to attest to the truthfulness of

the foregoing narration of facts and as basis by any offices concern to approve
my adoption and for whatever purpose or purposes it may serve.
