1. Preface
On 18/07/2023, a FNR issued by CCR stating << Four fire alarms & 34 troubles persist in
Simplex fire fighting panel & the time of the controller doesn't match with actual time >>.

3. Findings & Recommendations


1. Spider net and other dust blocks the smoke detector and multisensor detector which causes
‘Dirty’, ‘Excessive Dirty’ trouble and ‘Fire’ alarm in some cases. These detectors have been cleaned.
However, two SD and two MSD found faulty and replaced. Regular maintenance work will be done
to prevent these trouble as much as possible.

2. Two detector base found faulty which caused ‘No Answer’ trouble. These bases are replaced with
spare.

3. Glass of one Manual Call point found broken which has been replaced. Looseness in another MCP
caused ‘Open Circuit Trouble’ which has been tightened. In other two MCP, monitor module found
faulty which caused ‘No Answer’ alarm repeatedly.

4. One flame detectors monitor module found faulty and replaced. This solved its repeated ‘No
Answer’ alarm.

5. No tools for breaking glass of MCP in case of fire, not found in most of the MCP. In case of a fire
breaks out, personnel will not be able to break glass and use the call point. These tools must be
placed near every MCP.

6. As part of IMS objective of I&C department, 27 new device configured in Truesite Workstation,
of which 3 devices already installed. Other 24 devices will be installed. Currently these devices
showing ‘No Answer’ trouble. These trouble can be ignored.

7. ‘BD Workshop Building’ Fire Alarm comes frequently caused by the three beam detectors blocked
by crane movement. Before moving crane in workshop, responsible personnel should inform CCR.
After finishing crane use, these beam detector should be hard resetted by turning off and on of the
specified breaker behind BOP FF panel.

8. In case of any ‘Fire’ alarm, Operation personnel should investigate and notify I&C department if
the alarm found to be a ‘False Alarm’.

4. System Description
Simplex 4100 ES panel is used for BOP FF with smoke detectors, heat detectors, Multi-sensor
detectors interfaced through IDNet and MAPNet communication. The devices are divided into 7
zones.

6. Work Details
1. ‘L7/40 MSD Guard House B - DIRTY’ – MSD found dirty and cleaned. For some reason, this MSD is
getting dirty frequently. Suspected cause is the dirt in air blowed by nearby ceiling fan.

2. ‘L7/47 Hydrogen Generation Plant – NO ANSWER’ – Detector base found faulty and replaced.
Detector head also found dirty, faulty and replaced.

3. ‘L6/127 Admin DGM Passage, First Flr, BFC – Excessively Dirty’ – this MSD first cleaned, then
replaced.

4. ‘L7/44 Hydrogen Generation Plant – NO ANSWER’ – both detector base and head found faulty
and replaced.

5. ‘L4/141 MSD Fuel Unloading & Transfer PMP – Excessive Dirty’ - MSD cleaned.

6. ‘L4/96 MCP Outside HSD Service Tank B – Bad Answer’ – this MCP address found wrong and
swapped with L4/99. Cleaned. Address corrected. Monitor module found faulty and replaced.

7.’L4/98 MCP Outside HSD Service Tank B – Open Circuit Trouble’ – looseness found and cleaned.

8. ‘L4/102 MCP Outside HSD Service Tank B – No Answer’ – monitor module found faulty and
replaced.

9. ‘L4/88 HSD Service Tank B – Heat Det 1 – No Answer’ – Heat detector need to be replaced.

10. ‘L4/60 HSD Storage Tank B – Flame Det-2 – No Answer’ – Monitor module found faulty and
replaced.

11. ‘L1/168 GT Control Package A ‘Fire’’ – found disabled. Enabled.

12. All MSD/SD in HGP and Fuel Forwarding area have been cleaned.

13. ‘L1/173 CO2 ‘Pre-Alarm’ GT Package – Fire Alarm’ – two resistor found shorted. Made them
separate.

14. L4/98 – MCP Outside HSD Service Tank B – No answer – Monitor Module found faulty. Replaced.

15. L4/96 – WP MCP Outside HSD Service Tank B – Bad Answer- address of two device may be
same. Unsolved.

15. Following devices are part of new fire device installation plan as part of IMS objective of I&C
department. ‘No Answer’ means devices are configured in the system but not yet installed. These
alarms will be gone after device installation. Please ignore the following troubles.
