ACCOMPLISHMENT REPORT

A. PUPIL DEVELOPMENT
1. Started the implementation of CI Projects: Project STARS (Grade 1
Reading), Project REACH (Grade 6 Reading), Project MAPA (Grade
3 Araling Panlipunan), and Project E-MATH (Grade 5 Math); and
Project MULTIPLY (Grade 2 & 3 Math)
2. Implemented Individual Remedial Reading to selected learners
3. Administered School-based Feeding Program to 30 learner-
beneficiaries for San Jose ES
4. Enhanced values formation program of learners through the
conduct of religious instructions every Wednesday

B. TEACHER DEVELOPMENT
1. Conducted regular meeting with teachers to discuss department’s
programs and projects under the new normal set up
2. Provided teachers with psychosocial support and technical
assistance through mentoring and conference with them

C. CURRICULUM DEVELOPMENT
1. Prepared a draft plan on the conduct of localization of reading
materials for Grade 6 as part of the Learning Recovery Program of
the school

D. PHYSICAL FACILITES DEVELOPMENT

1. The two (2) buildings for demolition were already demolished after
securing the necessary requirements from the Division Office and
Office of the Municipal Engineer.
2. Improvements were made in the school garden as part of the
intensification of the “Gulayan sa Paaralan” program of the school

E. COMMUNITY LINKAGES/RESOURCE GENERATION

1. Generated support from different stakeholders such as Bureau of
Fire-Calumpit, SPTA officers and members, and Tau Gamma Phi-
Calumpit Chapter for the Oplan Linis program of the school after
the massive flooding.

F. REPORT MANAGEMENT
1. Regularly practiced the 4S of report/record management
