Under penalty of perjury (law 1599/1986), I hereby declare the following:

1. The personnel working for my company as well as myself have never been convicted of any felony in relation to
theft, fraud, embezzlement, extortion, forgery of documents, giving false statements, or of any drug or illicit arms
trafficking-related crimes.
2. I have never been indicted for any of the aforementioned felonies or misdemeanors, including cases where the
statute of limitations has run out.
3. The personnel working for my company as well as myself are insured with a state insurance company and we all
absolve the Hellenic Air Force of any liability in the event of an accident in the course of our work on-base.
4. I am responsible for providing the 115th CW Security Office with proper documentation (insurance provider-
background check, copy of police ID/passport) for all the personnel working on the subject project, before initiation of the
project.
5. At the end of every month I shall provide an updated version of the list of names for the personnel working on the
project along with the corresponding updated insurance information.
6. Upon completion of the project, I am responsible for handing over to the 115th CW Security Office all individual
access passes, as well as any vehicle passes given to my personnel.
7. In the event that information provided herein proves to be inaccurate or incorrect or changes prior to the initiation
of the project, I am fully aware that I will not be granted access, or might be forced out, and will be subject to financial
and legal sanctions.
8. I consent to the handling of my personal information as well as that of my personnel’s.
