Hospitals report around 6.5 medication error-related adverse events per 100 admissions
(Carver & Hipskind, 2019). Over half occur during administration, a critical interception point. A
review found a 25.2% error rate during hospital administration, with severe harm or fatalities
linked to these errors (Azar et al., 2023).
