Evaluation Instrument

Jamie Lampkins - ITEC 7460

To collect feedback and assess a school-wide instructional technology coaching program, we will create
two online evaluation forms via Google Forms. The first form will be distributed immediately after the
training session, and the second will follow once participants can apply what they learned. We
encourage educators to offer honest feedback, as comprehensive input is crucial for meaningful
professional development. The questions on the forms are strategically designed according to Guskey's
evaluation levels for professional development, providing a structured framework to measure the
effectiveness and impact of the coaching program (Guskey, 2016).

Google Forms facilitates efficient data collection and analysis, enabling timely evaluations that guide
program improvements and decisions, thereby addressing the changing needs of educators. Each
question was assigned a specific level from Guskey’s levels of evaluation (2016). The levels are listed
below.
Level 1: Participants’ Reactions Level 4: Participants’ Use of New Knowledge
Level 2: Participants’ Learning and Skills
Level 3: Organizational Support and Change Level 5: Student Learning Outcomes
