4 Near Misses - is an incident which potentially could have caused injury or occupational illness and/or damage (loss) to people, assets, the environment, third p
5 First Aid Injury Cases – cases that are not sufficiently serious to be
reported as medical treatment or more serious cases but nevertheless
require minor first aid treatment, e.g. any one-time treatment and
subsequent observation of minor scratches, cuts, burns, splinters, and
6 Equipment/Property Damages – is damage to equipment or property
due to accident. Property damage may include harm to a facility,
vehicles, land, etc..
7 Medical Treatment Cases – is any work-related injury that involves
neither Lost Workdays nor Restricted Workday Case but which
requires treatment by, or under the specific order of, a physician or
could be considered as being in the province of a physician.
8 Restricted Work Day Cases – Any work-related injury other than a
fatality or lost workday case which results in a person being unfit for
full performance of the regular job on any day after the occupational
injury.
9 Lost Time Injury (LTI) – the sum of Fatalities, Permanent Total
Disabilities, Permanent Partial Disabilities and Lost Workday Cases but
exluding Restricted Work Case.
10 Work Related Fatality – fatality is a death resulting from an injury or
illness, regardless of the time intervening between injury and death.
For statistical purposes, a Fatality shall be recorded separately and as
a Lost Time Injury (LTI), see LTI definition.
11 Reportable Dangerous Occurrences – it is a significant incident
arising out of, or in the course of work that did not result in injuries
and/or fatalities but had the potential to have done so - shall be
reported within maximum 24 hours.

12 Reportable Serious Injuries – is an incident that shall be reported

Reportable 24 hours.Illness/Diseases – is any work-related

caused by exposure to environmental factors associated with the
employment. It includes acute and chronic illness or diseases that may
be caused by repetitive motion, inhalation, infectious organisms,

Incidents – is an incident where minor,
reversible damage requiring little or no remediation has occurred. No
material has escaped the local site or caused significant harm to the
environment and/or pose a health risk to workers or the community.
It can be contained, cleaned up and/or disposed of legally without
15 assistance.
Moderate Environmental Incidents – is an incident where moderate
reversible environmental impact has occurred or is likely to occur to
the environment and/or there is a moderate health risk to workers
and/or the community. Moderate, medium-term remediation and
regulatory intervention may be required. Substances or materials may
have escaped the site causing pollution of adjoining areas which may
require containment, clean up and/or remediation involving other
agencies and/or additional resources not available to local

Major Environmental Incidents – is an incident where irreversible or
long term environmental impacts have occurred or are likely to occur
to the environment and/or there is a significant health risk to workers
and/or the community. Significant, long-term remediation and
regulatory intervention will be required. Substances or materials may
have escaped the site causing significant pollution of adjoining areas
which will require containment, clean up and/or remediation
involving other agencies and/or additional resources not available to
local site management.
