1. The field is hereby informed that the deadline of submission of request/application

for reclassilication is on May 31, 2024. A11 requests submitted after the deadline
will still be accepted but will be processed and forwarded to DepEd-Regional Office
on January 2025.

2. The following enclosed checklist/templates must be attached to the requirements to

facilitate evaluation of documents:

A. Evaluation/Checklist of Requirements for Teacher II and III

B. Evaluation/Checklist of Requirements for SPED Teacher I to III
C. Evaluation/Checklist of Requirements for Head Teacher I to VI
D. Certification (Performance Rating)
E. Certification (Experience and Enrollment Data)
F. List of Teachers per Department
3. A1l applicants are requested to submit the COMPLETE documentary requirements
on or before the deadline to avoid delay in processing. Concerns or clarification must
be directed to Ms. Veronica G. Gimoro or Ms. Kristine P. Perocho of Personnel Unit
at 8882-5659.
