1.0 INTRODUCTION

1.1 GENERAL PROJECT INFORMATION


PETRONAS Carigali Iraq Holdings B.V. (hereinafter referred to as "PCIHBV”) is hereby
soliciting competitive Bid proposals for the establishment of Service Contract
(hereinafter referred to as "CONTRACT") for the PROVISION OF ENGINEERING,
PROCUREMENT, CONSTRUCTION, AND COMMISSIONING OF TOC STOCKYARD
PROJECT AT GCA (the EPCC) (hereinafter referred as “WORK” as specified in this
Invitation to Bid (ITB) Documents. CONTRACTOR Scope of Work includes the provision
of all things, whether of a temporary or permanent nature necessary to realize the
Project intent.

TOC Stockyard (GCCB) and its associated facilities (hereinafter referred to as


FACILITIES) is required to centralize the control and operation cater for the growing
need of the operation. The FACILITIES will be located inside GCA (between WPA+D
and WPP +Q) covering an area of approximately 100 meters by 170 meters comprise
of closed store structure, shaded area, office, guard post-wall, fencing, asphalt road,
toilets and associated infrastructure. The scope of FACILITIES shall include provision
for utilities and tie-ins with existing facilities.

1.2 PROJECT ORGANISATION


1.2.1 PMT’s Organisation
PCIHBV will form a Project Management Team (PMT) that includes with TOC for
management and supervision of the CONTRACTOR throughout the duration of the
CONTRACT.
PCIHBV will also assign a Resident Construction PMT to the WORKSITE(s) whose task
will be to monitor and audit the progress, quality and safety of the CONTRACTOR work
at WORKSITE(s). The Resident Construction PMT will also act as PCIHBV
representatives for all Construction and mobilise related matters.
1.2.2 CONTRACTOR’s Organisation
It is the duty of the CONTRACTOR to structure its organisation such that the
requirements of the Exhibit are strictly fulfilled without PCIHBV needing to be engaged
in the performance of the WORK. CONTRACTOR shall not rely on PCIHBV to check
and/ or verify CONTRACTOR’s WORK. CONTRACTOR shall inform PCIHBV in a timely
fashion, of any significant departure from agreed plans, compile and make available
statistics/ trends to support the performance of the WORK and provide for PCIHBV to
inspect all aspects of the WORK. CONTRACTOR is required to nominate a Project
Director to whom PCIHBV may address issues related to CONTRACT for effective
resolution.
The organization shall demonstrate the adequacy of organization structure to ensure
effective execution of the project and shall include the following:

1.3 CONTACTS WITH AUTHORITIES
1.3.1 Introduction
PCIHBV maintains and will continue to maintain, formal contacts and communications
with various Iraq Government and semi-governmental bodies.

For those matters which are CONTRACTOR’s responsibility under the terms of the
CONTRACT, it is PCIHBV’s intention that, after initial introduction by PCIHBV and
agreement of procedures between PCIHBV, CONTRACTOR and Authority concerned,
CONTRACTOR will conduct the contacts without involvement of PCIHBV. All
communication between CONTRACTOR and Authority shall be copied to PCIHBV.
1.3.2 Permits
1.3.2.1 Permits to be obtained by PCIHBV or PMT for which CONTRACTOR Assistance
is required
CONTRACTOR shall obtain and/ or assist to obtain, all permits, and approvals required
from the relevant Iraq Government and semi-governmental bodies on behalf of PCIHBV
and PMT to complete the WORK.
CONTRACTOR shall prepare the necessary documentation and assist PCIHBV or PMT
to seek applications and attend discussions with the authority concerned
(CONTRACTOR shall provide an Authority Coordinator to work closely with PMT or
PCIHBV).
CONTRACTOR is required to provide, as part of the WORK, all documents, calculations,
and other data in the required format for the use of PMT or PCIHBV.
1.3.2.2 Permits to be obtained Exclusively by CONTRACTOR
This section is deemed to cover all leases, permits and authorisations which are not
listed in Sub-Section 1.3.2.1 above. Unless PMT or PCIHBV at its discretion decides to
take all or part of the responsibility for a given application, CONTRACTOR is fully
responsible for obtaining all such permits, matching its schedules, whether applied for
PCIHBV/PMT or CONTRACTOR name, and for furnishing all necessary information
required by the authorities concerned. PCIHBV will only be involved in introductions as
stipulated in Sub-Section 1.3.1. CONTRACTOR shall be responsible for all costs of
obtaining permits and associated fees or royalties.
These permits include but are not limited to the following:
1) Visas and Permits to Work for CONTRACTOR and Sub-Contractors
employees, Supplier, Specialists, etc.,
2) Equipment registration, testing, operating and approval include pressurised
equipment and lifting equipment,
3) Electrical equipment certification,
4) Electrical installation and generator installation registration (if any),
5) Operation of radios during Construction (if any),
6) Import permits (if applicable) for Construction Equipment (CONTRACTOR
shall be responsible for all duties and/ or costs of Bank Guarantees),
7) Permits related to the disposal of excess soil and permanent waste,
8) All permits related to temporary facilities including licences for generators if
used on WORKSITE(s) (regardless of size), temporary sewage disposal,
certification of mobile, hydraulic cranes and other lifting equipment etc.,
9) Leases for temporary areas outside the WORKSITE,
10) Local Authority approvals to construct (if applicable) – this includes all Local
Authority matters associated such as Fire Department, Water Department,

Building Department, Public Health, Police, Army, Municipality, Road and


Bridge Department etc.
If necessary, CONTRACTOR shall complete forms, prepare Drawings, certified where
necessary by Iraqi registered Professional Engineers, and follow up to ensure timely
approval of applications.
1.3.3 Coordination with Approving Authorities and Other Parties
CONTRACTOR shall be responsible for obtaining the permits and approvals required
from relevant Regulatory Authorities.
The CONTRACTOR shall include in the Lump Sum Price, all fees payable to relevant
Regulatory Authorities to complete the WORK in the CONTRACT PRICE.
CONTRACTOR shall identify and provide a detailed schedule of when permit approvals
shall be obtained to prevent disruption to the WORK. A monthly status report for each
Authority Approval being sought and obtained shall be provided to PMT and PCIHBV.
CONTRACTOR shall be solely responsible for:
1) Identifying all Regulatory Authorities that have jurisdiction relating to the
execution of the WORK and installation of the facilities,
2) Determining the type of permit and approval requirements and submit to
PMT at least two (2) weeks after the date of CONTRACT Award (if
necessary)
3) Securing all necessary permits and approvals in a timely manner,
4) Ensuring that all regulatory requirements are met.
All relevant and required regulatory permits or approvals shall be in place before activities
are initiated. Copies of all regulatory approvals shall be submitted to PMT for reference
in a timely manner.

2.0 FACILITIES DESCRIPTION AND BATTERY LIMITS

2.1 GENERAL
The scope, as defined herein, shall give principal guidance of the WORK that are
required to be performed by the CONTRACTOR. Notwithstanding this, CONTRACTOR
shall read this Scope of Work in conjunction with the specifications and drawings given
in this Exhibit I and other acceptable International Codes and Standards, specifications
and the applicable Iraqi Government rules, regulations, and laws, such that a full
appreciation of the technical objectives and responsibilities pertaining thereto is gained.
Details Project Schedule and Milestones are as per Exhibit I –Schedule of Work.

2.3 PROJECT MANAGEMENT, ENGINEERING, PROCUREMENT, CONSTRUCTION


AND COMMISSIONING (EPCC)

2.3.1 General Scope of Work

The general PROJECT objectives are summarized as follows:

To design and build the FACILITIES in conformance and compliance with PCIHBV’s HSE
policy, GOVERNMENT and other relevant regulatory requirements, and BEST
INDUSTRY PRACTICES.

All design, site preparation, fabrication and construction work associated with the
PROJECT shall be performed in a manner that is safe for people and assets with as low
as reasonably possible (ALARP) impacts on the environment.

The overall objectives of the EPCC contract are:


• To meet all PCIHBV’s policies on Health, Safety and Environment;
• To adopt inherently safe designs for all installations;
• To minimize long term environmental impact;
• To minimize life-cycle operating costs (OPEX);
• To design and construct state-of-the-art installations that meet the requirements
of the PROJECT SPECIFICATIONS and all applicable codes and standards;
• To design and construct Installations that give maximum value to the PROJECT.
• To design and construct FACILITIES for a design life of twenty five (25) years;
• To supply and install the equipment not limited to the Appendix A. The
CONTRACTOR to get PCIHBV approval on the actual quantity.

It is the responsibility of the CONTRACTOR to design the layout, foundation and size the
dimensions of the FACILITIES including the support facilities thereof accordingly, in
order that the occupants shall be able to occupy the FACILITIES comfortably.

The FACILITIES shall be designed and constructed fully furnished with good quality
finishes to include the following:

• Complete Administration offices (workstations, furniture, printers and its


accessories),.
• CONTRACTOR to appoint Architect and consultant to further study on the
FEED, layout and room allocation as indicated in the preliminary plan layout and
provide recommendation for PCHIBV approval.
• The appointment of Architect is crucial to enabling a dynamic architectural inputs
and is key to governing the implementation of the building needs for the
minimum.
• CONTRACTOR shall appoint professional safety consultant to provide a detail
study on the human factor engineering (HFE) required for the office room this
shall include but not limited to lighting, ergonomics etc.
• Musolla, pantry and Dining Facility
• Providing fencing, T-Wall, main gates, escape gates, access gates in
periphery of the facilities.
• Ablution for male and female
• Toilet for male and female
• Prayer room for male and female
• Security guard building complete with toilets

The building shall be designed as Reinforced Concrete building with brick wall and
concrete roof. Steel frame design is not allowed for the building except for covered
parking and equipment sheds.
Smoke detection, fire alarm, sprinkler systems and others which to be finalised during
detailed design shall be provided for all structures within the FACILITIES as per
Specification for Building Fire Alarm System. Additional firefighting equipment, including
but not limited to fire extinguishers of suitable types and capacity, shall also be supplied
at all building corridors, rooms, offices, dining, Musolla and substation. The design shall
be in full compliance with PTS and/or NFPA requirement, whichever more stringent.
The building and open yard shall be levelled and finished in such a way as to facilitate
drainage of rainwater, which shall include drains and discharge points as necessary.
Landscaping work consisting of lawn, planter box and tree planting shall be required.
The building finishing shall be minimum as per Room Finishing Schedule provided
inside Technical Drawings and Document Reference
Additionally, the FACILITIES shall be supplied with all required utilities and services
including but not limited to electricity, lighting, water supply, drainage, ventilation and
air-split conditioning (hot and cold). Each building, including offices, Guard post,pantry,
toilet and Musolla, shall be fully furnished with good quality, subject to PCIHBV’s
approval. All the building room’s furniture and appliances i.e. tables, chairs, bench,
socket outlets, lightings, printers, cabinets, racks etc. quantities shall be adequately
designed and finalized during the PROJECT Detail Design Engineering. All cost
resulted from the detail design engineering shall be at CONTRACTOR cost.
International standard safety signs and illuminated exit signs shall be observed and
provided at all designated areas and emergency exits / escape routes respectively.
Overall building signage shall be supplied by CONTRACTOR. Details of the signage
shall be design during Detail Design Engineering and supplied accordingly during
building construction at CONTRACTOR as part of CONTRACTOR base proposal.
The electrical Substation building shall be designed as reinforced concrete structure,
elevated building type with sufficient clearance to allow for minimum cable bending
radius, and shall be continuously air-conditioned to maintain internal temperature below
25⁰C, with relative humidity below 60%. Cabling within the cable cellar shall be laid on
cable trays and ladders. The cable trays shall be neatly arranged and properly
supported above ground. Detail substation installation and electrical equipment
installation shall be complied as per Specification for Electrical Installation (Doc No. GF-
GNPB-E-SP-1005). The Generator sizing shall be finalized during the PROJECT detail
design engineering
The CONTRACTOR shall also design and construct a sewage system complete with
septic tanks, underground PVC pipes and concrete manhole with provision for pumping
out using vacuum trucks. Interconnecting piping shall be part of the scope.
Main access road, central parking and service roads to be designed and constructed in
the FACILITIES to ensure access to all units and facilities. Crash barriers shall be
provided in front of the parking lots and any installation close to the perimeter road edge.
Perimeter and internal road lighting shall be provided.
Cold Water Plumbing Services
The works consist of piping from external water reticulation system to the FACILITIES
buildings storage water tanks, the installation of water storage tank complete with
accessories, fittings, cold water distribution piping complete with accessories and
fittings and all other associated works, testing and commissioning to complete as per
drawings and design provided.

Hot Water Plumbing Services


The work consist of installation of electrical hot water storage heater complete with
accessories and fittings, installation of hot water distribution piping complete with
insulation, accessories and fittings and all other associated works, testing and
commissioning to complete as per drawings and design provided. Hot water to be
provided through out all the FACILITIES i.e. toilets, Musolla, common area, dining, etc.

All design, site preparation, fabrication and construction work associated with the
PROJECT shall be performed in a manner that is safe for people and assets with as low
as reasonably possible (ALARP) impacts on the environment.

CONTRACTOR scope of WORK shall include all provisions necessary to realize the
PROJECT and shall be included in the CONTRACT PRICE.
Storm Water Drainage System

The CONTRACTOR shall design the storm water system in such a way that the surface
runoff shall not be stagnated at the FACILITIES and the same shall be networked through
open ditches by gravity flow.

CONTRACTOR shall visit the site and familiarize itself with existing storm water drain
and possible discharge point.

In addition to these main ditches mentioned in above, small secondary concrete drain to
be constructed along the FACILITIES perimeter to collect storm water and discharge it
to the two main ditches.

All ditches shall be lined with concrete, fitted in such a way that plants cannot grow from
any joints or cracks. The Sumps shall be provided at drain intersection and where
necessary.

Sewerage Line

Sewerage line for the dining facilities shall be dedicated and separate from that of other
Stockyard Facilities. Grease tap must be provided before going into the sewerage
system.
The sewage system shall consist of piping system using HDPE or UPVC pipe of
approved standard, gully trap, inspection chamber, septic tank of sufficient capacity
complete with distribution inlet and outlet pipes with provision for pumping out using
vacuum trucks.
Septic tank is to be from Precast Glass Reinforced Plastic GRP septic tank restrained
with holding down straps to the concrete base to resist buoyancy due to shallow water
table. The CONTRACTOR shall carry out the restraining system in accordance with the
manufacturer’s instructions and specification.

The WORK includes, but not limited to, overall project management, planning,
monitoring, scheduling, and coordinating detailed engineering, procurement,
construction, pre-commissioning, and commissioning of the PROJECT.

CONTRACTOR shall be responsible to review and verify all such scope, specifications,
and components that are necessary to meet the overall requirement of this ITB. Any item
not identified due to any discrepancy or inaccuracy which results in not meeting the

overall framework and intent of the scope shall be deemed to be included and shall be
provided by the CONTRACTOR as part of the CONTRACT PRICE.

Detailed requirements concerning the CONTRACTOR's work scope are included in the
relevant sections of this ITB document.

The FACILITIES shall be designed, procured, constructed, installed, tested, and


commissioned in accordance with PROJECT SPECIFICATIONS which are part of this
CONTRACT and the applicable Local Government Authority Rules, Regulations and
Laws.

2.3.2 Specific Scope of Work

2.3.2.1 The civil works involved for the above section shall include: -

a) Preliminaries, mobilisation, demobilisation, supply of equipment, machinery and


manpower for the pre-construction and construction works.
b) Conducting an underground scanning at the identified areas as per the contract to
avoid any underground utilities damage.
c) Manually traverse pilot trenching every 40 m of the proposed project land to check
if there’s any underground utilities at the specified area.
d) Contractor to submit a method statement to protect the underground utilities in
case it found under the required project area.
e) Topographical Survey of the project area. Detailed survey of the original ground
or on existing sub grade and all its features including existing natural or manmade
structures along the proposed project area.
f) The CONTRACTOR shall perform necessary soil investigation and topography
survey as required at the CONTRACTOR’s sole cost. All soil samplings shall be
performed using labs from approved TOC labs or accredited Iraq government only.
g) Soil investigation for the proposed project area.
h) Temporary Traffic Management throughout the WORK duration.
i) Grading and survey and preparation of Horizontal and vertical alignment for
existing Sub grade. Contractor to ensure the slope or gradient for cut and fill
comply with Proposed design. Geometric d e s i g n o f the proposed project area
which shall include General Alignment, Plan And Profile, Horizontal and Vertical
Alignment, and Cross Sections.
j) Topsoil removal, with grading and compaction and lay and compact new sub base
layer, Type B, up to 95% maximum Dry Density for every 1000m2, CBR>30-50%
as per Iraq Standard Specification for Roads and Bridges (Department of Design
and studies,1983. Maximum sub grade layers shall be 200mm thickness.
k) Submission of subbase materials type B as per the soil reports and the designer
proposal.
l) Contractor to submit the sample and the sieve analysis for PMT approval.
m) Supply and lay and compacted the subbase type B materials up to 98% maximum
Dry Density for every 1000m2
n) To follow the submitted Engineering design by PMT Consultant and to appoint an
Engineering consultant to perform the ALL-other required drawings details.
o) Contractor to submit the concrete design mix formula for the concrete materials
including the specification and test report for every single material to PCIHBV.

p) Contractor to submit the concrete design mix formula for the TAR materials
including the specification and test report for every single material to PCIHBV.
q) Casting a plain concrete shall comply with American Concrete Institute (ACI).
r) Testing on materials and workmanship during pre-construction and construction
works
s) To ensure the construction area have sufficient installation of warning tape or hard
barricade and to install a warning signage board for safety matter.
t) Temporary Construction access road to the WORKSITE and facilities shall be part
of CONTRACTOR’s Scope of Work.
u) To take full responsibility if construction activities damages underground utilities,
affects environment and surrounding areas due to negligence, poor coordination,
etc.
v) Clean-up WORKSITE and surrounding areas after completion of WORK and
housekeeping in daily basis for the worksite.
w) Disposal of construction surplus and scrap during execution and upon completion
of the WORK to a designated place approved by PMT.
x) The below table stating the material testing details (for example and not limited
to):

3.0 CONTRACTOR / PCIHBV RESPONSIBILITIES


3.1 PCIHBV’S RESPONSIBILITIES
PCIHBV will but is not obliged to provide assistance to the CONTRACTOR in securing
necessary permits or data from Governmental Authorities and/ or agencies when
required under this CONTRACT. Failure by PCIHBV to obtain and provide such
assistance shall not relieve the CONTRACTOR from its obligation to secure the same at
its own costs, as follows:
1) Location for CONTRACTOR Construction facilities, such as site office and
warehouse.
The signing, countersigning or other endorsements of any drawings and documentation
by PCIHBV Representatives, agents or employees shall not be interpreted as implying

the responsibility for the correctness of such documents nor relieving the CONTRACTOR
of its obligations to review all information, data and drawings provided by PMT.
CONTRACTOR shall liaise through PMT or Local Iraqi Governments with other
Contractors in their respective interface areas related to WORK.

3.2 CONTRACTOR RESPONSIBILITIES


Notwithstanding other responsibilities specified in the CONTRACT, CONTRACTOR’s
responsibilities shall include, but not limited to the following:
1) Review and verification of the basic design contained in the ITB and the
CONTRACTOR shall assume full responsibility for the same. Any changes arising
from the verification of basic design shall be included as part of the CONTRACT
PRICE.

2) Development and supply of engineering for the FACILITIES based upon but not
limited to climatic conditions, geotechnical consideration, MATERIAL specifications,
environmental standards, codes and standards, reliability, and safety of operation
to the extent that comprehensive technical analysis can be performed.

3) Provision of competent engineering judgment and detailed engineering for all site
layouts, EQUIPMENT lists, EQUIPMENT data and specifications, piping and
instrumentation drawings and specifications, electric wiring diagrams, operation
and maintenance manuals, and emergency plans and procedures.

4) Provision of reference codes and standards, drawings, specifications, manuals,


test certificates of EQUIPMENT and report on test of MATERIAL of construction
during inspection by PCIHBV at CONTRACTOR or VENDOR’s shop.

5) Arrangement of all packing, import and export permits, transportation and


procuring insurances for EQUIPMENT and MATERIAL from point of supply to the
entry port in Iraq and onward dispatch to Site, including the clearance of
EQUIPMENT and MATERIAL at the port of entry and appointment of approved
shipping companies and forwarding agents.

6) Inspection and expediting of EQUIPMENT and MATERIAL during manufacturing,


as received, after installation and at site, and providing certificates of inspection
including performance of Factory Acceptance Test for all packaged EQUIPMENT,
control systems and key instrumentation components by qualified engineering
personnel familiar with the detailed engineering design requirements and
replacing any shortages of defect in receipt of, or for any damaged EQUIPMENT
and MATERIAL and for storage at site in accordance with approved procedures
and methods.

7) Provision of test certificates for EQUIPMENT and MATERIAL required by Project


Specification, Codes/Standards and the law and regulations of Iraq.

8) Accepting the WORKSITE on an “As is, where is Basis “and completing site
preparation work, including temporary access, roads, tracks, necessary
demolitions etc, as required for the WORK.

9) Supplying and providing its own Construction facilities, such as site office,
warehouse, electricity, mob water, communication, access road and other utilities
required until completion of the WORK.

10) To make available all necessary resources including but not limited to skilled,
knowledgeable, and competent personnel, specialist support, equipment, facilities,
computer software, management systems, home office support, etc., to enable the
CONTRACTOR to competently perform the WORK as required in the
CONTRACT.

11) Provision of project management, project control services, cost control, progress
reporting, reports, administration, and accounting to PMT and PCIHBV as defined
in CONTRACT for the whole of the WORK.

12) Provision of management, coordination and supervision services for interfacing and
coordinating with all other parties involved in the execution of the WORK.
13) To construct and install the WORK in accordance to Construction drawings,
specifications, and work procedures.
14) To perform all necessary activities to enable the CONTRACTOR to complete the
WORK in a safe and timely manner.
15) To obtain all approvals, licenses and permits from relevant Iraqi Authority necessary
for the CONTRACTOR to perform the WORK.
16) To comply with all regulatory requirements in all aspects of the WORK including the
procurement, Construction, and completion activities.
17) Preparation, submission for approval and implementation of Project Quality
Assurance plan suitable to ensure compliance with CONTRACT specifications and
other inspection requirements to PMT.
18) Preparation and submission of all Procurement, Construction, and Completion,
Quality Control documentation for PMT review and comment.
19) To warranty the WORK for a period as required in the CONTRACT and to avail all
the resources required during the WARRANTY PERIOD to the satisfaction of
PCIHBV.
20) Provision of services of an independent, recognised third party inspection agency as
required and including all inspection tools for critical items as defined herein and
elsewhere in the CONTRACT.
21) Procurement services and supply, including inspection, expediting, coordination,
acceptance, forwarding, payment of legal fees, payment of customs clearance of
equipment and bulk materials including sufficient overbuy, and procurement of
consumables for completion of the WORK.
22) Performing all material tendering, evaluation of bids and issuance of purchase orders
for all materials and equipment and deliver safely to WORKSITE in a timely manner,
per project schedule requirements.
23) Temporary Construction access road to the WORKSITE and facilities shall be part
of CONTRACTOR’s scope of WORK.CONTRACTOR is necessary to provide the
road diversion if the existing road obstructed for the public used.
24) Provision of operation and maintenance of Construction facilities and removal of the
same after completion of the WORK.

25) Implement requirement at WORKSITE for storage, handling and protection for all
CONTRACTORs procured equipment and materials.
26) Assignment of personnel, materials, equipment and support as required to perform
the WORK. This shall be done in accordance with Condition of CONTRACT –
CONTRACTOR’s Employees.
27) Warranting that the personnel, materials, equipment, consumables and logistical
requirements to ensure effective and safe performance of the WORK have been
determined. CONTRACTOR shall not change personnel assigned to the project
without prior approval from PMT.
28) In addition to the Schedule of Deliverables, CONTRACTOR shall submit for approval
its established document control system. This shall outline controls placed on all
documentation, received and originated by CONTRACTOR. A documentation and
correspondence pertaining to the CONTRACT. Successive revisions of each
document shall be retained for consultation and shall be updated in a controlled
manner.
29) Implementing a system of progress reporting by which PCIHBV remains regularly
informed of the status of the WORK in accordance with the CONTRACT
requirements.
30) Provision of inspection and testing services, welding qualification procedures and
test, facilities, equipment, materials, consumables and documentation necessary for
inspection and testing at all stages of the WORK, all as defined in the CONTRACT
and in accordance with other inspection requirements.
31) Procure and make available all equipment and materials necessary for the
successful Construction and Completion of the WORK, in accordance to the Scope
of Work, specifications and drawings.
32) Development and implementation of comprehensive equipment and bulk materials
control, traceability and tracking system throughout all phases of Procurement,
Construction and Completion of the WORK.
33) Construction of the facilities, including survey, site preparation, prefabrication,
fabrication, installation and erection as required in accordance with the scope of
work, specifications, drawings, etc as defined in the CONTRACT.
34) Provide all expandable/ consumable Construction supplies, fuel and lubricant for
Construction requirements including personal safety items (high quality hard hats,
eye goggles, raincoats, safety boots, etc) for CONTRACTOR’s workers and PMT’s
representatives.
35) Provision of general site security, including security for PMT’s (if any) and
CONTRACTOR’s work and accommodation areas, equipment, materials and tools
and also barricades or fence the areas as required.
36) Provision of English Spoken Supervisor/ Co-ordinator for efficient communication.
37) Provision, operations and maintenance of Construction accommodation camps,
temporary safety equipment and facilities during Construction and completion as
required for the performance of the WORK.
38) Disposal of Construction surplus and scrap during execution e.g existing Asphalt
materials and upon completion of the WORK to a designated place approved by
PCIHBV or relevant Regulatory Authorities.
39) Clean-up WORKSITE and surrounding areas after completion of WORK.

40) Management and coordination of external interfaces with all parties involved,
including provision of advice to PCIHBV or relevant Regulatory Authorities of any
interface matter arising therefrom. Obtaining and maintaining all permits,
authorisation and licences as may be required to be obtained in the name of
CONTRACTOR for the performance of the WORK.
41) Provision of all final documentation, “As-Built” drawings, maintenance management
system and close out report as defined in the CONTRACT.
42) Providing housekeeping at WORKSITE including Construction, accommodation,
office facilities and storage areas throughout the duration of WORK.
43) Provision of Construction All Risk Insurance in accordance with Condition of
CONTRACT – Insurance.
44) Provision of all water requirements including potable, utility and water for all activities
above. The supply of water shall be on a continuous basis throughout the duration
of WORK.
45) Provision of offices and related facilities for PMT or PCIHBV’s representatives in
accordance with the CONTRACT (if require).
46) Mobilisation all equipment, materials, consumables and personnel to the field and providing
transport for personnel to and from site.
47) Carrying out complete check for underground interference before commencing
Construction.
48) Providing temporary lighting at WORKSITE as per PMT HSE approval
49) Provision of Warranty Team to manage all warranty related problems/ defects items for the
entire duration of WARRANTY PERIOD.
50) CONTRACTOR shall provide the necessary power for their Construction work up to the
completion of the project and handing over to the PMT.
51) All temporary utilities, EQUIPMENT, MATERIAL, SUPPLIES and SPARE PARTS required
for the Construction, PRE-COMMISSIONING,
52) COMMISSIONING and initial Operation of the PROJECT shall be included in the
CONTRACT PRICE.
53) Provision of temporary sanitary facilities for Construction personnel and PMT personnel
stationed at WORKSITE with all regulatory approvals adhered;
54) Prepare and get approval for an Environmental Management Plan (EMP);
55) Maintain the WORKSITE as per regulatory requirements and undertake environmental
monitoring and audit to ensure compliance to the conditions imposed by regulation authority
and approved EMP.
56) CONTRACTOR shall procure all material and equipment necessary to complete the WORK.
57) CONTRACTOR shall comply with PCIHBV requirement or any relevant department.
58) CONTRACTOR shall ensure there is no vandalism act or any obstruction from outsider that
may harm during the work execution. And in case it happened, it will be the CONTRACTOR’s
responsibility to provide the works as per the SOW without any cost to PCIHBV and PMT.
59) In the event that any activity or WORK or the supply of EQUIPMENT, MATERIAL or
SUPPLIES of the nature necessary for the successful implementation of this CONTRACT is
not especially mentioned in the specifications, drawings or any of the CONTRACT

DOCUMENTS but becomes necessary to ensure the successful implementation of the


PROJECT according to the specifications laid down in the CONTRACT and the intent
thereof, such activity or WORK shall also become part of the CONTRACT as if the same had
been originally included. Such WORK to be provided pursuant to this Section shall not be
construed to be in accordance with Article 28 (Rectification of Defects and Modifications to
the Work) and Article 29 (Changes) and as such all costs and expenses incidental thereto
shall be included in the CONTRACT PRICE.
Monitoring and Control:
1) Planning, scheduling, cost control, project control services and progress reporting
for the whole of the WORK. Provision and implementation of detailed plans, work
time schedules and procedures for Procurement, Construction and Completion for
all GOODS at vendors’ workshops as well as at the WORKSITE. Implementing a
system of progress reporting throughout the CONTRACT
2) To construct and install the WORK in accordance to Approved for Construction
drawings, specifications, and work procedures.
3) Preparation and submission of all Procurement, Construction and Completion,
Quality Control documentation for PCIHBV review and comment.
4) Performing all material tendering, evaluation of bids and issuance of purchase orders
for all GOODS and ensuring the safe and timely delivery of the GOODS to the
WORKSITE in a timely manner, per project schedule requirements.
Governance:
1) The CONTRACTOR shall perform formal Design and Constructability Reviews prior
to commencing Construction activities;
2) Preparation, submission for approval and implementation of Quality Assurance plan
suitable to ensure compliance with CONTRACT specifications and ISO 9000 series.
3) Maintain the WORKSITE as per regulatory requirements and undertake
environmental monitoring and audit to ensure compliance to the conditions imposed
by regulation authority and approved Environment Management Plan;
4) The CONTRACTOR is required to ensure that all logistical approvals and permits
are obtained in a timely manner, in respect of CONTRACTOR PERSONNEL,
CONTRACTOR EQUIPMENT and the GOODS;
5) The CONTRACTOR shall ensure that all relevant approvals are obtained for the
WORK and all activities thereunder. The CONTRACTOR shall forward copies of all
such approvals to PCIHBV.
6) CONTRACTOR shall organize, coordinate and participate in PMT design review
exercises which include but not limited to, line department review (LDR), PROJECT
independent review (PIR), PROJECT risk assessment (PRA), and other third party
review initiated by PMT. All finding and recommendation from the exercise above
shall be incorporated by CONTRACTOR at no cost to PCIHBV to meet PROJECT
requirements
7) Regulatory Authorities requirements in regard the Facilities Handover procedures.
8) CONTRACTOR shall have key personnel as below:
a) Project Manager: A candidate possess bachelor degree in engineering and
has minimum five (5) years’ experience as Project Manager in construction
works (civil/Structural, Building, Infrastructures, Roads, Bridges, Mechanical,
Electrical and Pipeline).

b) Engineering Manager: A candidate possess bachelor degree in engineering


and has minimum five (5) years’ experience as Engineering Manager in
construction works (civil/Structural, Building, Infrastructures, Roads, Bridges,
Mechanical, Electrical and Pipeline).

c) Civil Engineer: A candidate possesses Bachelor degree in civil engineering


and has minimum five (5) years’ experience in civil works.

d) Surveying Engineer: A candidate shall have minimum five (5) years’


experience in Construction works (Civil/Structural, Building, Infrastructures,
Roads and Bridges) as Surveying Engineer, and possess minimum Bachelor
Degree in Engineering.

e) Mechanical and Piping Engineer: A candidate shall have minimum five (5)
years’ experience in Construction works (Civil/Structural, Building,
Infrastructures, Roads and Bridges) as Mechanical and Piping Engineer, and
possess minimum Bachelor Degree in Engineering.

f) Electrical Engineer: A candidate shall have minimum five (5) years’


experience in Construction works (Civil/Structural, Building, Infrastructures,
Roads and Bridges) as Electrical Engineer, and possess minimum Bachelor
Degree in Engineering.

g) HSE Officer: A candidate shall have minimum five (5) years’ experience as
HSE Officer and possess minimum Diploma in any background.

3.3 PMT RIGHTS TO STOP WORK ORDER


PMT Representatives shall have rights to prohibit or stop the commencement of any
WORK at any time if the work is carry out without approve procedure and if equipment,
personnel or work conditions are considered to be unsafe or not in compliance with
contractual rules and regulations.

Any PROJECT SCHEDULE delays and/ or the cost of such remedial measures shall be
borne by the CONTRACTOR and shall not be used as reasons for extension of
PROJECT SCHEDULE.

4.0 LOCAL INFORMATION AND LOCAL REQUIREMENTS

4.1 WORKSITE CONDITION


4.1.1 Soil Condition and Topography Survey
CONTRACTOR shall accept the WORKSITE “As is, Where is Basis” and undertake full
responsibility to carry out the following:
1) CONTRACTOR shall perform necessary soil investigation as required and
accordingly observe the existing soil condition or presence of underground
obstructions at CONTRACTOR’s cost.

2) CONTRACTOR shall perform necessary topographical survey as required and


accordingly observe the existing coordinate, elevation, and boundary limit at
CONTRACTOR’s cost.
3) CONTRACTOR shall adhere and comply with all regulation set of by the approving
authorities during the execution of WORK.
4) Accordingly, CONTRACTOR shall perform Site Preparation Work which includes
WORKSITE’s clearance, rough grading, general earthworks, levelling to grade,
opening of roads to WORKSITE, excavation, backfill, etc.
5) The final site levels shall be approved by PMT.

4.1.2 Access Road to WORKSITE


CONTRACTOR shall be responsible to provide access road from main road to the
WORKSITE.

CONTRACTOR shall ensure that surface of access road shall be immediately scraped
clean and washed down to remove silt and other material discharged from the site or
dislodged from vehicle during transportation of Construction material.

Wash bay facilities shall be provided by the CONTRACTOR at exit points of the
WORKSITE to wash clean the vehicle wheels immediately prior to travel on any public
bituminous road.

CONTRACTOR shall carry out transportation of construction material etc. in such a


manner not to cause damage or blockage to the access road, public roads, drains and
culverts, etc. and any such damage or blockage shall be repaired or reinstated to the
satisfaction of PMT at the expense of CONTRACTOR.

4.1.3 Public Roadways


CONTRACTOR shall ensure that all roads, bridges and other transportation
infrastructures are capable of handling transportation vehicles, loads and dimensions
and CONTRACTOR shall adhere to all Iraq Laws and Regulations.

The improvements required to upgrade roads, bridges and other transportation


infrastructures to meet the project requirements shall be the CONTRACTOR’s
responsibility.

When CONTRACTOR’s operations are conflicting with normal traffic, on roads,


highways, or other traffic ways, CONTRACTOR shall provide suitable detours around
Construction activities for such traffic. Continuous movement of traffic shall be
maintained at all time.

CONTRACTOR shall provide flag persons and safety supervisor(s) and shall supply,
install and maintain all temporary signs and other devices necessary for warning
oncoming traffic of construction activities. Such signs and devices and placement thereof
shall conform to those specified by the authorities having jurisdiction over such matters.

4.1.4 Historical Resources


In the event of encountering archaeological or paleontological resources,
CONTRACTOR shall suspend work on that portion of the WORK and shall notify PMT
immediately upon discovery. PMT, in accordance with regulatory requirements, will direct
appropriate personnel as to the disposition of the artefact that shall become the property
of the government.

4.1.5 CONTRACTOR’s Equipment


CONTRACTOR shall be responsible for supplying and maintaining all equipment, tools,
consumables and all other resources necessary to execute the WORK. CONTRACTOR
shall obtain all import licenses and other permits required for the importation into and use
in Iraq of all equipment and other Construction items to be supplied and made available
from sources outside Iraq. All such items shall be imported by CONTRACTOR in the
name of CONTRACTOR.

At the end of Construction activities, CONTRACTOR shall offer the sale of their
equipment and surplus materials to PMT and if requested by PMT, shall enter into such
negotiations.

4.2 LOCAL REQUIREMENTS


4.2.1 Language
English language shall be used for all documents and for communication between parties
in connection with the WORK.

All drawings, calculation sheets and documents shall be written in English except where
governmental authorities demand the Arabic language for their approval.

4.2.2 Laws, Acts and Regulations


It is the responsibility of CONTRACTOR to check which Iraq Laws and Regulations are
applicable and to ensure that all legal obligations, applicable for the realisation of the
WORK are met.

CONTRACTOR shall comply with applicable laws and regulations in carrying out the
WORK, particularly in respect to:
1) Importation,
2) Taxation,
3) Royalties,
4) Recruitment of labour,
5) Temporary employment,
6) Restrictions for foreign and non-local labour entry,
7) Employee relations,
8) Environmental matters,
9) Registration of personnel,
10) Housing,
11) Testing/ inspection.

4.2.3 Utilities for Construction


CONTRACTOR shall provide all utilities for Construction and be responsible for all
installation and removal costs, including any costs levied by public utilities themselves.

4.2.4 Construction Facilities


4.2.4.1 Temporary Facilities and Restrictions
The areas to be allocated for Construction facilities shall be discussed immediately after
the start of WORK. The usage to which these areas will be put by CONTRACTOR, and
the related procedures, will be subject to agreement between PMT and CONTRACTOR
before the start of construction activities.

CONTRACTOR shall provide premises to accommodate its own and Sub-Contractors


requirements, such as site offices, stores, etc. It will be the specific responsibility of
CONTRACTOR to set up, maintain, including housekeeping, and dismantle these
facilities.

Rigid procedures shall be in place and enforced for all parties on the WORKSITE,
comprising a permit system covering the access to the WORKSITE and the erection,
occupation and maintenance of all temporary facilities on the WORKSITE.

4.2.4.2 Disposal of Excess Soil


CONTRACTOR shall make all necessary arrangements at CONTRACTOR own cost in
order to dispose of excess soil and importation of clean fill material in a manner
acceptable to the Iraqi’s Governance Authorities. This includes all associated costs of
disposal, including royalties, if any.

4.2.4.3 Disposal of Permanent Waste/ Scheduled Waste


CONTRACTOR shall be fully responsible for and shall make all arrangements for the
proper disposal of permanent/ scheduled waste, litter and garbage in a manner
acceptable to the Iraqi’s Governance Authorities as part of WORK at no additional cost
to PMT.

4.2.5 Accommodation at WORKSITE


4.2.5.1 PCIHBV’s and PMT Site Offices
CONTRACTOR shall provide offices, utilities and all facilities associated with the site
offices for PCIHBV’s and PMT use during the execution of the WORK as specified in
Exhibit I

V –Administrative Procedures. (if require)

4.2.5.2 Accommodation for Construction Workers


Camp accommodation arrangements may be organised by the Sub-Contractors
concerned but CONTRACTOR remains responsible overall for the orderly
accommodation of construction workers and the social effects of its labour force.

CONTRACTOR is to ensure, particularly in regard to the setting up of any Construction


camps for Construction workers, that adequate sanitary, catering, transport and
recreation facilities are available to the labour force.

4.2.6 Work Procedure


Work procedure is a formal written statement used to control certain types of works,
which are identified as potentially hazardous. The procedure must be signed by a Qala’at
Suker Water Department or relevant Regulatory Authorities person, provides that the
holder can carry out the prescribed works, in the manner stated.

CONTRACTOR shall include work permit procedure in the Construction Plan, which will
be reviewed and approved by Qala’at Suker Water Departmentor relevant Regulatory
Authorities, prior to commencement of construction work.

4.2.6.1 Liaison with Authorities


CONTRACTOR shall comply with all the applicable regulations, codes and laws of Iraq
Governments and other duly constituted regulatory authorities having jurisdiction.

Activities requiring CONTRACTOR’s liaison with authorities will include, but not be
limited to transporting large or heavy equipment on public roads, crossing of
transportation ways, underground facilities and above and below ground utilities that may
be encountered in the course of executing the WORK.

Liaison with authorities will also be required for the use of bridges and culverts, and
residential roads, to determine the allowable loads on these facilities.

4.2.6.2 Liaison with Landowners


CONTRACTOR shall give due consideration to the interest and property of landowners
wherever involved, and shall carry out and perform the WORK in a manner, which will
result in minimum inconvenience to the landowner and minimum damage to the land.

CONTRACTOR shall resolve all landowner concerns in an expeditious manner to the


satisfaction of the landowners with a copy to PMT of agreement made between
CONTRACTOR and the landowners.

4.2.6.3 Liaison/ Interfacing with Other Contractors


During the course of construction, CONTRACTOR may encounter other PMT’s assigned
contractors engaged in other phases of the work, particularly in and around the WORK
area. Prior to commencing construction in such common areas CONTRACTOR shall, in
conjunction with the other contractors, develop procedures for coordination of
construction activities in such common areas so as to minimise inconvenience to all
parties.

CONTRACTOR shall co-operate fully with other contractors employed by PMT and shall
afford reasonable access and facility for the execution of their work. CONTRACTOR shall
ensure that any part of the WORKSITE, which is to be shared with other contractors, is
cleaned up and left in reasonable condition for use by others.

5.0 PROJECT MILESTONES

The PROJECT SCHEDULE and Milestones are detailed in Exhibit II –Schedule of


Compensation.
CONTRACTOR shall mobilise resources and effectively start the WORK within one (1) month
from the CONTRACT Award.
The Total Duration for the EPCC works is Twelve (12) Months.

6.0 MANAGEMENT AND PROJECT CONTROLS


6.1 GENERAL
CONTRACTOR shall ensure that all parties involved in the WORK are properly coordinated and
that all interfaces are dealt with in a prompt manner. CONTRACTOR shall initiate and implement
appropriate Project Management and Project Control and Administration Procedures. Provision
and implementation of the procedures for the WORK shall be as per the guidelines provided in
this Section.

6.2 SCOPE OF PROJECT MANAGEMENT


CONTRACTOR’S Scope of Project Management shall include the followings as a minimum:

1. Project management and supervision of all SUBCONTRACTORS, vendors, and ancillary or


temporary services which are necessary for the expeditious completion of the WORK.
2. Provision of a project management team or teams to manage the implementation of the
WORK.
3. Provision of the services of all CONTRACTOR personnel as required for execution of
WORK including during Warranty Period.
4. Co-ordination and control of each work group, e.g. engineering, procurement services,
project control, contracting and construction/installation to ensure the WORK is completed
within the stipulated completion dates and according to the specifications and drawings
within the CONTRACT.
5. Implementation of a Safety program throughout the execution of the WORK.
6. Implementation of a Quality Assurance program throughout the execution of the WORK and
submission of periodic reports to PMT.
7. Carrying out periodic quality, safety, technical and schedule audits with PMT attendance
and advising PMT of the results of such audits together with the appropriate action taken
and any recommendations.
8. Management and co-ordination of all external interfaces associated with the WORK,
covering detailed design and construction activities.
9. Attending all management meetings with PMT at CONTRACTOR’s home office, SITE,
SUBCONTRACTORS, vendors and PMT offices in Iraq or as designated in accordance with
CONTRACT.

10. Reporting to PMT in accordance with the guidelines provided in CONTRACT.


11. Provision of continuous services of CONTRACTOR personnel at Site during WARRANTY
PERIOD.
6.3 CONSTRUCTION AND WORK MANAGEMENT
CONTRACTOR shall provide all Engineering, Construction and work management,
Commissioning, supervision, and support services for the WORK. This shall include:
1. Engineering management for Details engineering and commissioning works
2. Construction Management and Interface Management
3. Construction Supervision.
4. Maintenance Work Management
5. Commissioning works
6. HSE Management.
7. Quality/Quality Assurance/Control.
8. Construction Engineering and technical control services.
9. Material control and warehousing.
10. SUBCONTRACTOR Management.
11. Certification Services.
12. Inspection Services.
13. Disposal of construction surplus materials.
14. Dismantling and disposal of construction aids and other temporary facilities, except
otherwise directed by PMT.
15. Liaison with PMT for obtaining of Permits to work, etc.

6.4 PROJECT CONTROLS


6.4.1 General
In order to ensure that the project implementation will be carried out in a professional
and efficient manner, CONTRACTOR shall formulate and implement a total project
control system.
A 30 days look-ahead project schedule is to be submitted within 2 weeks from the
effective date of contract. The schedule is to outline in detail the engineering and
procurement activities emphasizing the critical milestone to be achieved in meeting the
completion date.
Within 4 weeks from the effective date, the CONTRACTOR shall submit a Project work
schedule to PMT for review and approval. No change shall be made to any activity
without prior consultation with PMT.
The primary objective of this Project work schedule is to provide timely accurate
information needed by the project team, project management, and PMT to identify areas
of concern and opportunity and to initiate appropriate action for the successful execution
of the work. The Project work schedule shall be as a minimum level 2 activities.

6.4.2 Project Execution Plan


The project execution schedule will include the detailed schedules at different levels.
The schedules will be computerized in such a way that planned and actual progress can
be aggregated and automatically loaded from the lower level (Level 3 /4) to the higher
level (Level 1).
The project execution plan will be based on detailed breakdown of the Scope of WORK
into basic measurable activities on the basis of Work Breakdown Structure (WBS).
Breakdown structure will reflect various packages and units envisaged in the scope of
WORK. A common activity coding structure will be followed throughout project duration
for the planning and progress reporting infrastructures.
Associated with each activity at the different levels will be three sets of dates: “planned,
“actual” and “forecast”. The “planned” dated will remain fixed through the whole
CONTRACT period with clear indication on the schedule on the Early and Late Plan
The schedules will at any time reflect the total Scope of WORK, duration and the logic
for the different phases is:
a) Preliminaries
b) Engineering
c) Procurement
d) Construction
e) Commissioning
The project schedule will be updated on weekly basis and at any time requested by PMT.
The updated project schedule will indicate the planned and actual progress value to date
for each activity, remaining duration value and a revised forecast completion date.

6.4.3 Level 1 - Project Master Schedule


The Project Master Schedule (Level 1) will be derived from the activity network analysis.
The Project Master Schedule will contain a summary of activities contained in Level 2
and Level 3 / 4 Detailed CONTRACT Schedule; clearly indicating the dates set out in the
schedules and will contain all key milestones.
The Project Master Schedule will contain a minimum of 20 activities or as necessary to
clearly indicate the method of execution for the total work, covering the whole
CONTRACT period.
Level-1 Project Master Schedule will cover for each package the activities related to the
implementation of the following phases:
1. Engineering
2. Procurement
3. Construction / installation
4. Commissioning
5. This schedule will indicate the following:
• Mobilization Dates
• Discipline-wise key activities

6.4.4 Reporting
6.4.4.1 General
For the entire duration of the WORK, an accurate and timely reporting covering
engineering, procurement, construction and commissioning activities will be provided by
the CONTRACTOR.
The report will contain details on all aspects of the project over the reporting period and
give an up-to-date and clear picture of the progress and overall status of the work.
The reports will highlight all trends that impact the schedule and / or the CONTRACT
PRICE and will include detailed information of the progress of work, variations with
explanation for slippage in schedule and the recovery plan as may be required.
The progress curves and histograms will indicate the planned cumulative percentage
progress and the actual cumulative percentage progress. Manpower histograms shall
be categorized by resource trade grouping or phase of work.
During Material Procurement phase, Two-Month Look Ahead bar chart covering activities
related to engineering and procurement will be extracted (from the Level 3 / 4 detailed
project schedule), updated and issued on a fortnightly basis. Furthermore, six (6) weeks
look-ahead bar chart covering constructed related activities only will be extracted from
detailed project schedule, updated and issued on a fortnightly basis.

6.4.4.2 Daily Construction Report (Log Book for Job Site Work)
Daily Reports will be submitted only during the Construction phase. A daily construction
report will be submitted before 10:00 a.m. on the following day. This report will set out
the following items.
1. The number direct manpower present at site by trade
2. The number of supervisory personnel (indirect manpower)
3. The weather
4. Details of construction equipment available and used
5. The progress quantities achieved for the day with the target to achieve on the
reporting day inclusive of S-Curve actual and planned percentage
6. Chronological recording of main activities
7. Checks and tests carried out
8. Safety Report
9. Photo progress showing the daily activities progress of works

6.4.4.3 Weekly Progress Report

A weekly progress report will be submitted two (2) days after the weekly cut-off date of
Thursday. The weekly status will, as a minimum, include:
a) Overall CONTRACT Status (Narrative and Point Forms)
- Procurement - Brief description of tasks executed.
- Construction / Erection.
- Areas of concern and mitigation measures
- Constraints.
- Permits
- Forecast for next 2 weeks.
- Recovery actions (when project activities reported slippage)
b) QA / QC status
c) Construction status

6.4.4 Change Order


Any change to the original CONTRACT scope will be handled in accordance with the
change order procedure included under Exhibit IV Administrative Procedure.
Changes to the WORK shall comply with the Contract Form.
CHANGE ORDER progress will be measured and monitored outside the CONTRACT
Progress Measurement System, and CONTRACT ‘S’ curve in order to maintain a
constant percentage weighting for the

6.4.5 Invoicing
Invoices shall be submitted in accordance with CONTRACT Form. CONTRACTOR’s
invoices shall be consistent and reflect the Payment Schedule indicated therein for Lump
Sum WORK, or as agreed otherwise for work relating to CHANGE ORDERS.

6.4.6 Progress Review


Weekly progress review meetings will be held on every Monday for monitoring project
progress and implementation at Project Activities Center. The weekly meeting will be
held at CONTRACTOR’s Office at SITE. Agenda for the meetings will be circulated in
advance. Minutes of Meeting shall be written and recorded on the day of meeting by
CONTRACTOR and promptly submitted to PMT for review and approval within three (3)
days.

6.4.7 Monthly Management Progress Review Meeting


The Monthly Management meeting shall be held for CONTRACTOR to brief and present

shall be attended by the CONTRACTOR’s and PMT’s Management. The meeting will be
held at CONTRACTOR’s Site office in Iraq during Construction.

7.0 Services AND Facilities to PMT Personnel


7.1 GENERAL
CONTRACTOR’s scope of WORK shall accommodate the presence of PMT
representatives, and any third party nominated by PMT for such purposes as
CONTRACT co-ordination, monitoring and all other aspects of the WORK. Unless herein
indicated, cost of these services and facilities are included in the CONTRACT price.

8.0 QUALITY ASSURANCE AND QUALITY CONTROL


8.1 GENERAL
CONTRACTOR shall be responsible to plan, establish, implement and maintain a Quality
System for the procurement, fabrication, installation as per the CONTRACT in line with
ISO 9001 and ISO 9002.
CONTRACTOR shall submit a copy of CONTRACTOR’s corporate quality manual and
procedures including policy statement, as a part of their bid, for review by PMT.
CONTRACTOR shall be responsible for all Quality Assurance and Quality Control
functions and shall at all times provide dedicated, competent and qualified supervision
and inspection personnel, approved by PMT, to ensure that the quality of work is met
and timely inspected.
CONTRACTOR shall supply all the relevant quality assurance requirements to its
SUBCONTRACTORS, suppliers etc. involved in the WORKS for compliance and shall
be responsible for Overall Project Quality Assurance and Quality Control.

8.2 Project Quality Plan


CONTRACTOR shall prepare a specific Project Quality Plan for the CONTRACT,
detailing all quality aspects as defined above and submit to PMT for approval within 4
weeks’ from the Effective Date of CONTRACT. The Quality Plan shall cover all aspects
of on and off site inspections, inspection request forms, non-conformance reports,
remedial actions, records, scheduling, etc.
CONTRACTOR shall effectively implement the quality system defined in the plan, verify
execution of the implemented system and issue reports of audits performed. Any non-
conformance will be handled in line with the requirements of the Quality Plan. Any
deviations / non-conformance that require PMT approval will be submitted to PMT with
adequate back-ups justifying the acceptability of the deviations / non-conformance.
CONTRACTOR shall be responsible to extend these requirements to its subcontractors
and suppliers.

8.3 Quality Control


PMT is committed to the protection of the environment, and the safety of personnel
working on this Project as well as others who may be affected by WORK activities.

It is required that, the CONTRACTOR also fulfils this commitment and in so doing
provides for the protection of people, property and the environment. CONTRACTOR will,
in advance, endeavour to eliminate any foreseeable hazards, which could result in
accidental loss, including personnel injury, fire, and security loss and property damage.
CONTRACTOR shall ensure that all of its personnel including Suppliers, SUB-
CONTRACTORS and third-party employees to attend all the required HSE’s training
prior to entering the WORKSITE and performing any WORK.
It is mandatory for CONTRACTOR workforce to be fully attired with necessary HSE
gears, such as coverall, hard-hats, safety goggles/ glasses, safety boots, safety belts etc
when they are at the WORKSITE.
CONTRACTOR shall comply with the procedures and requirements defined in Exhibit
VII – Health, Safety and Environment (HSE) Requirement. CONTRACTOR shall
develop and provide all documents necessary for the safe successful completion of the
WORK.

9.0 HEALTH, SAFETY AND ENVIRONMENT (HSE)


9.1 POLICY ON SAFETY, HEALTH AND ENVIRONMENT
9.1.1 Policy Statement and Objectives
CONTRACTOR shall conduct its operations in such a manner as to:
1. Provide a safe working environment.
2. Ensure the safety and health of CONTRACTOR’s crew and personnel working
within the CONTRACTOR designated areas of operation.
3. Protect the public from injury or ill health and prevent loss or damage to
properties resulting from its activities.
4. Ensure and safeguard the conservation of the environment.
9.1.2 Safety Targets
In taking steps to ensure a safe working environment, CONTRACTOR shall aim for:
1. No fatalities.
2. No loss time and any significant accidents;
3. No roll over to vehicles.

9.1.3 Implementation Aspects


The policy is implemented with special attention to the following specific aspects:
1. The requirements of all relevant government legislation are followed;
2. Local Government Authorities standards, specifications, procedures and
regulations are applied;
3. HSE is given equal importance to productivity and cost;
4. Each employee is given specific procedures related to his work.
5. Each employee receives suitable technical and safety training;
6. Work instructions are clear and pay due regard to HSE requirements;

7. Experience gained, lessons learned from accidents/incidents and new technical


developments to be widely distributed amongst staff.
8. Work Sites, work area are designed, built and operated in such a way that work
can be carried out safely and in an environmentally sound manner.
9. Only materials, tools and equipment which meet high standards are used.
10. The safety aspects of Work Sites, work area, materials and tools are reviewed
continually.
11. CONTRACTOR is required to adopt and maintain the same high standards as
per PMT expectation.
12. All work carried out, whether by the CONTRACTOR or its Subcontractors, is
effectively monitored by CONTRACTOR.
13. Regular HSE meetings to be held at all levels in the organization to ensure HSE
occupies important aspect of work planning and execution.
14. Contractor shall provide adequate Safety Signboard and Warning Sign at Site to
prevent accidents and competence with Safety Regulation.

9.2 RESPONSIBILITY
9.2.1 HSE Manager and Officers
The Contractor shall have at all times a fulltime responsible person appointed as the
crew's HSE Manager and HSE Officers. The HSE Manager and HSE Officers shall
manage and oversee all matters pertaining to HSE in all crews operation and shall;
1. Conduct fortnightly HSE meetings with all his crews.
2. Follow-up HSE items raised during HSE meetings.
3. Ensure accident/incident reports are completed and forwarded to the PMT
Representative within twenty four (24) hours.
4. Set up a system to enhance the HSE attitudes and awareness of all his crews.
5. Participate in HSE program or meeting conducted by PMT.
6. Arrange or participate in the accident/ incident investigation.
The number of HSE Officer(s) shall be sufficient and meet the international as well as
the local regulatory requirement of Occupational Safety and Health.

9.2.2 All Personnel


It is the responsibility of every personnel to maintain a safe working environment, both at
his assigned work place and in other parts of the survey area.
Inappropriate conduct or mischievous acts shall not be allowed, as this presents a safety
hazard to the entire crew. Subject to regulation enforced, firearms, weapons, prohibited
drugs or alcohol shall not be allowed at the camp or work place.
All CONTRACTOR Personnel are to undergo an annual medical check-up at
CONTRACTOR expense, to certify their fitness for duties in harsh environment. Valid
medical certificates are to be kept together with the personnel records for inspection
purposes.

9.3 TRAINING REQUIREMENTS


First Aid, Resuscitation and Fire-Fighting
1. There must be an adequately trained first aider at work location.
2. All personnel must be trained to operate fire-fighting equipment at their own
workstations.
3. Confined Space Entry

Operation of Geological Equipment / Machinery


1. Formal training must be provided for operators of Geological equipment /
machinery
2. Trainee shall not be left to work unsupervised.

9.4 HSE MEETINGS AND AUDITS


9.4.1 HSE Inspections/Audits
Prior to WORKS, CONTRACTOR’s equipment shall be inspected by PMT
Representative(s), satisfied for operation and must meet all PMT’s HSE specifications
and regulations.
Subsequent inspections will be made to ensure that proper actions have
been taken to rectify earlier identified unsafe situations and that equipment
is in working order.

9.4.2 HSE Awareness / Meetings


CONTRACTOR must be responsible for maintaining and enhancing the HSE awareness
of its personnel and subcontractor personnel, including arranging and / or participating
in regular HSE meetings/briefing and emergency drills.

The objectives of HSE meetings are to:


1. Provide opportunities for personnel to voice their concern over unsafe situations
or procedures in their respective work places.
2. Provide information and warning for other personnel in regard to potential or
existing hazards.
3. Allow collective solutions to be put forward through discussion.

A Monthly HSE Meeting shall be conducted accordingly with full participation of


CONTRACTOR’s Management and key personnel.

9.4.3 Frequency

The meeting shall be held fortnightly and include an HSE talk or presentation on a chosen
subject aimed at enhancing HSE awareness on site.
HSE audits shall be conducted by the Party Chief / HSE Officer in conjunction with the
PMT Representative on a monthly basis or whenever deemed necessary. Tool Box
Meeting shall be conducted on daily basis by authorized HSE Officer.

9.5 REPORTING
9.5.1 All HSE meetings are to be minuted and forwarded to PMT Representative.
9.5.2 All emergency drills are also to be forwarded to the PMT Representative.
9.5.3 All HSE audits are to be reported to PMT Representative monthly, with action points
listed.
9.5.4 All accidents and incidents related to the survey shall be reported in accordance with
PMT Accident Reporting Procedure, within twenty four (24) hours in the event of:
1. Any loss of or damage to material or equipment supplied by either the PMT or
CONTRACTOR.
2. Any personal injury to any PMT or CONTRACTOR Personnel, its agents or
subcontractors.
3. Any injury to any third party.
4. A near miss incident.
9.5.5 A full detailed report via telex or fax or email sent to PMT within twenty four (24) hours
and an Accident or Incident Report filled out by the CONTRACTOR Contract
Administrator immediately.

9.6 SAFETY TOOLS AND EQUIPMENT


9.6.1 Use of correct, properly designed and serviceable tools and safety equipment is required.
All working personnel should be taught the proper and correct way of using safety tools
and equipment.

9.6.2 Fire-Fighting Equipment


1. All fire extinguishers are to be checked and certified semi-annually.
2. Fire extinguishers are to be prominently marked.
3. Alarm system to be tested during every drill.
4. Fire accesses are to be strategically located.

9.6.3 First Aid and Survival Equipment


1. Adequate number of first-aid boxes and resuscitation units are to be placed at
strategic points.
2. First aid boxes are to be inspected regularly and stocks replenished.
3. Prescription drugs are to be certified by Pharmacist and kept under lock. Drugs can
only be administered with the consent of the Party Chief or HSE Officer.

9.6.4 Personnel Protective Equipment


1. All Personnel Protective Equipment (PPE) shall be of types manufactured to
standards and approved by PMT.
2. Protective equipment shall be worn at all times at the WORK areas.
3. CONTRACTOR is responsible to supply PPE to all CONTRACTOR’s Personnel.

9.7 HOUSEKEEPING
9.7.1 CONTRACTOR shall ensure good housekeeping hygiene at the Work areas and Camp.
9.7.2 Washrooms and toilets shall be serviced regularly.

9.8 EMERGENCY EQUIPMENT AND PROCEDURES


9.8.1 Emergency Procedures
CONTRACTOR shall have in place an Emergency Response Procedure (ERP)
describing in detail the communication system, site emergency response operation,
duties and responsibilities of personnel and action to be taken in the event of an
emergency.
The CONTRACTOR ERP, which is to be consistent with the PMT ERP, shall be reviewed
and approved by the PMT prior to the commencement of the WORKS.

9.8.2 Emergency Response


CONTRACTOR shall where applicable at its own expense provide adequate first aid,
firefighting, lifesaving and other safety equipment and shall maintain this equipment in a
professional manner and where appropriate re-certify as dictated by legal and industry
standards. CONTRACTOR shall keep up-to-date records of all said equipment, including
equipment location plans.

9.9 ACCIDENT REPORTING AND INVESTIGATION


9.9.1 Accident is defined as any unintentional or unplanned event or condition, which has or
could have resulted in injury to a person and loss or damage to equipment, plant or
property.
9.9.2 It is PMT requirement that all accident, no matter how trivial, must be reported to PMT
Representative. CONTRACTOR shall ensure that its employees are aware of this
mandatory requirement.
9.9.3 CONTRACTOR shall be responsible to investigate, in a professional manner, all
accidents that occur during the performance of the WORK and the investigation report
shall be made available to PMT when requested. CONTRACTOR shall also be
responsible to assist PMT in accident investigation if so required. PMT may call for a
joint investigation with CONTRACTOR if necessary.
9.9.4 CONTRACTOR shall where applicable have, prior to commencement of CONTRACT,
accident reporting and investigation procedures and shall maintain accident statistics,
which shall be compatible with PMT Accident Reporting Procedures. Otherwise,
CONTRACTOR shall adopt the current PMT Accident Reporting Procedures.
9.9.5 CONTRACTOR shall submit the basic HSE information to the appropriate PMT
Representative not later than the first day of the month following the month under review,
by telex or fax or email.

9.10 ALCOHOL/ DRUG POLICY


9.10.1 CONTRACTOR warrants that its personnel, agents and subcontractors shall not perform
any WORK for PMT while under the influence of alcohol or any controlled substance
CONTRACTOR. CONTRACTOR personnel, agents and Subcontractors shall not
misuse legitimate drugs or possess, use, distribute, or sell illicit or unprescribed
controlled substances or drug on PMT business or premises. CONTRACTOR shall
adopt and enforce work rules and policies in order to assure compliance with these
obligations.
9.10.2 CONTRACTOR is to conduct alcohol and/or drug tests on CONTRACTOR personnel,
agents or Subcontractors while on premises owned or controlled by PMT where
reasonable cause exists.
9.10.3 PMT also reserves the right to conduct searches on possession of drug and/or alcohol
to the person, vehicles, and other property of CONTRACTOR, its personnel, agents or
Subcontractors while on premises owned or controlled by PMT. Any person who refuses
to cooperate with any such search shall be removed from the premises and not permitted
to return.
9.10.4 CONTRACTOR shall require its personnel, agents and Subcontractor to submit to
medical evaluation on alcohol or drug testing where cause exists to suspect alcohol or
drug use.
9.10.5 CONTRACTOR warrants that any of its personnel, agent or Subcontractor who either (a)
refuses to participate in medical evaluation or alcohol or drug tests, or (b) tests positive
for alcohol or controlled substance, shall be removed from the premises and not be
permitted to perform any work with PMT.
9.10.6 CONTRACTOR shall maintain strict discipline and good order among its personnel,
agents and Subcontractors, and shall not permit any of them to engage in activities which
PMT deems contrary or detrimental to PMT interests. If PMT should request that any
personnel of CONTRACTOR or of Subcontractors be removed from PMT property or

Work site pursuant to this CONTRACT for any reason CONTRACTOR shall accede to
such request and shall provide a replacement acceptable to PMT at no additional cost to
PMT.
9.10.7 In the event CONTRACTOR is unable to comply with these obligations, PMT shall have
the right to terminate this CONTRACT.

9.11 MEDICAL WELFARE


9.11.1 CONTRACTOR shall ensure that all its personnel and/or other personnel assigned by
the CONTRACTOR for the performance of the WORK are medically fit and healthy. Any
medical disabilities including such disabilities which CONTRACTOR may consider will
not adversely influence the person's ability to perform his role in the WORK should be
reported to PMT prior to the start of the WORK. CONTRACTOR, if requested by PMT,
shall provide medical certificates for CONTRACTOR and Subcontractor personnel.
9.11.2 CONTRACTOR shall subject its key personnel and its Subcontractor personnel to regular
medical examination at their cost. Records of such examination shall be made available
to PMT on request.
9.11.3 CONTRACTOR shall at no cost to PMT be responsible for the medical welfare of its own
and Subcontractor personnel and shall take care of arrangements for medical
attendance treatment or hospitalization if and when necessary and will arrange suitable
insurance coverage for such contingencies. In cases of emergency, PMT may make or
provide for, the necessary emergency arrangements, the costs of which shall be
reimbursed to PMT by CONTRACTOR.
9.11.4 CONTRACTOR shall make first aid arrangements for all of its personnel and ensure that
all personnel are informed of such arrangements.
9.11.5 Where applicable, CONTRACTOR shall provide a suitably equipped and staffed first aid
room with medical supply and assigned Ambulance Vehicle if the Work Site presents a
high risk from hazards.

9.12 REPORT
CONTRACTOR is responsible to document its performance in HSE for the duration of
the contract and submit to PMT on monthly basis and overall report at the end of contract
or when requested by PMT.

10.0 DESIGN AND ENGINEERING


10.1 DESIGN REQUIREMENTS
10.1.1 General
CONTRACTOR shall perform Engineering services necessary for WORK as specified in
the CONTRACT.
CONTRACTOR shall be responsible for verifying and completing the WORK based on
information contained in the subsequent section.
The Engineering design Scope of Work shall include, the following:
1. CONTRACT shall review, verify, validate and/or rectify the FEED deliverables and
update the documents as part of Detailed Design works.

2. The FEED Documents in the CONTRACT were prepared only with sufficient
information to define the PROJECT technical scope and shall be understood to
reflect preliminary design of the FACILITIES only. Thus, these documents may not
include all information required to complete detailed design documents, or all
information that would normally be on a document after detailed design has been
completed. As such, CONTRACTOR is responsible for the full engineering and
detailed design to meet all PROJECT specifications and PMT standards.
3. Detailed engineering design for the PROJECT shall be in full and complete detail
to provide a design of the WORK that meets the CONTRACT requirements,
including the preparation of all additional technical documents to fully define the
WORK or as necessary to fulfil the PROJECT Scope of Work. Engineering and
design activities shall be performed by CONTRACTOR to support all phases of
the PROJECT, including, but not limited to design document reviews, detailed
engineering design, procurement, fabrication, construction / installation, testing,
inspection, pre-commissioning and commissioning.
4. The Engineering Team shall be available and mobilized to the designated
Engineering Office one (1) month after contract award.
5. CONTRACTOR shall submit Engineering deliverables list, Project Execution and
Quality Plans for Engineering to PMT within four (4) weeks after CONTRACT
award for PMT’s APPROVAL. Engineering deliverables list should be defined from
the FEED deliverables. However, PMT has the right to add the Engineering
deliverables list if it is deemed required to meet CONTRACT requirements.
6. This includes all design analysis, design models, drawings, design reports,
specifications and datasheets, material take-off Vendor Data incorporation,
procedures, operating and maintenance manuals and final documentation
package as appropriate
7. CONTRACTOR shall be responsible for the design, completeness, quality and
conformance to requirements of the WORKS. Any errors or omissions on the part
of the CONTRACTOR shall be corrected at no extra cost to PMT and
CONTRACTOR will not be relieved of its responsibilities until such time when the
project deliverables have been satisfactorily completed and fully handed over to
PMTD.
8. Any additional information, analyses, data, drawings, specifications and
datasheets not included in the FEED documents and are necessary to complete
the WORKS shall be prepared and provided by CONTRACTOR as part of this
Scope of WORKS.
9. Prepare, update and issue Technical Document Register (TDR) on a weekly basis;
10. Preparation of technical specifications, detail calculations, Schedules, drawings,
procedures, method statements, required for the Engineering, Procurement and
Construction.
11. All studies, analysis, calculations, which are required as part of the design;
12. Any studies for optimisation shall be performed using established computation
techniques and programs such as STAAD PRO,FINITE ELEMENT etc.
13. All necessary site surveys and field subsurface verifications shall be established,
and all expenses shall be borne by the CONTRACTOR.
14. CONTRACTOR shall submit comprehensive reports of the above to PMT;
15. Submission of specifications, drawings, method statements, schedules,
calculations, test results etc. to PMT for review and approval.
16. All documents, drawings etc. which require PMT approval shall be identified by
PMT during approval of Technical Document Register (TDR);

17. CONTRACTOR shall review documents for compliance with QA/QC and HSE
requirements.
18. CONTRACTOR shall provide Engineering support to the Sub-Contractors as
required in the interpretation of drawings;
19. CONTRACTOR shall provide Construction Engineering assistance during
Construction, Completion, Warranty and Maintenance stages.
20. CONTRACTOR shall make available all Codes and Standards for PMT reference
during Project execution period.
21. During the WORKS, several reviews shall be held by either by the CONTRACTOR
or an integrated review with PMT personnel on the engineering documents. These
reviews are to ensure the update, close out and completeness of the Engineering
AFC documents following FEED.

10.1.2 Codes and Standards


The WORK shall be design in accordance with applicable International Codes and
Standards.
The most standards shall be applied are as follows: -
1. National or Local codes/ standards
2. Standard Specification for Roads and Bridges (Department of Design and studies,
1983)
3. AASHTO-Geometric Design of Highway and Streets 2004 or latest
4. ASTM-American Society for Testing and Materials (reapproved 1994 or latest)
5. International Industry Codes (API, ANSI, IEC, IP, AISC, ASME, BS, IEE, NFPA ,ISO)
6. Iraq Electrical Procedure and Standard (MOE)

No deviations to the applicable Codes and Standards are allowed and if any
waiver/non-compliance is required where there is a distinct advantage on
schedule, CONTRACTOR shall submit waiver request and its relevant justification for
PMT for APPROVAL.

Should there be any updates on Codes and Standards during the course of the
WORKS that will affect the schedule, CONTRACTOR shall bring to PMT‟s
attention of these changes and impact and obtain agreement as to which version
shall be adopted for the Project and PMT may choose on the most
stringent requirement to be followed.

10.1.3 Drawing/ Document Approval by PMT


CONTRACTOR shall prepare Technical Document Register (TDR) which will include all
technical specifications, drawings, schedules, manuals, calculations, procedures and
other documents planned to be generated/ updated as a part of Detail Engineering.
PMT will return the documents/ drawings within one (1) week, with comments/ approval
to the CONTRACTOR. CONTRACTOR shall seek re-approval from PMT in case of any
further changes with respect to the basic design, philosophy, scheme of PMT approved
deliverables.
Approval of documents/ drawings by PMT does not relieve CONTRACTOR of
Contractual responsibilities and guarantees towards the completeness, correctness of
the Engineering of the Facilities.

Engineering Deliverables
CONTRACTOR’s design and engineering work shall cover all disciplines and includes, but is
not limited to, the preparation of all necessary models, documents, and drawings, such as flow
schemes, data/requisition sheets, specifications, requisitions, instructions and detailed
drawings, piping isometrics, reports, calculations, within the scope of its work as is required:
1. To ensure smooth and uninterrupted execution of the next project phases, i.e.
procurement of materials and equipment, construction, start-up and operation.
2. To fulfil the requirements of the PROJECT SPECIFICATIONS and PMT
/PCIHBV’s Instructions.
3. To fulfil the requirements of local law and statutory authorities for granting of
permits, inspection and safety.
4. To complete the engineering record.
5. To provide PMT/PCIHBV with sufficiently detailed instructions and records of the
FACILITIES to allow safe and efficient operation and maintenance of the
PROJECT.
CONTRACTOR shall perform the detail design which shall reflect as a minimum all requirements
as detailed in the PROJECT SPECIFICATIONS and include but not be limited to the following
design activities:

1. CONTRACTOR shall prepare technical specifications, datasheets, reports,


calculations, material take-offs, equipment list, schedules, drawings,
procedures, method statements, technical and material requisitions and other
deliverables required for the detail engineering, procurement, construction and
COMMISSIONING of the FACILITIES.
2. CONTRACTOR shall perform all studies, analysis and calculations, which are
required as a part of design and for optimisation of equipment sizing and
selection using established and approved computation techniques and
programmes.

3. CONTRACTOR shall perform all necessary engineering surveys including


WORKSITE surveys and field verifications to establish the actual status of
locations and FACILITIES and not limited to CUT and FILL of the area.
4. CONTRACTOR shall conduct any geological, soils or geotechnical
investigations in full or supplementary, as CONTRACTOR deems necessary to
perform the WORK.
5. CONTRACTOR shall consider HSES issues relevant to the construction and
operation of the FACILITIES and incorporate all provisions necessary for
construction and operational HSE.
6. CONTRACTOR shall organize, coordinate and participate in PMT/PCIHBV’s
design review exercises and other 3rd party reviews initiated by PMT/PCIHBV.
7. CONTRACTOR shall manage the interfaces with other parties involved in the
PROJECT which may include other Contractors and Local Authorities.
8. The CONTRACTOR shall provide supervision, labor, equipment, materials,
tools, and consumables to perform the work in accordance with PROJECT
SPECIFICATION. Work not specifically shown on the drawings but that is
reasonably necessary for the proper engineering, construction and completion
of the PROJECT shall also be provided by the CONTRACTOR.
9. CONTRACTOR shall incorporate equipment and provisions that are mandatory
for the safe and smooth operation of the FACILITIES.
10. CONTRACTOR shall prepare tie-in schedules, method statements and
procurement.
11. CONTRACTOR shall be responsible to perform process simulation and sizing of
equipment.
12. CONTRACTOR shall carry out Pipe Stress Analysis for FACILITIES piping
system. CONTRACTOR shall provide all the necessary pipe expansion loops,
supports or any other stress relieving measures to the piping.
13. CONTRACTOR shall incorporate all applicable local statutory requirements and
HSE requirements. CONTRACTOR shall ensure that its design layouts shall
result is a safe, operable and maintainable system.
14. CONTRACTOR shall undertake complete venting/drain calculations relevant to
normal operating and maintenance operations, shut down, start up,
COMMISSIONING and emergency scenarios.
15. CONTRACTOR shall conduct soil resistivity surveys to determine cathodic
protection requirements for the buried piping/pipeline, tank and other facilities (If
any)
16. CONTRACTOR shall conduct bids evaluations, and review and incorporate
technical documentation and drawings produced by VENDORS including inter-
disciplinary checks.
17. CONTRACTOR shall review all documentation to ensure compliance with
QA/QC and HSES requirements.
18. CONTRACTOR shall provide and implement QA/QC system to ensure quality
and integrity of all deliverables.
19. CONTRACTOR shall prepare construction work packages, including
specifications, procedures, drawings, etc.

20. CONTRACTOR shall develop performance test requirements for major


equipment, units and the overall FACILITIES.
21. CONTRACTOR shall witness and report performance tests at VENDOR’s
shops.
22. CONTRACTOR shall implement Document Control System for design and
engineering documentation including VENDOR documentation.
23. CONTRACTOR shall incorporate process design, operation and maintenance
requirements in the design and engineering of all disciplines.
24. CONTRACTOR shall ensure that all control systems supplied are loaded with
the latest proven operating software. In addition, upgraded version of the
software shall be provided and installed as part of the CONTRACT PRICE if
such upgraded version is available prior to FINAL ACCEPTANCE.
25. CONTRACTOR shall provide field engineering support during construction,
PRECOMMISSIONING and COMMISSIONING stages.
26. CONTRACTOR shall incorporate EIA recommendations in the design as
required by the authorities.
27. CONTRACTOR shall prepare final documentations.
10.3 Civil and Structural
1. CONTRACTOR shall carry out all required complete detailed engineering and design
including all studies, specifications and procedure, analytical and numerical analysis,
calculations and interpretations, data sheets, engineering drawings, Material Take
Off (MTO) and any, and all other work necessary to support the design, procurement,
fabrication, installation, inspection, testing, commissioning and transitioning to
operations of the FACILITY.
2. Perform all engineering and Detailed Design to progress the designs and design
drawings to Approved for Construction (AFC) status, As-Built status, and Regulatory
approval.
3. Verify and analyse all FEED Documentation related to Civil, Structural and
Architectural works.
4. Conduct all necessary Geotechnical Investigations and Topographical Surveys for
all work sites, including underground services survey where required. Soil
investigation reports performed during FEED shall be treated as reference only,
CONTRACTOR shall perform its own investigations and surveys.
5. CONTRACTOR shall finalize structural and Civil layouts and equipment
arrangements to accommodate all requirements. Any changes errors, omissions,
discrepancies, and/or conflicts to the structural and civil model (i.e., changes due to
process layout and equipment confirmation) shall be resolved and corrected by the
CONTRACTOR. Foundation design and analysis shall be performed including all
members resizing from design and/or/ or analyses.
6. CONTRACTOR shall conduct all necessary structural and civil analysis
requirements as per the Iraqi Standard or International (or other codes e.g. API) for
the structure and Civil works in place and all pre-service conditions e.g. in-place
analysis, dynamic and fatigue analysis (IF ANY)
7. CONTRACTOR shall carry out other appurtenances and miscellaneous design such
as equipment supports, service platforms, Joints, equipment supports, handrail,

8. All design and analysis shall be in accordance with International or Iraqi Standard
and relevant codes and standards, as advised by PMT.
9. CONTRACTOR shall prepare, review and update the structural and civil drawings,
including configuration, member sizes, joint details, Buildings, beam,roads,
foundation, roads, welding requirements and details, dimensions, work points, etc.
10. The engineering deliverables shall include the following as a minimum:
i. Design Basis
ii. Drawing Index, Civil Plot Plan and Key Plan.
iii. Building architectural drawings elevations, cross-sections, longitudinal
sections and detail drawings.
iv. Structural general arrangement drawings, elevations, cross-sections,
longitudinal sections and detail drawing.
v. All sanitary/cooling & hot water installation drawings, drainage drawings,
and sewer/septic tank drawings and perform all Detailed Engineering
vi. Site grading and levelling drawings
vii. Foundation drawings for equipment, buildings, structures etc.
viii. General arrangement and fabrication drawings for structures
ix. Design calculations and design report for all foundations, reinforced
concrete structures, steel and other miscellaneous structures;
x. Layout and drawings
xi. Detailing and cross sections
xii. Drawings for buildings and associated facilities
xiii. Drawings for infrastructure items like roads, culverts, T-wall, fences, gates,
barriers, Walkways
xiv. Prepare all door and window schedules, architectural finishes finishing

xvi. Prepare all detail drawings and MTO’s for all adjustments, which may be
required to existing areas for the installation of new equipment and cables
etc.
xvii. Prepare all architectural general arrangement drawings, elevations, cross-

drawings for all water/utilities systems (i.e. drainage, trenches, piping,
cable, etc.) including methods to detect and identify existence of
underground facilities prior to start construction works;
xxxi. Temporary facilities layout plan and details (i.e. water, power, site office,
erection area, temporary site drain, sewerage, fences, gate etc.);
xxxii. Construction specification, method statement and procedures for all civil,
Structural and Architectural works i.e. tanks foundation, piling installation
and testing, deep excavations, earthwork, roadwork, heavy lifting, steel
structure erection, etc.;
xxxiii. Topographical survey Plan indicating Battery limit of the area drawing and
outer coordinates;
xxxiv. Preparation of AFC Drawings/Documents and as-built (Red Mark Up
Drawings and Final As-Built Drawings) are included in EPCC
CONTRACTOR scope.

The following activities are envisaged for the Electrical related to Civil Buildings Work:
1. Civil works related to electrical room with cable cellar, and foundation for transformers,
NERs and emergency diesel sets as per approved layout drawings. Transformer bays
with firewalls and oil containment pits shall be fenced with separate lockable access
gates. (if any)
2. Structural steel pipe rack to support cable tray.
3. Cable Tray Sleepers
4. Cable Trenches.

10.4 Piping Engineering Design


Piping Design Deliverables to be provided by CONTRACTOR for Detailed Design shall include
but are not limited to the following: -

i. Review, verify, update, optimize, and finalize the EQUIPMENT layouts drawing that
generated during FEED stage. CONTRACTOR shall show coordinates of all
EQUIPMENT, buildings, structures, and all other items related to the FACILITIES in
the EQUIPMENT layout drawings appropriate to all necessary areas in order to
properly reflect detailed engineering requirements.
ii. Review, verify, update, and finalise all pipe wall thickness calculations that were
generated during the basic design stage and prepare new piping classes & wall

process parameters or P&ID’s or other reasons.
iii. CONTRACTOR shall carry out complete review of SUBCONTRACTORS and
VENDORS design package including drawings, calculations, specifications,
procedures and others and incorporate all VENDOR Data and update all the affected
piping design drawing as required.
iv. Conduct any other studies, reports, calculations and/or any technical documents
which are required for successful completion of Engineering, Procurement,
Construction and Commissioning of the FACILITIES.
v. CONTRACTOR to develop Tie-in Procedure for each tie-in, Tie-in Isometrics with
MTO
vi. To update the Piping Design Basis.
vii. Piping General Arrangement drawing (aboveground and underground) including
sections and elevation as required and all pipe support.
viii. Piping isometrics with accompanying Bill of Materials (BOM), aboveground and
underground for all sizes. BOM shall list the following for each item on the Isometric
• Quantity
• Unit of Measure
• Brief Item Description
• Commodity Codes
ix. Underground facilities drawing that incorporates all underground piping, civil
foundations, sumps, drawing, underground electrical and instrument cable trenches,
etc as maybe required to complete the engineering design.
x. To update and finalize the Master Equipment List and to provide input for Electrical Load
List.
xi. To perform equipment configuration, selection, and sparing study
xii. To perform equipment design calculation Piping General Arrangement Drawings.
xiii. To review and approve the vendor equipment engineering package including calculation,
drawings, fabrication/erection procedures, lifting procedures, spare parts list, O & M and
other technical documents. CONTRACTOR is fully responsible to review/verify Vendor‟s
Data and drawings before submitting to PMT for review
xiv. To finalize the Mechanical Equipment and Nozzle Orientation and projection Drawings
based on latest development during Detailed Design stage Piping Drawing Index.
xv. To perform material handling study based on the agreed philosophy. The study shall
include the following as minimum:
• Material handling requirement
• Location of crane (IF ANY)
• Other lifting aids (monorails, lifting lugs, manual trolley, etc. IF ANY)
xvi. To provide input to other discipline as required.
xvii. Updated General Platform Layout
xviii. Tie-in schedule list/Piping Interfacing list in-line with final P&IDs

xxiii. Standard Drawings for piping such as pipe branch off, Utility Station detail Drawings
xxiv. Any other piping drawing which are required for the successful realisation of the
Engineering, Procurement, Construction and Commissioning of the FACILITIES.
xxv. Preparation of Construction Specifications/Procedures (isolation for Tie-in works,
welding, Pressure Testing, Non-destructive Testing, Fabrication etc.
xxvi. Preparation of Material Requisitions for all Piping Items including Pipes, Fittings,
Flanges, Gaskets, Stud bolts & Nuts, Valves, etc.

All engineering documents shall be subject to PMT review and approval (according to established
procedures). The details design documents shall be further developed during the construction
stage to reflect the as-built status. All changes shall be marked – up and approved by PMT’s site
engineer.

10.5 Mechanical Design Engineering


The design and engineering Scope of WORKS shall include, but not limited to the followings:
i. Review, verify and update all basic engineering deliverables but not limited to
mechanical design philosophies, specifications and datasheets, material and purchase
requisition, technical bid evaluation, Master Equipment list and calculations.
ii. The documents mentioned in the FEED deliverables shall be further developed in
Detailed Design engineering stage.
iii. Carry out all the necessary activities and studies if deemed required in order to
complete the Detailed Engineering work.
iv. All other engineering calculations, studies and analyses which are required for
completion of the Engineering, Procurement, Construction and Commissioning of the
FACILITIES
v. Carry out all the necessary activities and studies if deemed required that were not
done previously in order to complete the Detailed Engineering work.
vi. Perform interdisciplinary checks with other engineering discipline to ensure
consistency in tie-in points layout design etc.
vii. Coordinate with all EQUIPMENT and MATERIAL VENDORS, sub-VENDORS,
SUBCONTRACTOR and PMT’s other Contractors for the design interface and tie-ins.
viii. CONTRACTOR shall carry out the Vendor data/drawing review.
ix. All other specifications, documents and activities which are required for completion of
the Engineering, Procurement, Construction and Commissioning of the FACILITIES
x. Provide assurance on technical compliance with HSE, national and international codes
and standards, rules and regulations and good engineering practice.
xi. Prepare Vendor Data Incorporation documents for all the mechanical rotating equipment
including pumps, compressors etc.
xii. Conduct Constructability Review
xiii. Perform interdisciplinary checks with other engineering discipline to ensure
consistency in tie-in points layout design etc.

10.6 Electrical Design Engineering

In addition to the general Scope of WORKS, the following section describes the minimum
requirement for electrical engineering activities and that shall be carried out during detailed
design stage b the CONTRACTOR.

The development of engineering activities and deliverables during detail design stage shall refer
and according to FEED design and deliverables. Any additional information, data, drawings, or
specifications not included in the FEED documents and are necessary to be provided to complete
the project shall be prepared by CONTRACTOR, as part of his scope of work. All engineering
documents shall be subject to PMT review and APPROVAL (according to established
procedures) for AFC status.

Develop electrical documents, studies, reports, and calculations during engineering design stage
to complete and finalise the electrical system, philosophy, Equipment sizing and selection, which
are required for completion of Procurement, Construction and Installation, Testing and
COMMISSIONING Work and activities.

Review of other discipline drawings and documents related to Electrical discipline for inter-
disciplinary checks and updates.

Propose for power supply from nearby Existing 6.6kV OHTL layout

stage, which are required for completion of Procurement, Construction and Installation, Testing

10.7 Telecommunication
The section describes the Telecommunications requirements pertaining to the detailed design
and its deliverables for Telecommunication which shall consist of but not limited to Fibre Optic
Cable Connectivity, and Close Circuit Television system (CCTV) and Public Address and General
Alarm (PAGA) System to be undertaken by the CONTRACTOR.

The activities shall include but not limited to the following:


1. Prepare, plan and schedule all telecommunication works.
2. Review, verify, validate and update all FEED documents related to telecommunication
systems such as specifications, block diagrams, design basis etc. prior to starting the
detail engineering WORKS.
3. Develop and produce all detailed Engineering documents, data sheets, drawings,
equipment lists, schedules, location layouts, routing layouts, installation drawings, MTO
etc.
4. Generate Material Requisitions for respective telecommunication systems, equipment
and related accessories.
5. Check and evaluate supplier FAT/SAT/testing procedures.
6. Perform VENDOR Data Review (VDR) and VENDOR Data Incorporation (VDI).
7. Provide inputs to Operating Procedures Manual.
8. Testing, addressing, and labelling telecommunication equipment and cables for site
and related far site.
9. All other telecommunication design engineering activities required for completion of
engineering, procurement, construction, installation, testing and COMMISSIONING of
the PROJECT.

CONTRACTOR’s Detail Engineering key deliverables as minimum shall include the following:

10.8 Fire Safety Assessment


CONTRACTOR shall develop/engage a Third Party who is familiar with PMT requirements as
well as Local Authority’s requirements to perform the Fire Safety Assessment (FSA) study with
approval by PMT.
The FSA study shall be carried out in accordance with Iraqi Standard or International standard
via on-site verification. FSA shall meet the objectives as below:

i. To analyse all fire potential scenarios of the FACILITIES.


ii. To identify suitable fire protection measures as stipulated in the Fire Safety Design
Philosophy;
iii. To provide a list of fire hazards associated with pre-identified hazard (by area) on the
FACILITIES, fire protection options and record the evaluation in determining overall fire
protection system for design consideration.
Selection of fire protection system, fire alarm system shall first be made based on the local
legislative prescriptive requirements.
In the absence of local legislative prescriptive requirements, the selection shall be based on
performance of the equipment and system in mitigating the identified potential fire/explosion risk
at site.
CONTRACTOR shall be responsible for the engineering, procurement, construction and
commissioning of firefighting and fire prevention system for the FACILITIES.

The Fire-fighting system shall be provided by a combination of one or more but not limited to the
following systems and equipment:
1. Fire water
i. Hydrants
ii. Fixed fire water monitors
iii. Portable fire water monitors
iv. Hose reels
v. Water spray system/ Deluge systems
2. Foam-based
i. Fixed foam chambers for hydrocarbon storage
ii. Foam monitors
iii. Wheeled and mobile type foam trolley
3. Carbon dioxide - Portable and wheeled type
4. Dry chemical - Portable and wheeled extinguishers
5. Fire Fighting Cabinets
6. FM 200 for Substation
Each type of system/ equipment shall be selected according to the fire hazards present and shall
be provided and strategically located at certain interval to achieve optimum fire safety coverage
for the FACILITIES.

11.0 CONSTRUCTION
11.1 GENERAL
CONTRACTOR shall construct the WORK in accordance with the requirements of this
CONTRACT and with proven and generally accepted practices and procedures. The
WORK shall be safe, reliable, and suitable for their intended purpose.
CONTRACTOR shall perform the Construction works which includes the followings:
1. The Scope of Work comprises of all site preparation works, earthworks, roadworks,
foundation, concrete, Electrical, Mechanical, piping and steel structural fabrication
& installation for the successful execution of the CONTRACT.
2. The Scope of WORKS shall be read in conjunction with the FEED package, Detailed
Design (AFC) Deliverables, Specifications, Drawings, Codes and Standards,
Procedures and other related CONTRACT Documents. In the event of any conflicts,
the stringent document shall take precedence. However, PMT Representative shall
have the discretion to decide the order of precedence
3. CONTRACTOR shall provide all labour, materials and equipment, consumable and
other services necessary to complete the WORKS in accordance with the
CONTRACT.
4. CONTRACTOR shall be responsible for verifying and completing the WORKS
based on information contained in the specifications, DRAWINGS, procedures and
from information that will be provided by equipment vendors and good engineering
practices.
5. CONTRACTOR shall perform all engineering services necessary for the efficient
execution of WORKS. All the required engineering services including but not limited

to preparation of shop drawing, cutting plan, Model review and update and weld map
shall be executed at site. CONTRACTOR shall responsible to provide complete
engineering team and adequate facilities at site for the efficient execution of
WORKS.
6. CONTRACTOR shall prepare shop drawings, cutting plans, weld maps and submit
to PMT for review and comment. All shop drawings shall be based on “Approved for
Construction” drawings. PMT‟s review and comment of shop drawings, cutting plans
and weld maps shall not constitute a hold point and shall not relieve the
CONTRACTOR of the responsibility to fulfil the requirements of the CONTRACT. In
any circumstances, CONTRACTOR shall be responsible to expedite the outstanding
issues or approval of shop drawings affecting the schedule from PMT.
CONTRACTOR may proceed with the WORKS but shall immediately stop the
affected WORKS if so required to incorporate PMT‟s comments. All cost for
rectification and re-works shall be borne by CONTRACTOR.
7. CONTRACTOR shall carry out all Construction and installation in such a way that
the WORK envisaged under the CONTRACT can be built and completed in
sequence without disruption or interruptions to any ongoing jobs in the area.
8. The WORK shall comply with “Approved for Construction” drawings, specifications,
procedures, plans and the applicable specified installation and construction
standards and codes. CONTRACTOR shall carry out the construction and
installation in such a way that the FACILITIES can be built, Pre-Commissioned,
Commissioned and started-up in sequence without disruption or interruptions to
existing operating facilities.
9. CONTRACTOR shall provide all necessary resources and services including but not
limited to skilled, knowledgeable and competent personnel, labours, specialist
support, material, equipment, facilities, software, consumables, transportation,
power, water, and other Utilities necessary to complete the WORK as per
CONTRACT.
10. CONTRACTOR shall organise and assign a proper PRECOMMISSIONING and
COMMISSIONING Team on Site which shall include but not limited to all disciplines
COMMISSIONING Engineers and adequate discipline Technicians during the
Construction phase at least three (3) months prior to start of PRECOMMISSIONING
WORK.
11. CONTRACTOR shall complete Construction and PRECOMMISSIONING, System
by System before Start Up and COMMISSIONING of the FACILITIES as identified
in the CONTRACT.
12. Where a shutdown of the existing facilities is required to allow for safe
implementation of the construction activities e.g. tie-ins to live system, the schedule
of such pre-planned shutdown shall be finalized in agreement with PMT.
13. CONTRACTOR shall note that PCIHBV is not obliged to provide a shutdown window
for CONTRACTOR for executing the critical. CONTRACTOR shall explore all
possible ways to avoid a facility shutdown including the hot or power tapping.
14. The WORK shall be provided and constructed in a safe, systematic methodical and
reliable manner. The latest sound and proven Construction techniques and
equipment shall be used to provide an effective Construction approach, which
achieves the completion dates stipulated by CONTRACT.
15. CONTRACTOR shall comply with PMT safety and works permit procedures during
Construction and shall enforce a set of approved safety rules and precautions at
WORKSITE.
11.2 GENERAL CONSTRUCTION REQUIREMENTS

CONTRACTOR’s Construction WORK shall include the following:


1. Obtaining all required licenses, approvals, and permits for the WORK.
2. Establishing WORKSITE safety rules and providing a Security Officer, Safety Officer
and staff for the WORK.
3. CONTRACTOR shall provide all labour, materials and equipment, consumable and
other services necessary to complete the WORKS in accordance with the
CONTRACT.
4. CONTRACTOR shall allow provision for interface and coordination with other
construction Contractor with regards to scope of work, engineering and/or
construction interfacing between relevant roads, storm water drain etc. for the
successful completion of the WORKS.
5. CONTRACTOR shall be responsible to supply, construct and fabricate, all
necessary supporting structures, maintenance access roads, maintenance access
platform, ladders (if required) that are not included in scope of work but deemed
necessary to complete the work. Any clash mitigation, changes, rerouting, and
redesign, rework that may be required for successful road construction, operation,
and maintenance shall be at CONTRACTOR‟s cost.
6. Providing CONTRACTOR’s Representative, temporary work, temporary facilities
and other facilities required for the WORK. All temporary facilities construction and
installation shall meet all safety regulations.
7. CONTRACTOR shall be responsible for checking all drawings against all approved
equipment vendor data and for making all necessary design revisions prior to
construction. CONTRACTOR shall be responsible to finalize the design and
fabrication works to tie-ins of platform systems to all equipment packages and its
individual components at no additional cost to PMT.
8. CONTRACTOR shall conduct a power system study and protection coordination
study if there is significant change in actual equipment installation which include but
not limited to change in cable length, motor size, equipment rating, etc.
9. CONTRACTOR shall produce Fabrication "As-Built" revision of all AFC drawings
relevant for installation and commissioning works prior to handover.
10. CONTRACTOR shall be responsible for the preparation and verification of all
Materials Take-Offs (MTO). CONTRACTOR shall provide the detailed MTO for
PMT's review for the supply of materials in line with the execution of the WORKS.
11. CONTRACTOR shall provide proper sufficient and designated storage space at site
for COMPANY supplied items (if any). CONTRACTOR shall provide the storage
location within at most 2 Kilometres (km) of radius from CONTRACTOR‟s
WORKSITE and to allow full accessibility to PMT site team for the site visit and
inspection at any time.
12. CONTRACTOR shall provide sufficient and dedicated personnel to monitor and be
in-charge of the receipt, handling, loading, unloading, transportation, inspection,
storage and Installation of all materials and equipment.
13. CONTRACTOR shall also submit two (2) years operational spare parts list together
with part numbers and prices as recommended by the manufacturers for
CONTRACTOR‟s supplied equipment as stipulated in MTO.
14. CONTRACTOR shall ensure all the CONTRACTOR‟s supplied equipment shall be
installed under the supervision of Vendors at CONTRACTOR own cost.
15. CONTRACTOR shall comply to PMT's requirement on escape route and shall
relocate all items such as valves, equipment, instruments, lighting fixtures, panels,
piping, electrical, instrumentation, communication and related routings to allow for

operational and maintenance accessibility without sacrificing technical integrity at


CONTRACTOR's own cost as identified by CONTRACTOR or as directed by PMT.
16. Prior to commencement of any part of construction, CONTRACTOR shall submit the
relevant procedure or method statement to PMT review and endorsement
17. If any CONTRACTOR supplied equipment/material is found damaged of any nature,
the CONTRACTOR shall bring to PMT's attention immediately and propose
remedial action for PMT's approval. CONTRACTOR shall produce a formal report
describing the nature of damage and submit to PMT. PMT reserves the right to
accept or reject any damaged materials and equipment. CONTRACTOR shall be
responsible to replace new of any damaged materials and equipment which are
rejected by PMT.
18. PMT shall have the right at any time to inspect the following:
(a) Materials, construction, testing and painting preparation of all items related
to WORKS, without limitations.
(b) All correspondence with materials suppliers or SUBCONTRACTORs to
determine actual materials availability, delivery dates and MTO
Documentations.
19. Providing and maintaining lodging and transportation for CONTRACTOR’s
personnel and Sub-Contractors in-compliance with Iraq Laws and Regulations.
20. PMT shall have the right to reject any materials, equipment to be used in any part
of WORKS or WORKS which do not conform to CONTRACT requirements or found
unsafe.
21. PMT shall have the right to reject to WORKS which does not conform to CONTRACT
requirements at CONTRACTOR's own cost.
22. PMT shall have the right to request for any re-inspection or retesting to WORKS
which does not conform to CONTRACT requirements at CONTRACTOR's own cost.
23. Providing Site QA/QC and inspection and testing services for the WORK. This shall
include, but not be limited to civil, structural, welding, piping, buried pipeline/crossing
backfilling, soil/backfill compaction, mechanical, electrical, instrument and
painting/coating inspections.
24. Maintaining and operating an adequate, sound and proven system of document
control and availability of drawings, specifications, codes and standards at their
latest revision at Site.
25. All equipment to be used during the execution of WORKS shall be physically
inspected to ensure that equipment is clean, free from mechanically damaged, and
that all the fixing brackets, clamps, etc. are tightened correctly and securely
mounted.
26. All construction WORKS shall be subjected to inspection and testing in accordance
with the specifications, codes and standards at the WORKSITE by CONTRACTOR
and witnessed by PMT as per approved ITP by PMT. CONTRACTOR shall furnish
PMT with copies of records for all inspections.
27. Managing, coordinating and supervising the WORK of CONTRACTOR and Sub-
Contractors to ensure WORK is performed on schedule and in accordance to quality
and safety requirements, including preparing detailed schedules for the WORK in
accordance with the overall Project and Construction Schedules, regular monitoring
of this schedule and making appropriate adjustments to obtain efficient usage of
resources.
28. CONTRACTOR shall provide complete on-site facilities for the radiographic,
ultrasonic and other non-destructive testing and inspection of welds including bomb-

pit, bunker, standard dark room, equipment for development of films, viewing
facilities, and full time services of qualified technicians conversant with radiographic,
ultrasonic, and other inspection techniques, who will be competent to operate the
above equipment as directed by CONTRACTOR and/or PMT.(If require)
29. CONTRACTOR shall adhere to the Welding Qualification Test Procedure based on
PMT or AWS D1.1 specifications, code and standards. All Welding Procedure
Specifications (WPS) previously approved by PMT under pre-accepted procedure
are acceptable without the need to perform another test. CONTRACTOR shall
identify and submit to COMPANY the applicable WPS for PMT's acceptance.
30. The CONTRACTOR is required to submit the method statement of excavation in
advance to get PMT approval. The excavation works shall be done as per the
Approved for Construction (AFC) drawings and layouts including the required side
support (shoring), dewatering, and back-slopes and escape routes as per PMT HSE
recommendations. It shall include the protection works of underground utilities and
adjacent structures.
31. CONTRACTOR shall be responsible to perform all activities related to the
Fabrication Engineering of the piping components including the tie in spools and its
components. CONTRACTOR shall be responsible for the preparation of all
calculation notes, detailed specifications and procedures, reports, WORK plans,
shop drawings, AFC drawings, and the like necessary for the successful and
efficient execution of the prefabrication and fabrication WORK to be performed by
CONTRACTOR, SUBCONTRACTORS and VENDORS.
32. CONTRACTOR shall perform all pre-fabrication and fabrication drawings, including
shop drawings for CONTRACTOR items, detailed technical procedures, calculation
notes, data sheets, and the like, necessary to perform the WORKS.
33. The procedures and associated calculation notes, plans, layouts and drawings to be
prepared by CONTRACTOR shall, at a minimum, includes the following:
• Manufacturing
• Pre-fabrication, assembly, and testing for all permanent and temporary
WORKS.
• Application of anti-corrosion coating, field joint coating, internal line pipe
cleaning, and the like.
• All welding and NDT Procedures
• Cleaning, gauging and hydrostatic testing.

11.3 CIVIL AND STRUCTURAL

1. CONTRACTOR shall supply, fabricate, paint/coat, install and inspect all structural
works, in accordance with the CONTRACT.
2. CONTRACTOR shall supply all materials, equipment, labours and services
necessary to do the WORKS, including all materials required for temporary WORKS
such as erection aids, supports, scaffolding, etc. to complete the WORKS.
3. CONTRACTOR shall be responsible for the detailed design, mounting detail and
also to perform structural strength analysis of equipment seating and the foundation
at the point where the equipment will be placed. Such detailed design checks and
strengthening are required for CONTRACTOR and PMT supplied (if any) materials
and/or equipment.
4. In the event of changes to the equipment size and weight, CONTRACTOR shall be
responsible to verify and update the design and mounting detail of the equipment

seating and the foundation at the point where the equipment will be placed. This shall
be applicable for both CONTRACTOR and PMT supplied (if any) materials and/or
equipment.
5. CONTRACTOR shall design, supply and install strengthening structure and supports
at all piping, equipment, electrical and instrument items for PMT (if any) and
CONTRACTOR supplied material or equipment at CONTRACTOR's own cost.
6. CONTRACTOR shall ensure all equipment is accessible for operation and
maintenance activities. CONTRACTOR shall design, supply and install access
platforms with handrails, hatches, ladders and protection for equipment, electrical,
instruments and valves as indicated in the CONTRACT or as required by PMT at
CONTRACTOR's own cost.
7. Construction of buildings including interior and exterior finishes,
electrical/instrumentation installations, furniture, AC, plumbing and drainage, Septic
tank, fire detection etc. all necessary works in order to deliver finished building ready
for occupation including commissioning of all building utilities.
8. CONTRACTOR shall provide product catalogue for furniture to PCIHBV for approval.
9. CONTRACTOR shall ensure that all deck hatch covers are leak proof.
CONTRACTOR shall perform a hose test to prove water tightness. (IF ANY)
10. Construction of steel piles, concrete cast-in place piles, if required.
11. Foundation for buildings, shelters, Steel Gates, EQUIPMENT, structures etc. and
all associated civil / structural works.
12. Structural steel works for enclosed warehouse, gates, shade, equipment support
structures,, crossovers, platforms, stairs, ladders, handrails, etc.
13. Supply and installation of anchor bolts, grouts for structures, including post
installed chemical anchors as required.
14. Design, construct and install perimeter reinforced concrete T-wall and bunkers.
15. Concrete surface protection for below and above ground surfaces.
16. Installation of underground drainage system, pits, manholes and all associated
civil works.
17. Concrete paving, interlocking tiles, gravel spreading, surface finishing and
associated civil works.
18. Foundations for light poles, flood light mast and U/G earth pits and associated civil
/ structural works for electrical and instrument supports.
19. Trenching, padding, backfilling for U/G piping, electrical, instrumentation works.
20. Painting and protective coatings for structural steel works, buildings.
21. Temporary support and protection to existing underground services such as pipes,
cables, duct banks, drains etc. during construction and proper reinstatement.
22. Backfilling by natural soil shall be in layers with compaction as per the requirements
and surplus materials shall be removed from the site and transported to approve
area.
23. Temporary facilities including site office, accommodation, workshop, laydown areas
and temporary access road for the transportation and installation of the heavy
equipment.
24. Unless otherwise specified, a thorough compaction of a backfill shall be carried out
with vibrating rollers, plate compactors and tampers to the satisfaction of PMT.
Watering of fill shall be affected to ensure good compaction at optimum moisture
content. Backfilling shall be deposited in maximum loose lift/layer thickness of
200mm and compacted to 95% of maximum dry density determined accordance with
ASTM D1557.

25. Backfilling by sub-base type B shall be in layers with compaction as per the
requirements and surplus materials shall be removed from the site and transported
to approve area.
26. All backfilling materials of natural soil and sub-base type B shall be tested and
conform to the requirements.
27. All the test materials conducted by LABORATORY shall be accredited by TOC or
Iraq Government.
28. Should the laboratory be not acceptable by TOC or Iraq Government, PMT shall
have the right to instruct the CONTRACTOR to conduct the test at TOC or Iraq
Government laboratory without any cost.
29. Supply of all civil, structural and architectural MATERIALS including providing
necessary test results or test certificates.
30. The compaction rate of each finished course shall be tested by bulk density
(AASHTO T166) on core samples or by nuclear method (ASTM D2950-74). The bulk
density of the finished mixture shall not be less than 97% of the laboratory bulk
density of the mixture when compacted in Marshall Moulds with 75 blows on each
face.
31. Provision of material, soil skimming, site cleaning at distance of (80X150) M of grass
with leveling and removing waste out of the work site, removing plants preparing the
road that leads to the site, and preparing necessary supplies and main requirements
for the supervision and follow up committees.
32. Bury with clean, soft soil by spread of a layer of subbase under the concrete base,
with distance as per the design as layers with thickness of (20) CM and height as
per design, and the percentage of rolling compaction at (95%), to be higher than the
level of flood.
33. Supply material and spread of subbase layer under the concrete base with distance
as per design, type (B) with good spread and rolling, and the layer thickness shall be
(20) CM, and the rolling percentage shall be (95%), including the installation of plastic
pipes inside the subbase layer in accordance with the design to connect cables with
collected unit equipment
34. Excavation of ditch of the concrete base and removing of soil out of the work site in
accordance with the diagram attached.
35. Supply material and construction of Blanding concrete with mixing at (6:3:1) rate
under the concrete base with distance asper design with the use of the salt
resistance cement, and the final surface shall be flat and at the same level.
36. Supply material and construction of the concrete base with the use of iron bars, and
the distance as per design and the distance between the bars is (15) CM in the
longitudinal and transverse directions for every piece with the use of salt resistance
cement, as well as constructing the ditch with iron bars of (16) MM and mixing rate
of 4:2:1 and capacity of (35 Nut / SMM) according to PTS, and the concrete thickness
shall be (30) CM, taking into account the use of fresh water for concrete construction,
and the ground shall be covered with thick nylon layer before work, as well as the
use of Water Stop if the work of constructing the base and the ditch shall be in two
stages,
37. Supply material and fixing the cover of the ditches made from steel of (6 MM)
thickness and rectangle steel between the corners and juger Plate. They shall be
fixed by welding into pieces to cover ditches and to make alleys at the same level
with the concrete layer as per attached diagram.
38. CONTRACTOR shall produce a detailed method statement for all works and shall
be approved by PMT.

39. CONTRACTOR shall plan and execute all aspects of WORKS so that the safety of
personnel, the work and adjacent property is guaranteed and such that a minimum
inconvenience is caused
40. CONTRACTOR shall assume full responsibilities for safety of personnel and existing
works / facilities and utilities in nearby area at all times.

11.4 Mechanical and Piping


1. CONTRACTOR shall supply, fabricate, paint/coat, install, inspect, test, pre-
commission and commissioning all mechanical equipment, pipe fittings, valves and
pipe works as shown on the Drawings and in accordance with the CONTRACT.
2. CONTRACTOR shall ensure that the piping routing on the General Arrangement
(GA) and elevation drawings shall be read in conjunction with the P&ID's. The P&ID
will take priority in the event of conflict between the P&ID and GA drawings.
3. Supply of plumbing MATERIALS (including but not limited to pipes, pipe fittings, etc),
transportation to the WORKSITE, fabrication, installation, testing, painting and
COMMISSIONING.
4. Supply of all SUPPLIES and temporary MATERIALS, including all tools and tackles,
required for the execution of the job.
5. Plumbing fabrication, erection and testing shall be in accordance with the relevant
specifications included in the CONTRACT.
6. The CONTRACTOR shall identify/consider future Tie-in Interface locations and
plumbing installation for sewerage lines and septic tanks during phase-wise
plumbing installation.
7. Functional testing of the Plumbing and sanitary systems including leak tests and
integrity as required.
8. CONTRACTOR shall carry out suitable WORKSITE surveys to confirm the
correctness of all associated Tie-in lists/Tie-in schedules (phase-wise) with respect
to FEED P&ID information at proposed tie-in points, new EQUIPMENT locations and
other areas of specific interest.
9. Quantities indicated in the MTO report are net quantities required for final installation
of piping system. CONTRACTOR shall be responsible to determine all the
necessary contingencies for field adjustment, welding, etc. CONTRACTOR shall
indicate the contingencies percentage and quantity in the MTO for the PMT‟s
approval.
10. CONTRACTOR shall be responsible for the engineering, procurement, construction
and installation of all pipes, flanges, valves, fittings, bolts, gaskets, piping
appurtenances, pipe supports, application of paint and completion of all corrosion
protection of all piping systems and any other systems in accordance with the
CONTRACT to complete the WORKS. CONTRACTOR shall review and update the
design for the pipe routing and supports of all piping. However, modification of a
stress analysed piping will require PMT‟s approval (If any).
11. CONTRACTOR shall procure MATERIALS, fabricate and install pipe supports as
per the PCIHBV standard drawings and applicable PROJECT drawings, the
supports shall be as per support tags identified on construction isometrics, if there
are constructability issues, CONTRACTOR shall bring it to PCIHBV’s notice
12. CONTRACTOR shall be responsible to check and verify drawings for interference
prior to commencement of construction of all piping. Any necessary change that
needs to be done to the piping design as represented on drawings, as a result of
interference, vendor data incorporation or relocation of skids shall be carried out at
CONTRACTOR's own cost inclusive of engineering (inclusive of pipe stress

analysis), procurement, modification, construction and installation of additional pipe


runs and fittings.
13. CONTRACTOR shall complete equipment installation and piping assemblies to the
specified alignment and loading. Constructed assemblies shall be free from any
extraneous construction loading and strain.
14. EQUIPMENT installation shall include the supply and placement of grout for
equipment or materials and the supply and installation of anchors and fasteners as
required for a complete installation.
15. Motor driven equipment placement shall include alignment, assembly of belts,
sheaves, couplings, guards, etc. as required for a complete installation.
16. The CONTRACTOR shall inspect the location for the EQUIPMENT to be installed
prior to installation and to ensure readiness of area including anchor bolt locations,
size, projections and placement of support steel where applicable.
17. No installation of EQUIPMENT shall commence without release of the area for such
installation.
18. All MATERIAL and EQUIPMENT furnished by the CONTRACTOR shall be in
accordance with the PROJECT SPECIFICATION. Substitution of any MATERIAL
deviating from the PROJECT SPECIFICATION and drawings shall be approved in
writing by PMT. A description of CONTRACTOR supplied substituted material shall
be submitted to the PMT for approval.
19. CONTRACTOR shall review and understand the manufacturer’s installation
requirements prior to commencing the installation of the EQUIPMENT.
20. The CONTRACTOR is responsible to alter the drawing dimensions affected by using
different approved items where applicable. Any such dimensional changes shall be
recorded on “As-Built” drawings.
21. Before installation, all parts shall be thoroughly cleaned of all rust, grit and foreign
matter. Where accessible without disassembly, all holes and grooves for lubrication
shall be examined and cleaned where necessary. Bolts and screws shall be
tightened uniformly without over-stressing the threads. Anchor bolts and all other
bolts shall be tightened as per manufacturer's instructions.
22. During installation of EQUIPMENT, all small access openings shall be covered with
temporary covers made of tape, plywood or sheet metal whenever work is not
actually in progress.
23. Clearance around all EQUIPMENT shall be checked prior to installing the
equipment. Any interference or lack of access for maintenance that may be evident
shall be rectified accordingly.
24. All foundation elevations and bolt locations shall be verified prior to the start of
installation of EQUIPMENT. Bending of bolts to fit equipment base plate holes
beyond the limit of the bolt sleeves is not allowed.
25. Levelling and alignment of all EQUIPMENT shall be within the tolerance specified in
the manufacturer's instructions. Where equipment is received as a shop-assembled
unit, alignment shall be checked and adjusted where necessary.
26. Levelling plates, dowels, shims, and grout are to be furnished by the
CONTRACTOR. Grouting material shall be applied in accordance with the printed
instruction from the grouting material manufacturer.
27. Field alterations to EQUIPMENT to facilitate installation shall not be made without
written approval from PMT. Subassemblies shall be dismantled only to the extent
necessary to verify clearances, equipment condition and lubrication requirements.

Total dismantling of the subassemblies, if necessary, shall be carried out only with
the written approval from PMT.
28. Safety guards around rotating equipment furnished by the equipment manufacturer,
but shipped loose shall be installed by the CONTRACTOR in accordance with
equipment manufacturer’s instructions.
29. For all rotating equipment, the manufacturer’s instructions for installation and
approved drawings shall be followed for levelling, coupling alignment, grouting, and
final checking of alignment.
30. Items that may be damaged during equipment installation (sight glasses, gauges,
davits, thermowells, earthling cables etc.) shall be removed and temporarily stored.
All openings, bearing surfaces and threads shall be protected.
31. Supply of the FACILITIES piping material (such as pipes, bends, transition pieces,
barred tees, tees, fittings, flanges, gaskets, fasteners, valves, special items, etc.),
transportation to site, fabrication, installation, testing, painting and commissioning.
32. CONTRACTOR shall carry out WORKSITE survey and verification of above ground
and underground piping arrangement/routing and WORKSITE verification of tie-ins
for existing facilities.
33. Preparation of fabrication drawings, including field weld identifications for all pipe
WORK;
34. Identification, fabrication, inspection, testing and painting of piping spools that may
be prefabricated in CONTRACTOR’s yard;
35. Supply of all consumables, temporary materials including all tools and tackles
required for the execution of the job.
36. Preparation of Weld Procedure Specifications (WPS) and qualification of the
procedures using Procedures Qualification Records (PQR) and repair procedure
covering all types/range of welds for PMT’s approval. PMT shall approve all WPSs
and PQRs. CONTRACTOR shall qualify all welders in accordance with the PMT
approved PQRs.
37. All associated civil works such as trenching and back filling, pipe sleepers/racks and
other pipe supports, platforms, crossovers, etc. All underground pipes shall be
installed with marker for traceability in future works.
38. CONTRACTOR shall prepare and submit to PMT inspection and test plan (ITP);
39. The fabrication and erection WORK shall be inspected / tested in accordance to
PMT specifications and PMT approved inspection and test plan (ITP);
40. CONTRACTOR scope of supply shall include all consumables, temporary supports,
structural steel members, supports etc. for the successful implementation of the
PROJECT;
41. CONTRACTOR shall prepare detailed shop drawings including as a minimum spool
number marking, field weld marking, weld maps, parts and MATERIALS list along
with weights;
42. CONTRACTOR shall review the complete AFC package and check for
constructability of the designed system;
43. Grit blasting and painting of new/modified piping and structural steel WORK,
EQUIPMENT, etc, including pipe supports, EQUIPMENT base plate/support
structures and any touch-up painting required thereof shall be in accordance with
applicable FEED specification;
44. CONTRACTOR shall be fully responsible for the preparation of non-destructive
examination (NDE) procedures and to obtain PMT’s approval. Once approved, NDE

shall be strictly implemented in compliance with applicable codes/standards, PMT


specifications and established engineering practices (if require).
45. Prior to closing up the piping systems, CONTRACTOR shall conduct visual
inspection of the internal pipes to ensure that no foreign materials are left in the
pipes which cannot be flushed out during flushing. CONTRACTOR shall observe
sign of blockage of the internal pipes during flushing and shall notify PMT
accordingly
46. CONTRACTOR is required to properly preserve and blind off the opened end
47. CONTRACTOR shall make the necessary temporary piping WORK to supply water
at the hydrostatic testing designated location (if require)
48. CONTRACTOR shall be required to provide the necessary hoses, connections to
hydrants / water sources, blinds, etc. for successful hydrostatic testing of new and
modified pipe WORK as per FEED Specifications.
49. CONTRACTOR shall be responsible for hydrostatic and pneumatic testing, as
applicable, for shell and seats of all manual and actuated valves upon receipt of all
valves at construction site. After successful testing, the valve ends shall be covered
with close fitting protectors to prevent the expose sealing surfaces and machined
parts from being ingress with dirt/moisture.
50. Fabrication, installation, radiography, testing, coating, painting and box up of piping
systems under the supervision of an experienced and qualified construction
supervisor and QA/QC engineer.
51. Radiography and stress relieving shall be included wherever applicable.
52. CONTRACTOR shall visually check the equipment with manufacturers’
test/inspection record, but equipment shall normally not be opened unless there is
clear evidence that an internal inspection is necessary.
53. CONTRACTOR shall submit to COMPANY the leak test procedure of at least one
(1) week before the activities. The test procedure shall clearly specify which test
medium to be used, test pressure, method of leak detection, safety precaution and
leak test section on the mechanical flow diagrams or P&ID drawings. The leak test
package shall be reflected in the P&ID and isometric drawings
54. CONTRACTOR shall be responsible for holding co-ordination meetings with all
parties involved in the tie-in WORK and complete a tie-in responsibility matrix. The
purpose of this is to ensure that all parties associated with the installation of tie-in
WORK understand their part of responsibility for the tie-in WORK.
55. These tie-ins shall be planned, engineered, scheduled and implemented with prior
consultation and agreement with PCIHBV.
56. CONTRACTOR shall study related systems of the existing facilities and finalize tie-
in details.

11.5 Electrical

The CONTRACTOR shall install, test, and COMMISSION the electrical systems, as per
applicable Standards, Codes and project Specifications, Scope of Work and as shown on the
drawings and documents included in the CONTRACT.

CONTRACTOR shall perform installation, testing and commissioning based on approved


procedures. Refer to Specification for Electrical Installation IEC standards for complex
electrical system. Shall be commissioned with the assistance of Manufacturer.

CONTRACTOR shall verify the lighting lux levels of all rooms and open areas as per PCIHBV
requirements. Where the lux levels do not meet the requirements, CONTRACTOR shall
make the necessary modifications to the number and layout of the luminaries at his own cost
to meet those requirements.

CONTRACTOR shall comply with requirement for outdoor equipment with minimum IP54
Rating and indoor equipment’s with minimum IP41.

CONTRACTOR shall comply with requirement for outdoor installation with armor of the
supply cable with glanding, cable tray, and etc

In addition, CONTRACTOR shall comply with Portable Equipment & Temporary Installation
(PETI) standard IEC standards for any temporary installation and use of portable equipment
during installation at site.

The General Scope of Work includes but not limited to:

i. Design, supply and installation of all materials, hardware, steelwork, supports, cable
terminating, connection and glanding materials, cable protection tiles, river sand for
underground cable, conduit, warning tape, electrical tape, cable tie, cable tags,
cable tray and all other items which are necessary for a complete installation of the
electrical systems.
ii. Design, supply, and installation of all cable ladders/trays, supports, and all brackets
and holding down clamps, bolts and hardware accessories.
iii. Design, supply and installation of all conduits, conduit supports and mechanical
protection.
iv. Design, supply and install all of cooling system, heaters and ventilation for the room,
Cold Storage room and warehouse.
v. Design, supply and install all of circuit breakers and distribution boards.
vi. Design, supply, installation and COMMISSIONING the 6.6kV OHTL line tapping
from 6.6kV OHTL WPA to WPQ and all other cables necessary to complete the
electrical installation. Excavation, backfill and compaction of cable trenches
including removal and re-instatement of tarmac and paving required for buried
cables, OHTL concrete foundation.
vii. Design, supply, installation, and COMMISSIONING of 6.6kV OHTL facilities
including OHTL Tubular Steel Pole ( H Pole and T pole), 6.6kV Conductor & Stay
Wire, Insulator strings, 6.6kV lightning Arrestor, Knife switch, Insulator, Cable
Spencer, fitting and hardware , OHTL accessories but not limited to,
viii. Design, supply, installation, and COMMISSIONING of 6.6kV Substation facilities
including HV Room with cooling AC system, Transformer Step down, Ring Main
Units, UPS for relay, fire suppression, relay protection, smoke detector, cable but
not limited to
ix. Design, supply, installation, and COMMISSIONING of LV distribution boards, Sub
DB with provision of manual Transfer switch (MTS) and balance between phase

x. Design, supply, installation, and COMMISSIONING of complete indoor and outdoor
lighting, floodlight, installation and follow lux specification.
xi. Design, supply, installation, and COMMISSIONING of highmast outdoor lighting
installation and follow lux specification.
xii. Design, supply, installation and COMMISSIONING of power and convenience
socket outlets with safety arrangement & ergonomic for indoor and outdoor.
xiii. Design, supply, installation, and COMMISSIONING of a complete lightning
protection, earthing and bonding system, including tie-ins and surge protection
(whenever required) indoor and outdoor.
xiv. Design, supply, installation, and COMMISSIONING of all building electrical services
indoor and outdoor.
xv. Installation of cable route markers.
xvi. Design, supply, installation, COMMISSIONING and sealing of all floor/wall openings
after completion of electrical installation work.
xvii. Contractor shall provide the critical materials shall be from 4 origins (US, Western
Europe, Canada, and Japan).

11.6 Telecommunication
Telecommunication equipment installation shall be carried out by the CONTRACTOR in
accordance with the PROJECT SPECIFICATIONS.

CONTRACTOR's scope of WORK includes installation, tie-ins, inspection and testing of the
Telecommunication Systems (CCTV/ Telephone etc.) as per applicable Codes & Standards and
PROJECT SPECIFICATIONS.

General requirements for Telecommunication installation shall include but not limited to the
following:
1. Excavation and backfill of cable trenches where necessary.
2. Supply and Installation of all hardware, steelwork, supports, cable, cable
terminations, connecting and glanding material, cable protection tiles, cable
markers, transit blocks, nameplates, and all other items, which are necessary for
complete installation of the telecommunication systems encompassing
telephones, CCTV cameras, associated peripheral equipment and accessories
3. Mounting and installation of all field cameras, and associated devices through
marshalling and auxiliary cabinets, power distribution boards, local panels,
junction boxes etc.
4. Mounting and installation of all telecommunication associated assemblies, poles
and supports, push buttons, switches, junction boxes etc.
5. Installation, laying and termination of wires and cables, to all cameras,
telephones etc at all building, perimeter wall and gates.
6. Inspection, testing and commissioning of all telecommunication systems,
associated equipment and related accessories.

7. CONTRACTOR shall ensure that Telecommunication VENDOR's


representative is present to provide assistance and witness the Site
Acceptance Test and COMMISSIONING of the equipment.

11.7 FIRE AND SAFETY SYSTEMS


The CONTRACTOR shall design, construct and handover a fully equipped FACILITIES to include
the following:

1. CONTRACTOR shall provide and install the fire and safety system in accordance
with the specifications and drawings, MTO, codes and standards. Any necessary
items to meet the minimum safety requirements shall be provided by the
CONTRACTOR. Constructability Study,
2. CONTRACTOR shall provide durable frame „layout plaques‟ showing location of
Fire and Safety equipment together with escape route in the Plant as per approved
drawings and to be installed at strategic locations as directed by PMT.
3. CONTRACTOR shall provide adequate numbers (to be advised by PMT) of plastic
"layout plaques" showing location of Fire and Safety equipment together with
escape route as per approved drawings and to be installed at strategic locations as
directed by PMT.
4. CONTRACTOR shall mount all fire extinguishers adjacent to but clear of walkways.
5. CONTRACTOR shall design, supply, install, test and commission complete Fire
Fighting and Fire Prevention System for the FACILITIES
6. The overall design of the Fire Fighting and Fire Prevention System shall be in
accordance with the specification.
7. CONTRACTOR shall test and commission fire alarm system by using CEM.
8. Fire Fighting and Fire Prevention System for FACILITIES shall comprise of but not
limited to the following major sub-systems and components (if require):
• Fire water
• Fire Water Hydrant
• Hose Reel (c/w pressure regulator)
• Underground fire water network
• Carbon dioxide Portable 6kg size
• Dry chemical Portable 10kg size
• Smoke detector
• Heat Detector
• Fire Blanket
• First Aid Box
• Emergency Eye shower

9. Each type of SYSTEM/ EQUIPMENT shall be selected according to the hazards


present and shall be located strategically at certain intervals to ensure optimum
fire safety coverage is achieved.

10. The engineering activities and the corresponding deliverables shall include but not
limited to the following:

i. Engineering specifications for firefighting equipment, fire alarm, break glass/


manual call point and smoke detector

ii. Firefighting, fire alarm system, smoke detector equipment datasheet.

iii. Firewater supply system and Firewater Distribution/ fire water network
P&IDs;

iv. Fire and Safety Equipment Layout/drawings for fixed firefighting system,
portable firefighting equipment and fire alarm, smoke detector.

v. Escape route and Muster station Layouts; and

vi. FM200 layout

11. Any other Firefighting and Fire Prevention deliverables; drawings,


SPECIFICATIONS, studies, and analyses which are required for successful
completion of the FACILITIES.

11.8 SURFACE PREPARATION AND PROTECTIVE COATING

1. CONTRACTOR shall provide materials, equipment and labors, to perform all


surface preparation, painting and inspection of structural members, mechanical and
piping, instrumentation, miscellaneous items and all other facilities to complete
WORKS.
2. CONTRACTOR shall make available on site the painting/coating manufacturer's
representative to provide all supervisions and technical advice during all phases of
painting/coating.
3. CONTRACTOR shall carry out WORKS in such a manner and sequence that
surface preparation and painting can be carried out efficiently, with minimum
disruption to other trades, and with minimum amount of "touch-up" work required.
COMPANY shall have the right to inspect all painting/coating visually whilst the
paint/coat are being applied and after the paint/coat have been completed in order
to check whether painting/coating meet the CONTRACT requirement.
4. CONTRACTOR shall complete the painting of piping after non- destructive and
hydrostatic testing have been completed and accepted by PMT Representative.
5. CONTRACTOR shall consider severe corrosive atmosphere and shall be
responsible for surface preparation and painting of all equipment and fabricated
items in accordance with the approved procedure and applicable codes & standards
to the satisfaction of PMT. All blasting works for surface preparation shall be by grit
blasting using approved grit materials and the Painting Standards to be adhered to.
Any abrasives plan to be used for blasting activities shall undergo abrasive test in
order to determine the suitability and effectiveness. CONTRACTOR shall ensure
that correct size of abrasive is being used. Due to safety and health concern, copper
slag shall not be used.
6. Design requirement for painting and coating for all Civil and Structures
items/facilities shall be in accordance with ISO 8501, ISO 8502, ISO 8503, ISO
8504, and ISO 12944-4 respectively

12.0 PRE-COMMISIONING AND COMMISSIONING

A successful PRECOMMISSIONING and COMMISSIONING Program for the PROJECT will


require attention to detail and close coordination between the Project Management, Engineering,
Construction, Pre-COMMISSIONING and COMMISSIONING teams to successfully produce a
safe and reliable facilities on schedule and within budget:

The requirement shall include but not limited to:


a) Preparation of COMMISSIONING & Start-up Execution Plan;
b) Preparation of Project Specific Testing, Pre-COMMISSIONING &
COMMISSIONING Procedure;
c) Preparation of Initial Start-up Plan;
d) Vendor mobilization Plan
e) Training plan
f) Preparation and submission of commissioning philosophy during detailed
design
g) All electrical system testing, pre commissioning and commissioning shall be
made by a certified 3rd party Electrical Service Contractor established in Iraq.
h) Electrical protection study, relay calibration and testing shall also be
conducted by a certified 3rd party Electrical Service Contractor established in
Iraq.
i) CONTRACTOR has established the communication with all relevant
stakeholders and obtained all necessary approvals.
j) CONTRACTOR shall supply all necessary materials, personnel, etc. for
COMMISSIONING and Start Up.

12.1 General Pre-Commissioning Requirement


The CONTRACTOR shall prepare, and submit for PMT and PCIHBV’S approval, a
PRECOMMISIONING Plan outlining the approach and methodology for PRECOMMISIONING
of all SYSTEMS and FACILITIES thirty (30) days before the targeted MECHANICAL
COMPLETION. The CONTRACTOR will provide PRECOMMISIONING and COMMISSIONING
Manager and sufficient qualified CONTRACTOR PERSONNEL in the applicable disciplines as
needed with extensive experience to complete specified/identified PRECOMMISIONING
activities. The CONTRACTOR is responsible for the supply of all PRECOMMISSIONING
SUPPLIES (i.e hydro testing water, power etc.) Other PRECOMMISSIONING requirements:

a) The CONTRACTOR shall provide experienced, qualified and competent personnel to


perform PRECOMMISSIONING activities and achieve MECHANICAL COMPLETION.
b) CONTRACTOR shall be responsible for coordination of interfaces and liaison with PMT
and other internal and/or external parties such as other contractors, Third Parties, etc.
c) CONTRACTOR shall provide all consumables, chemicals, catalyst, lubricants, inert gas,
packing, and other SUPPLIES and MATERIAL necessary for the
PRECOMMISSIONING and COMMISSIONING of the FACILITIES.

d) The CONTRACTOR shall organize and assign a proper PRECOMMISSIONING and


COMMISSIONING Team at the WORKSITE, which shall include but is not limited to
COMMISSIONING Engineers for all relevant disciplines, and adequate Supervisors and
Technicians for all relevant disciplines, no later than the start of PRECOMMISSIONING
WORK.
e) The CONTRACTOR shall perform the inspection of the GOODS, field tests and field
work with the representative of PCIHBV or any third party authorized by PMT. Any
GOODS, testing or work which does not comply with CONTRACT requirements shall
be rejected. Such field inspection by PMT REPRESENTATIVE shall not relieve the
CONTRACTOR from its responsibility for faulty workmanship, GOODS, or compliance
with all CONTRACT requirements in accordance with EXHIBIT I - SCOPE OF WORK.
f) The CONTRACTOR shall perform co-ordination with all relevant internal and external
parties, including but not limited to VENDORS, SUBCONTRACTORS, GOVERNMENT
bodies, PMT’s third party contractors.
g) The CONTRACTOR shall ensure successful PRECOMMISSIONING and completion
activities and shall perform all activities and procure all items required in respect thereof.
h) The CONTRACTOR shall be responsible in the mobilization and demobilization of all
VENDORS, SUBCONTRACTORS and / or GOVERNMENT bodies for the WORKSITE
visit, inspection and / or COMMISSIONING.
i) The CONTRACTOR shall do cleaning up / housekeeping before, during and after
PRECOMMISSIONING activities to satisfaction of PCIHBV.
j) The CONTRACTOR shall follow VENDOR’s recommendation and procedure for
installing, checking, testing and PRECOMMISSIONING of the GOODS and/or
SYSTEMS, and shall obtain VENDOR’s agreement that the GOODS are satisfactorily
installed, PRECOMMISSIONED, COMMISSIONED and started up.
k) The CONTRACTOR shall utilize calibrated instruments which shall be witnessed by
PMT’s representative.
l) CONTRACTOR shall record in Commissioning Log Books any activities, data, running
parameters, set points and readings.
m) CONTRACTOR shall provide the sufficient number of communication equipment,
transportation and safety equipment required for COMMISSIONING the FACILITIES.
n) CONTRACTOR shall provide red line marked up P&IDs reflecting the most updated
installation WORK of the FACILITIES prior to COMMISSIONING.
o) CONTRACTOR shall operate the whole Systems, adjust and put into operation various
regimes as developed during COMMISSIONING and Start Up and shall keep the
FACILITIES fully operational and stable as per CONTRACT requirement without shut
down of the main Systems until hand over to PMT.
p) CONTRACTOR shall ensure all Equipment including stand by unit is ready during the
COMMISSIONING WORK. This is to avoid any interruption to the FACILITIES should
the primary unit need to be shut down for inspection and maintenance after certain
period of running-in.
q) CONTRACTOR shall provide VENDOR engineers for COMMISSIONING and Start Up
of the individual Equipment or package.
r) CONTRACTOR shall investigate and resolve all performance issues and deficiency
including VENDORS’ Equipment pertaining to the FACILITIES.

s) CONTRACTOR shall confirm all process instrumentation and controls including the
interlocking systems are in place and tuned properly.
t) CONTRACTOR shall provide VENDOR engineers for COMMISSIONING and Start Up
of the individual Equipment or package. (if any)
u) CONTRACTOR shall maximize the involvement of personnel from PMT during
PRECOMMISSIONING, COMMISSIONING and Start Up with full supervision from
CONTRACTOR. This however, shall not relieve CONTRACTOR from its contractual
responsibility.
v) At the end of the COMMISSIONING and Start Up, CONTRACTOR shall provide a
COMMISSIONING Report detailing the sequence of activities, changes made to
approved procedures, problem areas and remedial measures, test records, process
flow and records (pressure, temperature and flow rate). The report shall be
supplemented by drawings and photographs.
w) The integrated FACILITIES PERFORMANCE TEST shall proceed after initial operation
of the WORK has achieved the stable operations and test run on each
Equipment/package/System has been completed.
x) CONTRACTOR shall ensure that each test run is conducted in accordance with a
specific procedure to be established at the COMMISSIONING preparation stage.
CONTRACTOR shall conduct Equipment, Systems and/or FACILITIES test runs to
prove their reliability and stability. CONTRACTOR shall ensure VENDORs’ involvement
wherever applicable as per PCIHBV requirement.
y) CONTRACTOR shall conduct PERFORMANCE TEST in 72 hours uninterrupted and
maintain continuous operation at design capacities. In the case of interruption of the
PERFORMANCE TEST due to any reason whatsoever, the PERFORMANCE TEST
shall be reset / re-started. The reset / re-started of the PERFORMANCE TEST shall be
clearly described in PERFORMANCE TEST procedures and agreed by both parties.
CONTRACTOR shall submit the PERFORMANCE TEST report to PCIHBV for approval
and acceptance.
z) After the successful of PERFORMANCE TEST, demonstration of reliable operation of
plant and achievement of system completion or provisional acceptance,
CONTRACTOR shall retain key commissioning personnel in CONTRACTOR’s
commissioning team, who have full knowledge of the operation and maintenance of
work, and assist PMT or related authority for their operation work after provisional
acceptance at no cost to PMT for two (2) weeks.

12.2 PRECOMMISSIONING PLAN


The PRECOMMISSIONING Plan shall cover, as a minimum, the following scopes:
a) The PRECOMMISSIONING Organization and its administrative program;
b) Providing a detailed PRECOMMISSIONING SCHEDULE with the assistance / input
from PMT’s COMMISSIONING Team / Organization;
c) Specific PRECOMMISSIONING testing requirements for PROJECT components and
SYSTEM;
d) Incorporation of PMT's procedures and testing requirements (if any);
e) Assembly of Operations and Maintenance manuals;
f) Preparation of "Scoped" drawings / databases for turnover package development;

g) Development and preparation of the site-specific Training Program (If required).

PRECOMMISSIONING Activities

a) Mobilization of the PRECOMMISSIONING Team;


b) Receive / administer system turnovers from Construction to the PRECOMMISSIONING
Team;
c) Coordinate with construction the completion of system punch lists in preparation for
turnover to the PRECOMMISSIONING Team;
d) Coordinate and conduct the PRECOMMISSIONING testing program;
e) Coordination of the site-specific Training Program;
f) Administer systems turnover to COMMISSIONING Team.

12.3 SITE MOBILISATION


CONTRACTOR shall mobilise to WORKSITE in a timely manner.
The Construction team individual personnel (Exhibit V – Contractor’s Personnel and
Resumes shall be mobilised as required prior to actual start of Construction activities.
CONTRACTOR shall ensure that the key members of Construction team are conversant
with administration of first aid and emergency handling procedures.
CONTRACTOR shall prepare a detailed Construction Plan and submit to PMT for
approval at least one (1) week in advance prior to starting any Construction WORK. The
Construction Plan shall cover:
1. Site organisation with assigned responsibilities,
2. Constructability Study,
3. Sub-Contracting strategy for Construction work and local content percentage,
4. Construction equipment/ machinery/ tools, separate line layout of temporary
facilities including site office, fabrication workshop, NDT facilities, stores, medical,
waste disposal transportation, etc.,
5. Arrangements for Construction utilities like Construction power, water, potable
water, communication, etc.,
6. Living accommodation arrangements for CONTRACTOR personnel,
7. Permit to Work Procedure,
8. Material coordination, receiving and handling procedure,
9. Material Handling for Piping,
10. Construction detailed schedule,
11. Construction HSE and security,
12. Emergency handling and evacuation procedure,
13. Construction work procedures,
14. Construction work sequence,
15. Quality assurance control during Construction,
16. Weekly and monthly progress reporting and verification format,
17. Other inspections and field change formats.

12.4 SITE ORGANISATION AND FACILITIES


CONTRACTOR shall organise a qualified and competent Construction team headed by
a Construction Manager for Construction work.
The Construction Manager shall be responsible for:
1. All aspects of Construction, mobilisation and Handover of the WORK,
2. Coordination and reporting to PMT on all Construction related matters,
3. Arrangement of all necessary licenses etc.,
4. Interface management with field Sub-Contractors and agencies of other concurrent
PMT projects,
5. Coordination with CONTRACTOR Head Office for all technical and material support,
6. Implementation of CONTRACTOR’s HSE & Security Policy and HSE & Security
Plan,
7. Quality Assurance and Quality Control during Construction,
8. To ensure compliance with Iraq Laws and Regulations during Construction phase.
Construction site facilities will include, as a minimum:
1) Site office with conference room,
2) Fabrication workshop with lay down facilities,
3) Material stores (as well as fenced yard covered storage),
4) First Aid/ Medical Room, Paramedic staff and ambulance,
5) Area lighting, HSE Sign boards, Fire Protection, Equipment, Telecommunication
Equipment,
6) Grit blasting and painting shop,
7) Toilet, washing rooms, change room,
8) Vehicle and Construction equipment maintenance facility,
9) Security & surveillance facilities
CONTRACTOR Site Office shall be equipped with the following office facilities, as a
minimum:
1) High speed heavy-duty photocopier machine (A4, A3 size) with auto feeder and
sorting facility,
2) Personal Computers with printers, rewritable CD writer and scanner,
3) Writing Boards,
4) Telephones with international dialling facility and telecommunication radio base
stations,
5) Local area network and E-mail with Internet access for data transmission,
6) First Aid Kits,
7) Notice Boards,
8) Secretarial assistance.

12.5 SITE SURVEY AND ACCESS


CONTRACTOR shall accept the WORKSITE “As is, Where is Basis”.
It is responsibility of CONTRACTOR to carry out an extensive site verification,
topographic/ boundary survey and underground mapping survey to verify the actual site

conditions to familiarise with the WORKSITE and the other PMT facilities/ projects, which
are associated with the Scope of Work.
At the WORKSITE, CONTRACTOR shall erect a painted wooden notice board giving the
following information:
1) PMT Name,
2) CONTRACT Title,
3) CONTRACT No.,
4) Consultant Name,
5) Main CONTRACTOR Name,
6) Sub-Contractors Name,
7) Site Location Name,
8) Additional information as directed by PMT.

Information shall be stated in English. Notice boards shall be self-supporting and have a
minimum size of 2m x 2m.

12.6 SAFETY AND SECURITY


CONTRACTOR shall take sole responsibility for safety and security of the WORK and
shall comply in all respects with the requirements of Project Plan. The scope of safety
and security shall include the following:
1) Providing safety and fire prevention procedures at WORKSITE,
2) Providing emergency evacuation and first aid facilities in accordance with the Iraq
Laws and Regulations, for all personnel assigned to the WORKSITE,
3) Providing and implementing adequate safety management and personnel, safety
procedures, safety plans, safety organisation charts, safety instructions, and
emergency response plan and safety talks,
4) Supervising all personnel assigned at WORKSITE to ensure that they abide by
Project HSE Plan and procedures,
5) Regularly verifying adequacy of CONTRACTORS and Sub-Contractors crane-age,
rigging and scaffolding to function safely and reliably in compliance with PMT
requirements,
6) Maintaining safety records,
7) Issuing safety report to PMT as part of progress reports on a weekly basis,
8) Provide personnel safety apparel to all its personnel and ensuring its Sub-
Contractors and Vendors personnel are also provided likewise. CONTRACTOR
personnel shall be fully equipped with personal safety protection equipment, as
required, during performance of work at site,
9) CONTRACTOR shall also ensure that adequate first aid and medical facilities are
available at all times for both CONTRACTOR and PMT personnel. Adequate mineral
drinking water, toilet facilities and first aid equipment shall also be provided by
CONTRACTOR, at WORKSITE,
10) CONTRACTOR shall strictly comply with all local, municipal, territorial, provincial
and federal laws, orders and regulations pertaining to health and safety, which are
applicable to the location where the WORK are being performed. CONTRACTOR
shall prepare Safety Manual for PMT review and approval. The manual shall be
strictly adhered throughout the duration of the CONTRACT.

12.7 SITE PREPARATION


12.7.1 General
The CONTRACTOR shall perform the Site Preparation for the WORKSITE,
This work involves clearing, grubbing and stripping topsoil in the areas designated
hereunder and/ or shown on the drawings and as per directed by the PMT. The work
shall also include the demolition and disposal of structures except where otherwise
specified.

12.7.2 Existing Structures


All fences, buildings, structures, and encumbrances of any character within the limits of
the proposed Project, except those to be removed by others or designated to remain,
shall be demolished and removed by the CONTRACTOR.

12.7.3 Disposal of Trees and Unwanted Material


All equipment, labour and material required to load, unload, transport and store material
shall be supplied by CONTRACTOR.

12.7.4 Drainage during Construction


Similar to existing structures.

12.7.5 Slope Protection


Slopes shall be restored to their original condition or as otherwise approved by PMT.
CONTRACTOR shall ensure that all slopes are geotechnical stable.

12.7.6 Clean-Up
CONTRACTOR shall clean-up the WORKSITE and any additional areas used in any
phase of the WORK in a manner satisfactory to PMT, all landowners, tenants, and any
authority having jurisdiction.

13.0 PROCUREMENT
General
Procurement includes all activities to perform purchasing, expediting, inspection, quality
assurance, storing, transportation, insurance, payment, importation, tax exemption
application, customs clearance and related services required for the equipment,
materials and completed of the WORK.

13.1 The CONTRACTOR’S Responsibilities


The CONTRACTOR shall procure all GOODS required for the WORK, and shall also be
responsible for the transportation, freight forwarding, custom brokerages, labor,
subcontracting, supplies, and services in respect thereof.

The CONTRACTOR shall prepare bid requests, bid evaluations, requisitions and
purchase orders in all disciplines for all GOODS. The CONTRACTOR shall review, revise
and accept vendor data in all disciplines in respect of the GOODS, concurrent with PMT
review and acceptance.
PMT acceptance of VENDORS/SUBCONTRACTORS shall be required prior to issuance
of Request for Quotation (RFQ). This requirement is not limited to only those items noted
on the equipment list. Any changes or substitutions shall be approved by PMT.
The CONTRACTOR shall provide to PMT copies of un-priced purchase orders placed by
the CONTRACTOR for the procurement of GOODS and services.

13.2 Local Content of Scope of Work


The CONTRACTOR and SUBCONTRACTORS shall maximize the utilization Iraqi
products and services for any application for which these products or services are found
to meet the requirements of the CONTRACT, all technical specifications thereunder, and
international safety and quality standards.

13.3 Government Regulations


The CONTRACTOR shall procure GOODS and services in accordance with the
applicable laws and regulations of the Republic of Iraq, and shall pay particular attention
to related restrictions regarding the use of Iraqi products and services.
The CONTRACTOR furthermore affirms its understanding of the procurement controls
imposed upon PMT by the various agencies of the Iraqi Government and accepts full
responsibility for meeting the requirements so placed upon PMT.

13.4 Procurement Services


The CONTRACTOR’s procurement services shall as a minimum include:
i. A dedicated procurement organization team as part of CONTRACTOR’s team at
Engineering and Procurement office to handle all procurement matters in relation
to this CONTRACT;
ii. Procurement of all equipment and material required as per the approved
purchase specifications, datasheets and PMT requirements;

iii. Procurement of material for capital, construction, installation, tie-ins,


PRECOMMISSIONING, COMMISSIONING, SUPPLIES, SPARE PARTS, first
fills (fuel, water, fire water, chemicals and lubricants), special tools and
handling equipment whichever required and recommended by VENDOR;
iv. VENDOR documentation and control;
v. Inspections, expediting and certification of equipment and material;
vi. Customs clearance, including tax exemption formalities;
vii. Overland, sea or air worthy packing, marking, freight forwarding and
transportation (with insurance), as required;
viii. Monitoring of Procurement activities and other procurement procedures
necessary for the successful completion of the WORK;
ix. Requisition of VENDOR services required for field assistance during
Construction, PRECOMMISSIONING and COMMISSIONING phases.

Requisitions and documentation of selected VENDORS for the GOODS/services


purchase orders shall be submitted for review by PMT.
The above procurement services and necessary procurement obligations described in
this section shall also be extended to monitor, control and manage the VENDOR for
PMT’s supplied materials to ensure that there is no delay to WORK SCHEDULE with all
GOODS are available at the WORKSITE in a timely manner.

14.0 PROVISIONAL ACCEPTANCE


14.1 PROVISIONAL ACCEPTANCE DATE
1) The PROVISIONAL ACCEPTANCE Date is considered as the date of the
PROVISIONAL ACCEPTANCE Certificate by PMT.
2) As soon as the CONTRACTOR has completed the WORKS and is ready for use
and put in service by PMT, the CONTRACTOR shall issue written Handover Notice
to PMT specifying the Handover Date for the Facilities. PMT shall promptly perform
such inspection and test of the WORK as it deems appropriate and, if agreed by
PMT, within ten (10) working days of receipt of CONTRACTOR’s written notice,
PMT shall counter sign the Handover Notice to confirm the Handover Date.
3) Upon approval of Handover Notice, PMT shall issue PROVISIONAL ACCEPTANCE
Certificate which signifies PMT’s acceptance of the WORK with exception to any
deficiencies attached thereto which CONTRACTOR is still obliged to remedy under
the CONTRACT.
4) The PROVISIONAL ACCEPTANCE Certificate for the WORK shall not be issued
until the CONTRACTOR has been completed and meets and satisfies the following
criteria:
a) It conforms to the CONTRACT requirements and all specifications and
Drawings referred to in this CONTRACT or thereafter furnished and approved
by PMT in accordance with this CONTRACT,
b) It is free of any defect in fabrication, installation, construction on workmanship,
other than a minimum number of minor defects subject to prompt correction
and that said minor defects are fully described and attached to the said written
notice as a Punch List,
c) It meets all requirements of a certifying authority, if such a certifying authority
has been appointed by PMT,
d) Equipment, material and supplies furnished by CONTRACTOR for the WORK
are in accordance with the CONTRACT specifications, and are fit for the
intended purposes,

5) CONTRACTOR shall, within thirty (30) calendar days from the date of
PROVISIONAL ACCEPTANCE Certificate for the WORK, submit to PMT a Work
Closure Certificate stating outstanding punch-list items and billings under the
CONTRACT for PMT’s approval. Such items may include outstanding billing or
payment of CONTRACT PRICE, Change Order, reimbursable and other charges.
CONTRACTOR shall expeditiously resolve whatever outstanding punch-list items,
billings and any other obligations under the CONTRACT.

6) PROVISIONAL ACCEPTANCE shall be as per Article 49 Acceptance of the Work.

13.2 PROVISIONAL ACCEPTANCE DOSSIERS


CONTRACTOR shall prepare PROVISIONAL ACCEPTANCE Dossiers for the WORK.

The PROVISIONAL ACCEPTANCE Dossiers shall include the following documentation,


as a minimum requirement

COMPLETION Certificate with a “Critical Punch list” clearance acknowledgement signed-


off by PMT.

1) Non-Critical Punch-list;
2) Record of site modifications in “AFC” drawings and documents (Red-Line mark-up);

14.3 OPERATION/ FACILITY IN SERVICE


Once the PROVISIONAL ACCEPTANCE Certificate for the Facilities is issued by PMT
and received by the CONTRACTOR, then the WORK is considered fully handed over to
PMT and ready to be put into service.

15.0 WARRANTY
15.1 The Warranty Period shall be effective from the date of PROVISIONAL ACCEPTANCE
Certificate for a period of twelve (12) months and shall be as per Article 52 – Warranty
Period.
15.2 For the fulfilment of CONTRACTOR obligations throughout the Warranty Period,
CONTRACTOR shall:
15.2.1 Submit for PMT review the proposed warranty team and warranty procedure two
(2) weeks prior to the start of the Final Inspection.
15.3 Within thirty (30) days from the end of the Warranty Period of the WORK and after all
repairs, rectifications, replacements and making good thereto have been carried out in
accordance with CONTRACT, and all requirements of the CONTRACT have been
satisfactorily met, CONTRACTOR shall issue the Contract Closure Certificate.

16.0 FINAL ACCEPTANCE


16.1 FINAL ACCEPTANCE shall be as per Article 49 – Completion and Acceptance of
Work.
16.2 CONTRACTOR shall apply for final completion of the WORK when CONTRACTOR
considers it has completed the WORK in all respects that it fully meets and satisfies the
requirements of the CONTRACT and the following additional criteria:
1) All documentation required by this CONTRACT has been delivered to and received
by PMT,
2) hand-over of complete Final Documentation by CONTRACTOR to PMT’s
satisfaction and completion of DEFECTS LIABILITY PERIOD. The final Index for
the Final Documentation shall be proposed by CONTRACTOR and approved by
PMT.
3) All Punch List WORK items have been completed to PMT’s satisfaction,
4) CONTRACTOR shall submit to PMT the Contract Closure Certificate within thirty
(30) days from the end of the Warranty Period (refer Article 73 Liens and Claims
and Exhibit IV - Administrative Procedures (Appendix IV-K));
5) The Warranty Period, pursuant to Article 73 - Liens and Claims has ended and no
further warranty issues are outstanding.
16.3 Once CONTRACTOR has to apply for final completion above, PMT shall within ten (10)
working days of receipt of such notice, either issue a FINAL ACCEPTANCE Certificate

or otherwise inform CONTRACTOR in writing of PMT’s reasons for not accepting the
WORK. CONTRACTOR shall continue in this fashion until all deficiencies have been
corrected to PMT’s satisfaction and a FINAL ACCEPTANCE Certificate has been issued.

17.0 DEMOBILIZATION
17.1 Upon the PROVISIONAL ACCEPTANCE of WORK by PMT, CONTRACTOR shall
demobilise, which shall include, but not limited to:
1) Dismantling and removal of all temporary facilities (Site Office, Workshop, Stores,
Camp accommodation etc.) and utility systems;
2) Removals of all balance CONTRACTOR Equipment, Machinery, Tools, Rigging,
Cranes, Scaffolding etc. from WORKSITE;
3) Hand over all surplus equipment and material to PMT wherever applicable (i.e. PMT
Free Issue Material);
4) Reinstatement of the areas of temporary facilities to a clean and tidy condition to the
satisfaction of PMT;
5) Reinstatement of any damaged facilities (such as existing infrastructures, etc.)
caused by CONTRACTOR during execution of the WORK.
17.2 CONTRACTOR is required to mobilise required personnel, equipment and facilities to
perform the Warranty Work at WORKSITE during Warranty Period.

18.0 PROJECT DOCUMENTATION


18.1 The Exhibit IV - Administrative Procedures broadly describes the scope of Project
Documentation together with the basic instructions, procedures and other requirements
to be performed by CONTRACTOR. Clauses below explained general requirement for
Project Documentation which shall be complied by the CONTRACTOR.
CONTRACTOR shall be responsible for preparing and submitting the Final
Documentation as per industrial standard practice.
18.2 DOCUMENTATION DURING PROCUREMENT AND CONSTRUCTION PHASES
16.2.1 The documents and drawings, which are to be submitted to PMT for review and
approval CONTRACTOR shall submit three (3) sets of the required
documentation for PMT review.
16.2.2 PMT approval cycle shall be seven (7) calendar days from the receipt of the
document. CONTRACTOR shall plan and schedule all related Project activities,
taking into account the seven (7) calendar days PMT approval cycle. PMT shall,
however, endeavour to review critical documents on priority, with shorter
approval cycle, to the extent possible.

18.3 FINAL DOCUMENTATION


16.3.1 After successful completion of the WORK, CONTRACTOR shall Handover the
Final Project Documentation to PMT as outlined below:
1) The number of sets of Final Documentation to be handed over, shall be:
 Hard Copy - Three(3) Sets,
 Electronic Copy - Three (3) Sets. Native Files
 Electronic Copy - Three (3) Sets. PDF Format

2) Hard copies shall be properly labelled, bounded with 3-ring hard-cover files
with complete documentation index in each volume, in addition to the index
of the respective volume.
3) Draft of Main Table of Content (MTOC) and Volume Table of Content (VTOC)
shall be made available three (3) months after mobilization to site, or after all
POs has been issued, whichever is later.
4) CONTRACTOR shall submit As-Built of all drawings and the As-Built 3D
PDMS model before handover to PMT. These As-Built shall be re-viewed with
CONTRACTORS discipline engineers before they are issued as final to PMT.
5) CONTRACTOR shall also be responsible for providing manuals and other
training aids such as videos for operations & maintenance training where
applicable in dual language i.e. English and Arabic.
6) Unpriced purchase orders for all other items and bulk items shall be included
in final documentation.
7) The above purchase orders shall not be subject to any review, audit or
reimbursement and shall be submitted for PMT’s future reference only.
8) Electronic copies shall be on CD or DVD with labels and List of Contents.
9) The Final Project Documentation shall include, but not limited to, the
following:
 As-Built Documents and Drawing (ACAD and PDF),
 Construction Documentation and Records,
 FIELD TEST Records.
 Coring Test Records
17.3.2 Purchase orders for all bulk items will be included in final documentation.
17.3.3 The above purchase orders shall not be subject to any review, audit or
reimbursement and shall be submitted for PMT future reference only.
17.3.4 FINAL ACCEPTANCE Certificate shall be issued by PMT, only after Handover
of complete Final Documentation by CONTRACTOR to PMT satisfaction and
completion of Warranty Period.

19.0 PROJECT CLOSE OUT


19.1 CONTRACTOR shall be responsible for preparing the Close-Out Report after completion
of the WORK.
The final Project Close-Out Report shall be submitted within two (2) months after
issuance FINAL ACCEPTANCE Certificate. This Project Close-Out Report shall
incorporate documents, data, PMT comments, etc which has been developed during the
execution at the WORK. The report should be updated and compiled progressively so
that it could be completed on time.
a. Project Close-Out Report will include an updated PROJECT SCHEDULE,
S-curves, histograms, and material tracking record showing scheduled
versus actual data.
b. The close-out report shall generally cover the following aspects of the
PROJECT:
• Project Overview and History
• Chronology of Event and Major Milestone Achievement
• HSE performance against project target.
• QA/QC performance against project target

• Technical aspects and deviations from design basis / intent, if any.
• Facilities performance against design.
• Project challenges, lessons learnt and recommendations for future
projects.
