Achievement Requirements DIUED


To achieve Accreditation, a Facility must receive a:

• Met or Met with Recommendation rating for all LA and SA elements with the exception of up to a maximum of
6 (total) LA or SA elements with NM ratings with Moderate or Low risk only (no P1 Recommendations), and
• Met or MR rating for all MA elements.

Duration Limit
There is no limit on the number of Cycles for which a Facility can maintain Accreditation. Once a Facility
achieves Accreditation, a Foundation or Recognition Achievement is not permissible in the future.

Final Assessment
During an Accreditation Cycle, a Final Assessment will be triggered after an OWA or SNA if any of the
following apply:
• More than 6 (total) LA or SA elements are rated NM or any LA or SA elements are rated NM with a High or
Extreme Risk
• Any MA element is rated NM, or
• Priority 1 recommendation received

The scope of the Final Assessment will include any elements rated NM, any LA or SA element rated MR,
and all Priority 1 recommendations. If the requirements cannot be met after a Final Assessment, then
Accreditation will not be granted and the Facility can attempt another OWA at any time in the future.
