2. Identify key sources of national, local and state health care data.
3. Design and develop an assessment and evaluation plan for your needs assessment;

Priorities Table (first four columns only)
INSTRUCTIONS: Please follow the instructions as outlined below. You may use ALL resources from Modules 1-5 to assist you. Please look back at your
journals and comments to assist you.
A. Define your targeted population (Area I & Standard II):
1. Location: Greece Arcadia High School 120 island cottage road Rochester, NY 14612
(Monroe County).
2.Who are they: Greece Arcadia High School is a Public High School.
3. Mission/Vision:
The mission of Greece Arcadia high school is to provide each student with many opportunities to be successful. We understand that success
comes in a variety of forms and achieving goals is an ongoing process. We focus on building positive relationships with our students and provide
a safe and engaging atmosphere. We create opportunities to explore all academic and career pathways available. Our goal is to have each student
leave Arcadia with a plan for a successful and fulfilling adult life.
4. What does the school provide to the community?
Arcadia high school and Greece central school district is a great place that has been growing their Community Schools strategy for the last
several years. Greece is passionate about building deep connections with families and creating strategic partnerships so that their students can be
happy and successful, families can be strong, and the whole community can thrive.
5. Describe what the research showed about your targeted population for assets (strengths) and areas of growth (needs).
Research showed that students at Greece arcadia High School have multiple opportunities to be successful within the classroom as well as out.
There are classes that student can take to go straight into the working field once they leave high school. The students take classes off of campus
that helps to certified them in the job field they desired. Students also get a chance to earn college credits that are connected to some of class, they
take at the school which are dual credits. Students also have the opportunity to participate in extracurricular activities such as sports, clubs, and
field trips. Several students attend top universities across the country and earn academic and athletic scholarships. However, research shows that
mental health, drugs and alcohol are a huge deal when it comes to the high school level. There are more and more students using inside and
outside of school which is affecting their mental health. There are also students that come from low-income families or families that are English
language learners. Many students don’t have the support they need which leads to unwanted behaviors as well as poor metal health.
6. Contact Information (name, email, phone):
The main contact person for this school is the principal, Christina Wawrzyniak. She can
be reached at Christina.wawrzyniak@greececsd.org (585) 966-3000.
B. What is the goal of your needs assessment (Area IV)? The goal of my needs assessment is collecting data on mental health and drugs/ Alcohol use
of high school students (9Th-12Th grade) at Greece Arcadia HS, located in Rochester, New York. The data that is collected will be used to identify
what services and resources the school must be incorporate for the students. According to the YRBS and data collected Greece arcadia suffers
from a history of mental health and drug addiction within the school and at home there has been family members that are showing the same
pattern as well. Students may have to take care of their family members who have this problem.

D. Determine a timeline (Area IV): Timeline: This needs assessment will be implemented for the 2024-2025 school
year.
Beginning of school year (September 2024) – Students will complete a brief survey on their behavior within the past year.
Beginning of October 2024 – The survey will be analyzed and discussed with the important team
members.
End of October 2024 – Determine how important mental health and substance /drug use
needs are within the school. Find additional sources that help students with support and present the importance of health education programs that
help with students dealing with these issues.
Beginning of November 2024 – Present the needs assessment and research to broad of education and Greece central school superintendent.
End of November 2024 – Get the yearlong health education curriculum approved that focuses on
mental health needs, drug/alcohol use.
December 2024 – February 2025– Create a yearlong health education program that focuses on the needs of mental health and substance/drug use.
March 2025 – Ask the health education teachers and center of youth staff as well as any other employee associated with health education to
participate in professional development trainings, seminars and over the summer trainings.
April 2025 – Organize additional health services that students will have the opportunity to utilize. This includes but is not limited to counseling,
support groups,
May 2025 – Create a mindfulness for the body program for students regarding mental health needs, and drug/alcohol use.
September 2025 – Begin the yearlong health education program with the focus on mental health
needs, and drug/alcohol use and start the mindfulness and body program.
E. Decide where, how and from whom you will obtain the information (Area I and Standard II):
1. What did the professional literature say about your site and their needs?
Greece arcadia suffers from 3% individuals within the home have history of mental health or suffer from it. Also, not having both parents
living within the home that is 43%. Student also have had difficulties with their emotions which is 33%. The students that have felt sad in
2+ weeks is 29%. Students also students have answer that they have considered suicide within the past year this is 15% and the attempt
of suicide is 7 % within the past year. Students have also used drugs such as tobacco 10%, vaping 8%, marijuana 10%. 55% of students
answered that they had school encouragement.2% of students said there was a history of adult physical altercations in home.3% of
student has experienced physical abuse. 7%of students said that family does not give support.
2. What type of data will you be collecting? Primary, secondary, qualitative, quantitative?
I will be collecting secondary data through the research and evidence I find to support the idea of a yearlong health education program with a
focus on mental health needs and drug/alcohol use. Qualitative data will also be collected through the behavior of the students based on their
view on their mental health status and feelings.
3. How will you collect the data – I will collect my data through a secondary source which is YRBS district, county, SAMHSA, and BRFSS
4. Where will you collect this data?
I will collect this data in this data from the Recent YRBS survey that was given to the students at the beginning of the year. The students would have
needed access to a computer and link to get to the survey.
F. What resources are needed to implement your needs assessment (Standard II & Area I)
In order to implement my needs assessment, I will need to make sure I have access to the school secondary data from their YRBS survey so that the needs to
the students with in the school are up to date. Students will also need to have access to a computer or library computer if they woul like to make sure they are
apart of the anonymous results that come from the school through the county.
• Who will be responsible for obtaining the information (Standard II & Area I)?
• Matt– High School Physical Education Teacher
• Kristine – High School Health Teacher
