SUBJECT: INCIDENT REPORT

DATE: 01 December 2022, around 1830H-1900H

To Whom it may Concern,

This is to inform you regarding the incident happened around 1830H in the evening, night duty.
Around 1800H, Transport driver Mr. Bobby requested to check the steering of KIA Karga Plate #
(NEU 8425). The available SMCC mechanic who checked was Mr. Sonny boy Machon and Mr. Randy
Ablaza together with the help of Mr. Salvador Arcenal and their SGDC assistant mechanics: Jeremy
Villanueva and Jophel Fajardo. The mechanics has noticed the damage of moving cross-joint
assembly by checking-up the service and decided to send the service car to the elevated service bay
to be parked because it is unsafe to drive. After thorough assessment, Receptionist on duty requested
the sample of cross-joint to be ordered immediately so mechanics-on-duty pull-out and
disassembled the cross-joint assembly to get the spare sample. In-order to take-out the spare parts,
they need to turn the wheel on to other side to easily pull-out the assembly, as per instruction of
SMCC Mechanic it is needed to turn-on the engine to power the power-steering and manage to
maneuver the steering. Assistant Mechanic Jophel shifted the knob on neutral and turned-on the
ignition. The mechanic under-neat the service working in the cross-joint shouted “Lagay mo muna sa
Neutral, Peng” (pertaining to instruct the helper before turning on the ignition to switch gear to
Neutral). Unfortunately, as per start-up of engine the shifter did not switch to neutral gear as it is
supposed to and the clutch pedal released causing sudden acceleration of the service through the
elevated platform. Luckily, no one was injured and they managed to escape as the service wildly
crashed through the platform. Causing minor damages on the unit. The service was now recovered
immediately from the crash and parked to the platform after retrieval of the service for assessment of
the damages incurred.
