HEALTH & SAFETY PLAN

OVERVIEW OF CONSTRUCTION WORK

PROJECT: Luano City Development

LOCATION: LUBUMBASHI, DRC

1. PROPOSED ACTIVITY

This contract consists of Luano City development (new residential units) in Lubumbashi, DRC.

2. GENERAL HEALTH & SAFETY PROVISIONS

 Notification of Intention to Commence Work - ANNEXURE A, as Principal Contractor.


 Appointments of PC Responsible Persons to Supervisor Construction Work.
 Appointment of Contractor’s Responsible Persons to Supervise Health and Safety on site.
 Compensation of Occupational Injuries and Diseases Act 130 of 1993 (COIDA), Registration - WCA or
FEM.
 Occupational Health and Safety Policy.
 Company organogram - is on Safety File.
 Recording of Incidents and Accidents - Annexure 1 GAR 9.

3. GENERAL HEALTH & SAFETY PROVISIONS

3.1 A risk assessment of the intended work has revealed the following, but not limited to, potential risks:

 Working on ladders
 Brick work
 Plastering
 Scaffolding
 Electrical installation
 Allocated delivery times as well as demarcated “On & Off” loading area’s
 Stacking & storage of materials
 Portable electrical tools
 Hand tools and extension leads
 Power tools (including jackhammers)
 Finishing trades

4. SPECIAL HEALTH & SAFETY PROVISIONS

4.1 PUBLIC SAFETY

Ready fence panels to be used to cordoned off sections where construction will be taking place.

4.2 TRAFFIC CONTROL

Speed restriction (15km/h) will be obeyed at all times.

5. SAFE WORK PROCEDURES

Safe work procedures will be developed and in response to all hazards imposing high or medium risks on the
employees, operators or members of the public. These safe work procedures are to be practical for implemen-
tation on site. Persons who are to perform specific tasks will be instructed to do so according to the safe work
procedures.

6. HEALTH & SAFETY REPRESENTATIVES

Health and Safety Representatives will be nominated and elected by workers and appointments will be made in
writing if more than 20 workers are employed. All contractors are required appoint a Health & Safety Represen-
tative for their companies.

Not sure yet how many workers will be employed on site.

7. HEALTH & SAFETY COMMITTEE

 Monthly meetings will be held with all parties appointed and all relevant safety committee members will
be required to attend these meetings.
 A copy of these minutes will be sent to the relevant personnel.

8. TRAINING

 A start up Risk/Fall Prevention Assessment will be conducted and all workers will be notified of the
findings and required to sign a register as proof thereof.
 Work/area/activity specific risk assessments will be carried out and those workers involved/affected will
be informed of the findings and be required to sign a register as proof thereof.
 Weekly toolbox talks will be held with all Forum Properties Africa employees, and contractors are
required to ensure that similar talks are presented to all of their employees. All attendees will be
required to sign an attendance register. The register will be made available on file at the site office.
 Forum Properties Africa/LGP, as the Principal Contractor, will manage all Induction Training
requirements. However, sub-contractors will be required to notify the Forum Properties Africa site
management team and/or safety rep should any new persons be introduced to site during the course of
their involvement on site. All inductions will be recorded on the Induction training register and these
registers will be filed in the safety file on site.

9. GENERAL RECORD KEEPING

Incident and Accident Investigation Procedure:

 An incident/accident investigation team will consist of: the Site Manager, Health and Safety Officer and
Health and Safety Representative.
 On completion of an investigation, corrective measures will be discussed with the safety committee and
implemented to prevent a recurrence of similar incidents.
 Relevant documentation will be completed and submitted to the Client’s appointed Health & Safety
Agent

Record keeping and reporting:

 The Health and Safety Officer, together with the Site Supervisor - CR 6(1) appointee, will be respon-
sible for all investigations and compiling the necessary paperwork.
 Relevant documentation notifying the Compensation Commissioner will be completed as soon as
possible after an accident.
 Upon completion of investigation conducted, the Annexure 1 in terms of GAR 9 (recording and investi-
gation of incidents), will be completed and a copy will be forwarded to the Client’s Agent.
 In the event of an injury, where the employee is likely to be off work for longer than 13 days, or if it is of
an extreme serious nature (e.g. loss of a limb or part thereof or a death or near death incident), and
machinery ran out of control etc., the Department of Labour will be notified immediately together with
the Client and their appointed Health & Safety Agent, as per Section 24(1) & 24(2) of the OHS Act
detailing the circumstances under which an incident is to be reported. The person responsible for this
notification will be the Company Safety Officer.

10. INSPECTIONS, MONITORING & REPORTING

10.1 INTERNAL AUDITS

 The appointed Health & Safety Representative will be required to complete a monthly checklist which is
to be submitted to the Site Supervisor/Site Manager for assistance with implementation of non-com-
pliances where noted. This report, if deemed necessary, will be reviewed during the monthly Health
and Safety meetings. All general site inspections and equipment inspections will be undertaken as re-
quired (daily, weekly and monthly) by the relevant appointees.
 The company Safety Officer will carry out inspections per month. This will cover the administration of
Forum Properties Africa and its Contractors (Subbies) and their site hazard inspections. Records of
these inspections will be available on file on site.
 Health and Safety Representatives will be responsible throughout the day for reporting unsafe condi-
tions/acts to the appointed Site Supervisor/Site Manager.

10.2 EXTERNAL AUDITS

Forum Properties Africa is committed to the process of transformation, from non-compliance to compliance,
through a process of continuous improvement. To ensure the Company’s success in this regard, the MBSA
Safety Advisor is invited to conduct safety legislation required audits.

11. PENALTIES

12. EMERGENCY PROCEDURES

 The emergency telephone numbers will be available/posted up on site.


 The emergency procedures will be posted conspicuously near the site office, as well as any other
location deemed necessary.
 The emergency procedures will be the responsibility of LGP/Forum Properties Africa appointed site
management team to implement as/when required.

13. FIRST AID

 A fully stocked first aid box will be available on site at all hours.

 Accident/Incident Reporting and Investigation Procedures are explained in detail in the site file. A copy
is to be displayed in the site office on the notice-board.
 A Certificated First Aid Person will be appointed on site.

14. PERSONAL PROTECTIVE EQUIPMENT

 The wearing of safety shoes, hard hat/head protection and overalls will be compulsory for all personnel
on site. Visitors are required to wear closed, flat shoes and visitor hard hats will be provided.
 The use of all portable electrical power tools, generators, vibrators and dumpers will require personnel
to wear hearing protection. Where chipping or grinding is undertaken, eye or face protection will be
required.
 All mandatory PPE (safety boots, hard hat and overall) issued to Forum Properties Africa employees
will be recorded and the recipients will need to sign upon receipt thereof. Failure to wear the mandatory
PPE will result in disciplinary actions as laid out in the Company’s disciplinary codes/procedures.
 Additional PPE (goggles, earplugs, masks, etc.) will be issued by the store man with the relevant small
tool/plant.
 Contractors will be required to produce proof of the above.

15. SAFETY SIGNAGE

 Symbolic signage will be posted as required throughout the site.


 General signage will be erected to inform employees of the requirements for the wearing of safety
related equipment.
 Safety signage that will be used on site, for example “no unauthorized entry, report to site office”,
directions to site office, “beware of excavation”, “hard hat area”, report for induction prior to work
commencement, etc. Safety and Health awareness posters will also be posted at the workers change
rooms, toilets, etc.
 Health & Safety signage will be well maintained and will be inspected weekly.

16. PERMITS, WAYLEAVES

These might consist of:

 Working at heights

LGP/Forum Properties Africa shall ensure all responsible persons obtaining these permits, way leaves abide by
the safety conditions imposed thereon.

17. CONTRACTORS & SUPPLIERS

 Forum Properties Africa will evaluate all sub-contractors before they commence work on site.
 Included with the Mandatory, is a list of items required from the contractor such as proof of COIDA
registration, etc. - copies thereof will be available in the Health & Safety File.
 Each contractor will be given a copy of the Health & Safety Specification, in order to make adequate
provisions for health and safety related needs.
 Site Induction will be conducted by the Forum Properties Africa Health & Safety Rep. However, every
contractor is required to produce proof of their company’s trade specific induction.
 Contractors are required to ensure that a representative attends all safety meetings and said person is
to be appointed as a Safety Committee member.
 Forum Properties Africa Health & Safety Officer will audit contractor’s Health and Safety Files & Plans.

 Contractors are required to ensure that all non-compliances be addressed timorously.


 All contractors will be required to manage their own Health and Safety, as well as the safe keeping of
records on file as required under the OHSACT.

18. PROPOSED SPECIALISED APPOINTED CONTRACTORS

 Fire Services, etc.


 Plumbing installations, etc.
 Electrical installations, etc.
 Fire Protection services with complete suppression installation, etc.

19. STACKING & STORAGE

 An appointed stacking and storage supervisor will manage the entire site as far as stacking and
storage is concerned. All material to be stacked on site will be well managed at all times. Material may
not encroach onto any roadway adjacent to the site and/or between the site and the site office. Mate -
rials may not be stacked in such a way as to risk collapse.
 All loading will be done in liaison with the appointed Stacking Supervisor.

20. WASTE MANAGEMENT

 All builders rubble due to demolition work will be transported to the designated demarcated dumping
site by means of:
 The appointed contractor will transport the rubble to a marked/allocated dump site.
 Rubble will be removed when necessary.

21. VEHICULAR MOVEMENT

Speed restriction will be obeyed at all times. Visitors and delivery vehicles will similarly be afforded the right of
way as far as possible when moving on site. Access will be controlled by means of a flagman and security
personnel with a visitors’ register to be completed.

22. HAZARDOUS CHEMICAL SUBSTANCES

 Hazardous chemical substances such as bitumen epoxies, cement paints, stabilizing agents and timber
preservatives as well as dishwasher, hand cleaner and Handy Andy, etc. will be stored in a store under
strict segregation principles, as per HCS Regulation 9-10 & CR 27. Decanting of substances will be
done in a designated area by the appointed store man.
 All relevant MSDS will be requested from the suppliers and made available at the general store and at
the first aiders station.
 Fuel storage will be in a designated camp with the relevant fire extinguisher and signage, quantities
stored on site will not exceed the allowable liters as per the bylaws.

23. ENVIRONMENTAL PROCESSES

Forum Properties Africa is committed to its Environmental Friendly Procedures and Processes and will
therefore ensure that:

 All stationary plant is equipped with a drip tray underneath.


 Where the process of mixing of mortar or concrete is undertaken the immediate surrounding area is
protected with black waterproofing plastic to serve as a catchment for all run-offs.
 A storage container is made available to store cement and other hazardous chemicals.
 The cleaning of paint brushes, mortar boards, etc. will be done in a drum and the water removed
accordingly.
 Ablution facilities will be monitored closely for any blockages and possible sewer spillage.
 In the event of any unforeseen spillage Forum Properties Africa, will ensure that the removal of this
waste is carried out appropriately and the ground rehabilitated. All documentation detailing this work
will be kept on file on site.

24. PLANT & MACHINERY

The following equipment or plant may be used on site as deemed necessary:

 Portable electrical power tools


 Scaffolding
 Ladders
 General tools
 Mobile plant
 All certified operators appointments will be specific to the plant they operate and will be in possessing
off a medical fitness card

All plant and equipment used on site will be inspected as required. These inspection registers will be kept on
file at the site office.

25. HIRED PLANT & MACHINERY

All hired plant will be inspected before being commissioned for use on site. In the event of plant and machinery
needing to be hired, Forum Properties Africa will ensure that the service provider provides the relevant test
certificates and service records. Should an operator be required, the service provider will furnish us with all
relevant competent certificates, medical fitness, etc.

26. FIRE EXTINGUISHERS & FIRE FIGHTING EQUIPMENT

Fire extinguishers will be displayed strategically throughout the site. Symbolic signage indicating the location of
the fire-fighting equipment will be displayed as well. All Extinguishers will be placed on a register in order to
monitor service dates and usability.

In case of fire – alarm shall be raised by means of a whistle – training on this will be done at Induction stage.

27. SCAFFOLDING, ROOF WORK & WORKING AT HEIGHTS

27.1 WORKING AT HEIGHTS

All personnel who work at heights (heights in this case being 3meters above ground) will have to undergo a
“medical evaluation” questionnaire - the process for evaluating their physical and psychological fitness. This
evaluation will be conducted by the HR department in conjunction with the medical fitness certification, issued
by a medical practitioner registered with the department of Health Professional Council.

 Training for working at heights will be conducted via Toolbox talks by the team supervisor and will be
task specific for the week’s activities.
 Risk assessments will be conducted and method statements draw up if there are any
 Major hazards encountered as a result from working at heights. The method statements will be
discussed with the team during the Toolbox talks.
 All build - up access routes, if required, will be provided and planned for by the scaffold erector and/or
LGP/Forum Properties Africa site management. Harnesses will be available on site at all times.
 All exposed edges will be made safe by means of hand-railing
 Penetrations will be covered or cordoned-off to prevent persons from falling or stepping into the
openings.
 All stairways will be fitted with handrails.
 The inspection registers will be kept on file for audit purposes.
 All contractors working at heights, will also have to comply with all the aspects of the OHS Act as far as
fall protection plans and safe work procedures are concerned

27.2 SCAFFOLDING

 All scaffolding will be erected and inspected by a competent specialized contractor


 The contractor will need to adhere to the requirements as listed in point 17 but not limited to their spe-
cific safety requirement as per the OHASCT
 Internal, e.g. column box scaffolding not exceeding the height of 6m will be erected and inspected by
Forum Properties Africa competent appointed staff

27.3 ROOF WORK

 All roof work will be done according to the fall protection plan and working at heights.

28. LIFTING MACHINERY & TACKLE

29. EXPLOSIVE POWER TOOLS

30. GENERAL MACHINERY

30.1 PORTABLE ELECTRICAL TOOLS

All portable electrical tools will receive regular monthly inspections by a person appointed in writing. These
registers will be kept on file for auditing purposes. All portable electrical power tools will be issued by the store
man on a daily basis, and he will ensure that all tools issued are safe for use.

30.2 ELECTRICAL RETICULATION

 The appointed Electrical Contractor will ensure adequate temporary electrical power supply to the
portable DB’s on site and furnish Forum Properties Africa with an inspection checklist.
 The said contractor will solely be responsible for all electrical commission and will furnish Forum
Properties Africa upon completion of works with a COC - Certificate of Compliance

31. PUBLIC SAFETY

The necessary symbolic signage relating to Public Safety will be displayed around site

32. NIGHT WORK

33. GENERAL FACILITIES

 General facilities will vary from project to project.


 Forum Properties Africa is responsible to ensure adequate ablution and washing facilities are provided.
These are to be allocated for use by various contractors. All contractors will be expected to commit to
using the facilities and keeping such facilities clean at all times.
 Sufficient drinking water will be available for all on site.
 Health posters will be displayed accordingly at workers mess rooms, ablution facilities, meeting rooms
etc.
 Bins for rubble and garbage will be located and marked accordingly.

34. INDUCTION

The Induction will cover items such as, but not limited to:

 Reporting of injuries sustained


By Law it is compulsory for every worker to report any unsafe acts and injuries.
 Alcohol and Drugs use
No alcohol and drugs to be used on site while working – will lead to immediate dismissal. Workers
must report for duty sober.
 Site rules pertaining to security.
All workers must obey the site rules to always assist security.
 Horse play
No workers are allowed to play on site during working hours.
 Smoking
Smoking can only be done at designated areas.
 First Aid and ablution facilities
Ablution facilities to be used on site and first aid box are in the site office.
 Medical Emergency Procedures
All workers to follow the correct medial emergency procedures and must report it immediately to the
Site Foreman.
 Evacuation Procedures
All workers to follow the evacuation procedures as displayed. Workers must report immediately at the
designated emergency assembly point.
 Fire alert by means of a whistle.
Workers to respond immediately when a fire alert is sounded and to report at the designated
emergency assembly point.
 Medical conditions (medication usage)
Workers to report any medical conditions when report for work.
 Demarcation, no unauthorised entry area’s
Nobody is allowed outside the boundary walls of their working area. This is a high-risk security area.
 Reporting of unsafe conditions and acts observed

All workers to report all unsafe conditions and unsafe acts – these must also be reported to the Site
Foreman.
 Signage
All workers to obey the signage on site and the correct PPE must be worn at all times.
 Conditions of tools
Workers to ensure that the condition of their tools is in good order to prevent any injuries.
 The use/possession of fire arms and dangerous weapons
No worker is allowed on site with any dangerous weapons or fire arms.
 House-keeping
House-keeping must be in good order at all times.
 Risk assessment training
Risk assessment training will be done on a regular basis.

The Health & Safety Plan is to be implemented by the appointed Site Manager on behalf of the LGP/ Forum
Properties Africa Directors.

All administrative systems and requirements will be kept up to date and it is the responsibility of the CR 6(1) to
ensure that all delegated authority and responsibility is taken seriously by all persons on site.

This plan has been accepted by the Forum Properties Africa Site Supervisor for implementation on site and the
requirements herein contained are understood:

35. GENERAL

 Appointments will be done with file implementation.


 Security firm will be appointed by LGP/Forum Properties Africa – to be confirmed.
 Design changes by Architect – will be done by Site Manager.
 Welfare facilities will be done by LGP/Forum Properties Africa.
 All Safe Working procedures are included in the Safety File.
