10- Flooring (4” Pouring of 1:2:4 Ratio) and also rising the height of 4 manhole covers as
per new floor level
11- Curing of New Floor with water
12- Mobilization & Placement of New manhole covers manually by manpower using wheel
barrow
13- Housekeeping and De-mobilization of material and manpower

Emergency Response / Rescue Plan for
Confined space
After make ensuring that following controls are put in place then confined space entry allowed.
● Review the rescue protocol outlined in the space specific hazard assessment.
● Inspect and set-up all needed rescue equipment. Stage all rescue technicians’ equipment at the point
ofentry.
● Our workers will enter inside tank safely when make sure all necessary reasonable control measure
is put in place.
● Stand by man (SBM)/ Hole watcher monitor the activity by verbally or visually.

Method of Rescue
The Supervisor will be responsible for coordinating any emergency response.
The N&S emergency response plan for this Tank is below.

If any worker Injured/Unconscious inside the tank, then following action should be taken

● Make verbal contact with the employee to help them stay calm.

● SBM (stand by man) stop the work and inform to the rescuers rescue the victim person.

● All rescue equipment (such as breathing apparatus) is present at nearest to entrance point to
recovering the victim person.

● Workers perform duty inside the tank free of safety belt and lifeline.

● When a victim person injured/unconscious inside the tank then rescuer’s wearing their BA set
and rescue him by either pushing/lifting to the entrance point and the other two rescuers recover
him from manhole.

● After rescue the victim person shifted to suitable fresh air environment by stretche.
● Inform to the emergency response team of SHELL all about the accident and submit one copy of
accident investigation report to management with on seven days.

● Work will continue after the situation becomes normal.

Stretcher, SCBA, First Aid Kit and a DCP Fire Extinguisher. Rescue equipment
will be set up at the work location before tank entry and will remain in position for the
duration of the work. All equipment will be visually inspected and confirmed as in
good working order prior to work beginning. Any changes necessary to adapt to
conditions on the day of the work will be risk assessed and agreed prior to the work
beginning.

Emergency Response/ Rescue Plan for Working at Height

Procedure for rescuing a suspended worker:

• All work will be immediately stopped


• Make verbal contact with the employee to help him stay calm
• The rescuers will start the rescue operation while the on-duty supervisor will inform the
Emergency Response Coordinators and the First Aid Team
• 2 rescuers wearing safety harness will climb up to the platform with stretcher
• They will attach the stretcher with one end of the lifeline
• The rescuers on the scaffold will then release the D-ring of victim’s harness to free him
from the lanyard
• The 2 on-ground rescuers will confirm that they have full control of the lifeline and that
the lifeline successfully supporting the victim
• The rescuers will ease the victim into the stretcher
• The on-ground rescuers will gradually lower the victim to the ground level
• The victim will be sat on the ground for 15 minutes with his legs stretched to avoid any
impacts from fall trauma
• The first aid team will provide first aid to the victim and if needed, the victim may be
transported toa nearby hospital
• Before shifting victim to hospital inform to terminal in charge
• The supervisor will consult the ERP before resuming any activity

ERP for Injured Person

If a worker injured at work site in depot

(1) Immediately stop the work.


Keep the victim as calm and comfortable as possible.
Do not move a seriously injured person unless there is a life-threatening situation.
(2) Our rescue team (stretcher man) rescues the victim and moves him to a safe area where first aider will
examine the victim and give him first aid.
It is important that severe bleeding be attended too quickly. Rapid loss of blood may lead to shock and loss of
consciousness; therefore, the first aider must know how to stop the flow of blood immediately.
If victim is bleeding severely from an accident, it is important that ERT to quickly assess the situation and act
accordingly.
If it is immediately apparent that professional help is needed, assign a co-worker calling for help.
(3) If the victim feeling not better, then move him to nearest hospital for further treatment.

5.How will the work be completed, i.e. Procedure step by step
Steps: 1 Joint Site Visit, Preparation, Mobilization of manpower to site

• Before mobilization of equipment joint site visit shall be conduct by Shell and operation teams to identify,
eliminate, or control the hazards related to work for preparation of MS-JSA accordingly.
• Pre-Inspection of the equipment shall be conduct by HSE, inspection witness and approve byclient
teams.
• For equipment, valid third-party certificate should be available and must be attach with work permit.
• Access & Egress routes shall be cleared & marked and communicated to all work forces.
• Working area will be hard barricaded and prohibited for un-authorized persons.
• Material Storage area shall be prepared & barricaded at the designated place.
• Equipment stand by area shall be prepare and barricaded at the designated place.

Step 2: Ensure Intimal Gas Testing by Permit Issuer before starting work

• Ensure gas test by PI / AGT before Starting work

Step 3: Shifting of material & tools to work site manually by labor and using of hand lifters and
wheelbarrows.
• Visual inspection of tools and equipment must be performed prior to before entry inside the terminal.
• Mobilize equipment and hand tools and material to site manually on wheel barrow by manpower
• Trained and competent resources must be deployed
• Ensure Equipment’s is third party certified and verified according to third party
