The Supervisor will be responsible for coordinating any emergency response.
The N&S emergency response plan for this Tank is below.
If any worker Injured/Unconscious inside the tank, then following action should be taken
● Make verbal contact with the employee to help them stay calm.
● SBM (stand by man) stop the work and inform to the rescuers rescue the victim person.
● All rescue equipment (such as breathing apparatus) is present at nearest to entrance point to
recovering the victim person.
● Workers perform duty inside the tank free of safety belt and lifeline.
● When a victim person injured/unconscious inside the tank then rescuer’s wearing their BA set
and rescue him by either pushing/lifting to the entrance point and the other two rescuers recover
him from manhole.
● After rescue the victim person shifted to suitable fresh air environment by stretche.
● Inform to the emergency response team of SHELL all about the accident and submit one copy of
accident investigation report to management with on seven days.

Method statement & JSA will be communicated to all workers in daily toolbox talk by the Supervisor. The daily
hazards & procedure shall be communicated to the workers through:
• Daily Toolbox talks on daily basis as per PTW activities.
• Daily HITS
• Compliance of LSR 100%
• PPE’s Compliance 100%
• MS-JSA explained to all workers before start of job by supervisor.
• Through CSSS and Site induction.
Safety Checks:
• Shell PTW prior to start any activity shall be received from SPL operation by designated permit
holder. Permit filled as per the approved MS-JSA. Daily renew and close by permit holder and
issuer. Shell PTW procedure shall follow.
• Permit issuers review the MS-JSA before issue of permit.
• Permit Holder to monitor all activities should be compliance as per permit and full time available at
site.
• Permit issuer to review the site activities and protocols as per the requirements.
• Ensure all the personnel are aware of work and permit requirements.
• Following documents should be attached with work permit.
• MS, JSA, Equipment Third party certificates, Manpower list etc.
PPE’s Requirement:
• Before mobilization of equipment joint site visit shall be conduct by Shell and operation teams to identify,
eliminate, or control the hazards related to work for preparation of MS-JSA accordingly.
• Pre-Inspection of the equipment shall be conduct by HSE, inspection witness and approve byclient
teams.
• For equipment, valid third-party certificate should be available and must be attach with work permit.
• Access & Egress routes shall be cleared & marked and communicated to all work forces.
• Working area will be hard barricaded and prohibited for un-authorized persons.
• Material Storage area shall be prepared & barricaded at the designated place.
• Equipment stand by area shall be prepare and barricaded at the designated place.
Step 2: Ensure Intimal Gas Testing by Permit Issuer before starting work
Step 3: Shifting of material & tools to work site manually by labor and using of hand lifters and
wheelbarrows.
• Visual inspection of tools and equipment must be performed prior to before entry inside the terminal.
• Mobilize equipment and hand tools and material to site manually on wheel barrow by manpower
• Trained and competent resources must be deployed
• Ensure Equipment’s is third party certified and verified according to third party

Step 4: The area will be barricaded, Safety Sign Boards will be displayed, A fire watch & 2-fire
extinguishers must be on-site during work.
Step 5: First, inform to gantry supervisor about daily working activity in Pump House and Take
permission to Start work & Work will be scheduled according to the window provided by Filling
Gantry Operation Team in the allocated.
• After taking permit then inform the gantry supervisor on site about daily working activity in pump house
and take permission to start work
• Complete all the necessary tools and equipment before start of work
• Schedule and plan work according to the window provided by Operation team
• Start dismantling the damaged floor from a side to the other end with the help of Hilti (Jack Hammer)
