Chronology :  On Thursday, March 28, 2024 at 09.15 Ilham Ramadan (Loss

Prevention Supervisor) received information from Shelly (FOH
Supervisor) by WhatsApp group that guest room 925 Mr. Liu had a
stomach ache

Detail chronology

 28 March 2024 at 9.15 AM, Mr. Liu 925 (Friends of Mr. Qing
Shi Titanium Elite Member) sitting in the lobby and look like
have stomachache. Shelly approach the guest and he said he
feels diarrhea since yesterday. Shelly asked what he did eat
yesterday or the day before? But the guest was not sure. Then
Shelly took and accompanied the guest to the doctor at Kimia
Farma in front of the hotel. The doctor said it because of the
microbe in the food. After that, the doctor gave some medicine
for Mr. Liu and explained to Shelly. After from the doctor,
Shelly gave a bowl of porridge with soy sauce only, and make an
oralit for Mr. Liu, also asked him to get his medicine. Shelly
offering Mr. Liu to have another porridge in the afternoon and
evening if he needed. Now, he is rest in his room 925, and will
inform if he needs any assistance.
Action Taken :  Shelly (FOH Supervisor) escorted guests to Kimia Farma Clinic for
treatment
 Dr. Ratna Nurhayati (Kimia Farma Clinic) gave medicine for
stomach ache to Mr. Liu
 Shelly (FOH Supervisor) gave a bowl of porridge to Mr. Liu
 Make incident report
 Submit to MIRA

Conclusion :  Based on information from Dr Ratna Nurhayati (Kimia Farma

Clinic) the cause of guest room #925 Mr. Liu had a stomach
ache because of the microbes in Mr. Liu food.
 According to Mr. Liu statement, he did not eat food at the hotel
which caused stomach ache
 Currently Mr. Liu is resting in his room 925.
