I. DESCRIPTION FORM

This form is used only to identify the unit and its owner. This tag is not aimed to show
the condition of the unit. This tag must be used on every unit or component that has been
removed from an aircraft for operational reasons, such as gaining access to another area or
unit or just as long as it is in maintenance or repair process.

II. FILLING INSTRUCTIONS

 Description: Name of the unit.

 Removed from: Place or aircraft from which it was removed.
 Part number: Part number of the unit.
 Serial number: Serial number of the unit.
 Removed by: Name of person in charge of the removal.
 License Number: License number of the mechanic or person performing the work.
 Signature: Signature of the mechanic.
 Date: Issue date of the tag, use the format dd-mm-yy.
