RE:AN INCIDENT THAT OCCUREDON THE 19th NOVEMBER, 202

Dear Sir/Madam

On the morning or January 21,2024, an unfortunate accident occurred the workshop,


resulting in an injury to one of the workers. The accident took place at around 1:30 pm
during a woodworking task. A worker was operating a table saw when their hand
accidentally came into contact with the rotating blade. As a result, the worker suffered a
deep cut to their hand. Immediate first aid was taken, workers nearby quickly stopped the
machinery and called for immediate assistance. Safety protocols were in place, including
the use of PPE. However, it was discovered that the worker was not wearing a required
glove at the time of the accident.
