PARENTAL AUTHORISATION FORM FOR FILMING /PHOTOGRAPHYING
STUDENTS UNDER 18
Required in terms of the
GENERAL DATA PROTECTION REGULATION (GDPR)
effective from the 25th May 2018 *
Dear Parent/ Guardian,
During the course of our Young Learners Summer Programme there will be a number of occasions
where photographs, as well as videos, involving our students will be taken.
Apart from regular lessons, we will have excursions and activities, which may include extra-
curricular classes, for example drama, where still and moving images will be part of the activity.
These images will also provide nice memories for the students, especially in years to come.
In terms of the GDPR, however, we are obliged to seek your authorisation for photos of any child to
be taken and images reproduced for publicity purposes on social media platforms and other
marketing and publicity-related purposes and used in other School publications that may be
published on the website or another website or elsewhere.
In the absence of our receiving your authorisation, your child will be asked to absent themselves
temporarily from any activity where photos will be taken or filming will take place.
If you agree to your child having their photo taken and/or appearing on film you are kindly requested
to complete and sign this form.
Names and/or contact details of children will not appear on photos and/or on film.
I, the undersigned, agree to allow authorised staff members of International House Malta to take
photos of and/or film the child named below and for these images to be used for any diligent
purpose.
I understand that:
images will be held in accordance with the Data Protection Act for a period of up to 5 years;
images captured in the video recordings and/or photographs will be the copyright of
International House Malta and any other intellectual property which arises in the recordings
will also belong to IH Malta;
I have the right to ask IH Malta to stop using the images captured in the video recordings
and/or photographs at any time, in which case they will not be used in future but may
continue to appear in publications already in circulation.
