POLICY & PROCEDURE FOR CARE OF VULNERABLE PATIENT:

1. POLICY:

1. Risk assessment for vulnerable patient should be performed for all the patients are coming
in clinic.

2. Safe and secured environment for this vulnerable groups need to be maintained i.e trolley
side railing, wheel chair safety belt, hand rail in toilet, clean and dry floor. Safe and
secured environment is provided the personnel of maintenance department, housekeeping
department and nursing department.

3. Nursing staffs are trained enough to take care of vulnerable patient and they should take
care patient as per the assessment done by duty medical officer.

2. PURPOSE:

 To identify the groups of patients who will be considered ‘vulnerable’.


 To define a mechanism of assessment and management of the vulnerable patients.

3. SCOPE:
All elderly, physically and/or mentally challenged and children coming in clinic.

4. RESPONSIBILITY:
Nursing staff, Doctors and Support Staff (Operational staff, Housekeeping staff)

5. DEFINITION:
A vulnerable patient may be a person who-

 Age above 65 years / below 12 years


 Post-operative
 Physically challenged
 Mentally challenged
 Person on immune-suppressive drugs
 Person on chemotherapy

6. PROCEDURE:
1. Provide prompt attention and service and minimize waiting times in Clinic and
diagnostics area-
 Patient above 65 years
 Minors / any other patient in apparent discomfort /distress
 Patients on wheel chair/trolley/walking aids
2. Depending on the vulnerable aspect, each patient shall be provided with actions and
support tailored to ensure that care and safety aspects are addressed.
3. Vulnerable patients shall not be left alone at any given time.
4. Female patients and children of both genders shall be attended for their physical
interventions such toilet by a female attendant.

The following steps are to be taken to protect our Vulnerable Patients-

1. All members of staff interacting with patients should be trained in the care of
vulnerable patients, especially those with any disability or mental health problems.
2. When a patient becomes delirious or aggressive during the course of consultation,
special care should be taken to ensure that the patient’s aggression is treated by
keeping the dignity and safety of the patient in foremost in our minds.
3. A patient should not be physically restrained unless treating physician written order
is obtained.
