    Traffic incident
    Location: (Pad # 10)

    While reversing from the parking area southwest side of MS1 Building the TOYOTA Car HID-179 was hit by the MAC Loader CAT 267,
    which was traveling from Pipe Rack 9 towards north side of Compressor “D” for back filling purpose.
    The rear glass, rear body, roof and right back door of the car was damaged. No personal injuries sustained.

    Basic Causes:
    • Failure to react to an unusual situation. •Failure to check that it was “all-clear” to reverse out of the parking
    • Improper car parked (Near to construction activity). spot.
    • Inadequate knowledge about site driving conditions.
    Recommendation to prevent repetition of the situation:
    • Advise all drivers in toolbox meeting to obey the site rules & regulations and park the vehicles/ equipments in designated parking area.
    • Post more awareness signboards in the site.
    • Remove and / or restrict access to the work area by “private” vehicles.
    •Remind Operators to be aware of other vehicles, etc, especially when passing parked vehicles and when approaching pedestrians.
