DentaSmile is one of the successful running clinic in Apalit, Pampanga. They

started their operation in 2015 and since then, the clinic has been
successful. The clinic serves an average of at least 15 patients each day and
an approximation of 400 regular patients in 3 years.

As the number of customers are increasing, also the number of file records
to keep are becoming excessive. Because of this the clinic needs more file
cabinet. It does not only take time to find files, but also requires space and
money. Updating and monitoring of these files has become dificult. So
patient record-keeping with payment monitoring system for DentaSmile will
not only save them a great deal of time and money, but also expected to
provide a more efficient storage and retrieval of patient record.

is be paid by only one PATIENT.

4. A TREATMENT PLAN contains one or more TREATMENT. Conversely,

each TREATMENT contained by only one TREATMENT PLAN.
