(TEACHER OBSERVATION)
• The child can identify some letters of the alphabet
and its sound. (/s/, /x/, /z/)
• She can also identify color yellow, which is her
favorite color.
• She loves books and listening to stories.
• She shows interest to learn new lessons by paying
attention when her teacher is saying something.
SOCIAL SKILLS (DEV. PED.)
• According to her Developmental Pediatrician, she shows
ability of a two-year old child in social skills.
• She is more aware of other children and may play with them.
• She would pull one’s hand when she has needs and does not
yet use gestures in pointing.
• She has outburst of words but when asked, she could not yet
respond verbally.
• The child would cover her ears to certain music and would cry
to a particular song.
GROSS MOTOR SKILLS (DEV. PED.)

• Based on her reevaluation result last October 06,


2016 the child is currently showing the attributes of a
four-year old child in gross motor.
• In school she can perform the following skills:

EMOTIONAL SKILLS
(TEACHER OBSERVATION)
• In class, she tend to be impatient when it comes to doing her
activities. She will immediately perform the task even without
the instruction from doing so.
• When she is playing a certain toy and her classmate will grab
it from her, she tend to be upset by making some actions like
crying without tears and will do head bang at the couch.
• In eating at the restaurant, she can now wait her food to be
served and will just sit down.
• She can identify when her teacher is mad at what she has
done and will run to escape from her.
SPIRITUAL SKILLS
(TEACHER OBSERVATION)
• Part of our daily routine is praying before going home,
Keziah will stop from what she is doing while everyone
is praying, if she is interested to go home, if not yet she
will definitely roam around the classroom and distract
others who are praying.
• Before taking recess, everyone in the class is
encouraged to pray before eating, Keziah will not
verbally recite the prayer but she will pause for a while
and let the teacher to finish the prayer then take her
recess.
SELF-HELP SKILLS

(DEV. PED. & TEACHER OBSERVATION)
Eating/feeding and drinking: She can eat alone with minimal spillage and would
sometimes disregard to use spoon. In drinking, she can manage to drink even
running or walking without spillage. Recently, she can wait for her food to be
served.
• Grooming: She managed to dress alone with her school uniform and even wearing
socks and shoes. She can also take off shoes in entering the class and put it in the
rack but hesitate to use slippers inside the classroom.
• Personal hygiene: She washes her hands before eating and when we do hand
activities which is messy. She does not like to have her fingers messy with small
amount of glue. When taking a bath, there is a need of minimal assistance from the
elder.
• Toileting: She does not say anything when she wants to use the comfort room,
instead she will just do gestures like someone who look like to pee or poop. She
will use the comfort room without hesitation. Someone will need to attend to her
while peeing or pooping.
