 FACTORS
The materials and pipe supports were well stored and secured, the distance to the edge of
platform from the material storage is about 2.8 m; hence a striking against/trip may not
lead to move or fall the object considering the distance and the weight of object.
Work profile and personnel behavior towards health and safety is dedicative and well
known if the intentional act taken to consider.
So far there is no safety breach against the personnel who were present there at the location
and they are not involved in any safety and security violations at the site or in company
profile.
The area or the environment where the subject occurrence was classified as high noise
area where the noise level is above 85 dB and double hearing protection is mandatory.
On the specific date and time the wind speed was high (42 km/h or 22.6 knots) within the
RLIC and this aspect also may affect the ability to hear the noise.

 Previous history –He was recently joined in Trags company & he
was not involved in any safety and security violations at the site
or in company profile in this time period.
Incident statement –I was making welding habitat by using fire
blanket on PRV deck area at train 07 (MR top side elevated
platform) & after completion of my work one of my colleague
said (he was standing down side)one pipe support fell down but
we didn’t heard any side when we are working at top due to
high noise (above 85 db.)

 Incident statement-I was working in train 7 doing pipe

support alignment ,after completion of my job around 5.30pm
I came down to the rest shelter after that I heard from my
colleague one pipe support fell down from top

2008 & he was not involved in any safety and security

Incident statement-Myself and Mr.kul Prasad working at
scaffolding platform making welding booth after the job
completion I came down through a ladder and the remaining
personnel's came down from other ladder ,am not aware about
the incident when I was in top side of the platform

 Previous history: He has been working in Trags since 2005 & he
was not breaching any safety and security violation in this time
period at the site as well as company profile.

 Incident statement –we are making welding booth in train 7 at

the top of the elevated platform when we completed the work one
of our colleague said (he was standing down side )one pipe
support fell down from the top, but we don’t know about the
incident when its happen

 Previous history :He was working in Trags since 2013,as well
as he was not breaching any safety and security violation in this
time period in the company profile
 Incident statement –On 23-01-2017 ,four personals were
engaged including me for open flame activity at 17.40 hrs.
myself came down to close & remove fire hydrant hose &
scaffold planks ,that time remaining four personnel's are on the
PRV deck for house keeping ,when removing scaffold planks I
heard some sound of falling object beside me at approximately
2mtr distance ,when I saw I noticed a beam on the ground

 Conclusion of the occurrence
By analyzing the evidence and the probability, investigation team
considering that
The beam was kept on the existing Grating (top side)may chance
to slide to come on the fire blanket(During vibration or Striking
against the object by personnel/tools/materials ,nevertheless The
fallen object was hidden or was not visible owing to it may cover
with fire blanket that found at the location.
The probability taken to conclude owing to the study on the
frequency of similar dropped object occurred in the industry as
well as it was included in personnel statement that they were
pulled and shifted the fire blanket.
