harus disetujui oleh Owner sebelum kick off meeting di lokasi kerja. The main
contractor as the winner of the tender, has responsibilities to finalize OHSE plan

COHSEMP INTRODUCTION
Tujuan/ Purpose
- COHSEMP mendefinisikan persyaratan minimum untuk praktik dan prosedur

contractors/ vendors

MANPOWER & MCU
Sebelum mobilisasi, Kontraktor diwajibkan untuk memastikan.
Prior to Mobilization, it is mandatory for Contractor to:
1. Perencanaan tenaga kerja, dan fungsi sesuai keahliannya/ Manpower Planning with role and responsibility
2. Pekerja Kontraktor melakukan MCU, dan list pekerja yang sudah lulus MCU diberikan ke Client/ Medical

MOBILIZATION PHASE
• Sebelum mobilisasi, Kontraktor, Sub Kontraktor dan Employer wajib mengadakan kick-off
meeting untuk memastikan semua pihak memahami aturan dan persyaratan K3 di projek./ Prior
to start the mobilization, Contractor, Sub-Contractor and Employer must conduct the kick-off
meeting to ensure that all parties understand the safety rules and requirements on site
• Saat kick-off meeting, Kontraktor wajib mempresentasikan projek safety misalnya: perencanaan
mobilisasi dan kondisi jalanan, program K3L yang dituangkan dalam project safety plan./ During
kick-off meeting, Contractor must present the site safety assessment such as: mobilization and

terlepas dari apapun aktivitasnya di lapangan./ During the course of construction,
the contractor shall properly organize all activities on the job site to the extent
that good housekeeping shall be practiced at all times.

- Semua kabel listrik harus dipastikan digantung di ketinggian
2.1 meter, tidak boleh terletak di tanah./ Electric cable shall
be kept at the minimum height of 2.1 m overhead. At a
minimum of once per working day, the contractor shall roll up
all electric cable, welding leads, and reposition only those
are required.
- Tiap pekerja wajib melakukan housekeeping ini jadi
tanggung jawabnya./ Each employee shall be instructed to
practice the required housekeeping as part of his/ her
assigned duties.

employees (refer to Permenkes No. 70/2016). All portable toilets shall be kept
clean and sanitary and shall be located in an easily accessible area. If they
are to be used at night, the area shall be adequately illuminated.

Kontraktor akan memastikan bahwa semua personil
yang bekerja di projek telah di training dan kompeten
untuk tugasnya masing2. semua training wajib
didokumentasikan./ The contractor shall ensure that
all personnel working on the site have been properly
trained and are competent to perform the tasks to
which they have been assigned. All training shall be
documented (recorded in "writing") and made
available at all times to Employer representative

Semua personil Kontraktor diwajibkan mendapatkan
induksi di awal sebelum bekerja untuk memahami
persyaratan keselamatan kerja di projek. Semua

personnel shall be required to receive an initial
induction by the contractor's safety officer regarding
contractor safety procedures and the requirements of
this specification. Escorted visitors shall be provided

Weekly meeting
Tiap Kontraktor wajib melakukan rapat

Each contractor shall conduct weekly safety
meetings with all contractor personnel and its
subcontractors. Minutes and attendee's
signatures shall be kept and submitted to the
Employer representative.

Toolbox meeting, Safety Talk
Rapat pagi 10-15 menit sebelum bekerja wajib dilakukan
dan dihadiri oleh semua pekerja. Hal ini memungkinkan
semua pekerja faham akan bahaya dari pekerjaan dan
cara penanggulanagannya./ A daily informal 10–15-minute
toolbox meeting shall be held at the beginning of each

Inspeksi peralatan & alat. Sebelum semua alat dibawa ke
site, Kontraktor wajib:/ Before any tools and or equipment
are allowed on site, the contractor is mandatory :
- Daftar peralatan di pos keamanan dan dicek oleh
Employer rep./ register equipment & tools using the
appropriate inspection form at the guard house, and to
be inspected by Employer rep before allowed on site.
- Jika lulus cek, akan ditempel stiker./ If allowed,
equipment and tools will receive a color approved sticker.
- Jika ada alat yang tidak aman, maka Employer akan
mengeluarkannya dari lokasi./ If Employer representative
identify any unsafe tools and or equipment, Employer
has the authority to immediately remove them from site.
- Kontraktor wajib menyediakan stiker inspeksi dengan

Kuning artinya berhati-hati dan perhatikan amaran./ Yellow warning tape shall

METHOD STATEMENTS/ JOB SAFETY ANALYSIS (JSA)
Suatu metode kerja/ JSA menjelaskan bagaimana suatu pekerjaan dilakukan
dengan aman. A method statement / Job safety analysis sets out how a job or
process will be carried out, including all the control measures that will be applied.
Informasi yang ada didalam method statement/ JSA./ Method statements/ JSA
should include most of the following information:
Description of task;
Details of means of access;
Protection of existing work locations;
Personal protection requirements;
Safety of others (public, other employees);
Other health precautions;
Plant and equipment to be used;
Maintenance requirements for above;
Training requirements;
Waste disposal requirements;
Emergency procedures for each works mentioned in the method statement;

IJIN KERJA/ PERMIT TO WORK - PTW
Employer mewajibkan sistem “ïjin kerja” untuk memastikan semua
aktivitas di projek tercatat. Ijin kerja disetujui oleh Employer kepada
Kontraktor./ Employer requires a "permit to work" system to keep track
of all construction work in an operating area. The permits are issued
by the Employer representative to contractors/workers.
Ijin kerja akan dikeluarkan per shift kerja. Ijin kerja terdiri dari ijin

KERJA PANAS/ HOT WORKS (Welding, Cutting, and Grinding)
Kontraktor wajib melakukan pekerjaan pengelasan dan pemotongan sesuai dengan
aturan pemerintah sbb./ The contractor shall perform welding and cutting according
to governmental requirements:
- APAR wajib disediakan di area pekerjaan panas. Pengawas api wajib berjaga
saat pekerjaan panas berlangsung./ Fire extinguisher must be located within the
immediate area of any welding, cutting, grinding, or open-flame work. A fire
watcher shall be provided when cutting or welding is performed.
- Pekerja wajib memeriksa semua bagian, grounding, clamp, mesin las, selang, dll
sebelum mulai bekerja. Jika ada yang rusak maka wajib diganti segera./ The
user must inspect all leads, grounds, clamps, welding machines, hoses, gauges,
torches, and cylinders before they are put into operation. Any defective items
shall be replaced or repaired prior to being placed into service.

SCAFFOLDS
- Kontraktor wajib menunjuk pekerja yang kompeten (certified scafolder) sesuai

determine the feasibility, appropriate training, and fall protection required during
erection, use, and dismantling of a scaffold.
