I. FASE ADMINISTRASI Judul
• Setelah kontraktor utama di tunjuk sebagai pemenang tender, maka kontraktor
tersebut harus menyiapkan program kerja K3 di proyek secara menyeluruh, dan
harus disetujui oleh Owner sebelum kick off meeting di lokasi kerja. The main
contractor as the winner of the tender, has responsibilities to finalize OHSE plan
based on job description of the contractor before kick off meeting on site project.

• Kontraktor wajib memiliki pernyataan atau kebijakan K3L yang merupakan bagian
daripada kontrak project. Contractor to have a commitment of OHSE policy which
also part of contract document

Tujuan/ Purpose
- COHSEMP mendefinisikan persyaratan minimum untuk praktik dan prosedur
yang harus dipenuhi oleh kontraktor konstruksi/ COHSEMP defines the
minimum requirements for practices and procedures to be complied with by
contractors/ vendors
- COHSEMP digunakan untuk mengontrol OHSE konstruksi diarea project/
COHSEMP is to be used in order to control the construction OHSE on site
- Kontraktor harus bertanggung jawab atas semua peraturan dan aturan untuk
proyek dan secara langsung bertanggung jawab atas keselamatan karyawan
mereka/ The Contractor shall be responsible for all regulations and rules for the
project and are directly responsible for the safety of their employees

Sebelum mobilisasi, Kontraktor diwajibkan untuk memastikan.
Prior to Mobilization, it is mandatory for Contractor to:
1. Perencanaan tenaga kerja, dan fungsi sesuai keahliannya/ Manpower Planning with role and responsibility
2. Pekerja Kontraktor melakukan MCU, dan list pekerja yang sudah lulus MCU diberikan ke Client/ Medical
Check Up - MCU is a mandatory. List of workers passed the MCU to be submitted.
3. Pekerja FIT secara medis untuk bekerja sesuai dengan pekerjaannya/ Workers must be fit for work as per

MOBILIZATION PHASE
• Sebelum mobilisasi, Kontraktor, Sub Kontraktor dan Employer wajib mengadakan kick-off
meeting untuk memastikan semua pihak memahami aturan dan persyaratan K3 di projek./ Prior
to start the mobilization, Contractor, Sub-Contractor and Employer must conduct the kick-off
meeting to ensure that all parties understand the safety rules and requirements on site
• Saat kick-off meeting, Kontraktor wajib mempresentasikan projek safety misalnya: perencanaan
mobilisasi dan kondisi jalanan, program K3L yang dituangkan dalam project safety plan./ During
kick-off meeting, Contractor must present the site safety assessment such as: mobilization and
traffic review, program and plan based on the finalized project safety plan
• Employer mewajibkan Kontraktor untuk melakukan penilaian risiko dan analisa bahaya daripada
method statement dari setiap pekerjaan./ Employer requires Contractors to undertake workplace
risk assessments and job hazard analysis, as the basis for a full written method statement for
each part of the work
• Kontraktor wajib menyiapkan Alat Pelindung Diri (APD) sebelum mobilisasi./ Contractor is
required to provide required Personal Protective Equipment (PPE) before mobilization

• Semua Kontraktor wajib melakukan pelatihan darurat selama projek berlangsung.


Minimum setiap 3 bulan harus dilakukan dengan review selama proses pelatihan
tersebut untuk meingkatkan kesiapan darurat di projek./ All contractors must
perform emergency drills during project. A minimum of once every 3 months drill
must be performed with a review on each drill for an evaluation of site
improvement
• Pelatihan yang dilakukan harus mencakup berikut namun tidak terbatas kepada
pelatihan tentang kebakaran, evakuasi medis, P3K, dll./ The drills must be
included but not limited fire and explosion drills, medical evacuation, first aid
treatment, etc.
• Setiap projek wajib memiliki tim tanggap darurat yang telah ditraining untuk
melakukan tindakan emergency, misalnya: fire fighter, first aider./ Each project to
establish the emergency response team (ERT) and have trained personnel for fire
fighter, and first aider.

PENCEGAHAN KEBAKARAN/ FIRE PROTECTION AND PREVENTION
Kontraktor wajib membuat program pencegahan kebakaran yang akan diterapkan
selama proses projeck berlangsung, dan wajib ,menyediakan peralatan pemadam
kebakaran sesuai dengan yang diatur dalam undang-undang. Program tersebut wajib
mencakup dan tidak terbatas pada./ Contractor shall develop a fire-protection
program to be followed throughout all phases of the construction work and shall
provide for the fire-fighting equipment according to governmental requirements. This
program shall include, but not limited to the following:

PENCEGAHAN KEBAKARAN/ FIRE PROTECTION AND PREVENTION

- Kontraktor wajib memberikan pelatihan terhadap pekerja dalam menggunakan


APAR dan kegunaannya./ The contractor shall give particular attention in training
contractor personnel in the use of fire extinguishers and their limitations.
- Area merokok akan ditentukan oleh Employer. Kontraktor akan menyediakan
fasilitas dan alat pemadam api ringan di area tersebut. Dilarang merokok di area
yang mudah terbakar. / Smoking areas will be designated by the Employer
representative(s). The contractor shall provide suitable receptacles and fire
extinguishers in these designated areas. Smoking shall be prohibited in areas
where flammable or combustible liquids and materials are used or stored.

Ketika diwajibkan menggunakan APD sesuai dengan kebutuhan dan persyaratan
pemerintah, prosedur, atau oleh Employer, maka Kontraktor wajib memastikan pekerjanya
menggunakan APD yang sesuai dengan pekerjaannya masing-masing dimana ada kontak
dengan bahaya dan untuk meminimalisasikan bahaya tersebut./ When it is required by the
specifications, by the government requirements, by the plant operating procedures, or by
the Employer, the contractor shall require all employees to wear appropriate personal
protective equipment (PPE) for all tasks for which there is an exposure to hazardous
conditions or for which there is a need to use such equipment to reduce the hazards to the
employees.

Helmet pekerja yang sesuai standar ANSI Z89.1.2003 atau setara


dengan pengikat dagu untuk bekerja di ketingian, celana panjang,
rompi kerja, dan sepatu safety./ Approved industrial hard hats (refers to
standard ANSI Z89.1.2003 or equivalent) with chin straps, full-length
trousers, high visibility vest and safety-toed leather shoes/boots are the
minimum required equipment and must be worn at all times when on
the job site.

Saat bekerja dalam kondisi berlumpur dan basah, sepatu boot karet
diperkenankan namun yang memiliki plat baja sebagai pelindung./
When working in wet and muddy conditions, rubber boots may be worn.
If the boots are the slip-over type, they shall have a steel toe and mid-
sole or shank for protection against protruding objects.

wajib digunakan untuk pekerjaan tertentu misalnya pemotongan,
pengelasan, penanganan zat berbahaya, dll./ The appropriate safety
glasses (refers to standard CE-EN 166 & AUZI Z87.1 or equivalent)
shall be considered to be required for each special task such as
cutting, welding, toxic, hazardous materials, solvent, thinner, oil & gas
and grease, etc., as mentioned in following part.

Untuk pekerjaan pengelasan, pemotongan, dan grinda, maka penutup


wajah diwajibkan untuk digunakan yg dapat menempel pada helmet./
For Welding, Cutting, Grinding; A face shield with a flip-up lens and
safety glass that attaches to a hard hat is required to be worn.

Sarung tangan wajib digunakan untuk melindungi tangan dan


pergelangan. Sarung tangan kulit, jaket tahan api, apron akan
melindungi bagian tubuh saat melakukan pekerjaan panas tersebut./
Gloves shall be worn to protect hands and wrists. Heavy-leather work
gloves, flame-resistant jackets, aprons or similar clothing made of
leather or other suitable material and providing complete protection to
the arms and torso shall be worn as protection against radiated heat
and sparks when welding, torch cutting, or grinding

Selama masa projek, Kontraktor wajib memastikan area kerja bersih dan aman
terlepas dari apapun aktivitasnya di lapangan./ During the course of construction,
the contractor shall properly organize all activities on the job site to the extent
that good housekeeping shall be practiced at all times.
- Semua area kerja harus dijaga
kebersihannya setiap saat./ As the job
progresses, work areas must be kept
clean at all times.
- Semua bahan, alat dan peralatan harus
disimpan di tempatnya./ All materials,
tools, and equipment must be stored in
a stable position to prevent rolling or
falling.
- Akses jalan yang aman harus terjaga ke
area kerja dan tempat penyimpanan./ A
safe access way to all work and storage
areas must be maintained.
- Sampah harus selalu dibersihkan setiap
hari./ Scrap must be cleaned daily
before, during and after work shift, to

- Semua kabel listrik harus dipastikan digantung di ketinggian
2.1 meter, tidak boleh terletak di tanah./ Electric cable shall
be kept at the minimum height of 2.1 m overhead. At a
minimum of once per working day, the contractor shall roll up
all electric cable, welding leads, and reposition only those
are required.
- Tiap pekerja wajib melakukan housekeeping ini jadi
tanggung jawabnya./ Each employee shall be instructed to
practice the required housekeeping as part of his/ her
assigned duties.
- Kontraktor akan menugaskan petugas kebersihan di projek,
dan jika kontraktor gagal dalam housekeeping ini amka
Employer akan menerapkan metode lain dan segala biaya
akan dikenakan ke Kontraktor./ The contractor shall
designate or provide an adequate number of individuals to
maintain cleanliness in the site and work area. If the
contractor fails to clean up work areas in a timely manner nor
provide an adequate number of personnel, the Employer
representative(s) will pursue other methods, and all the cost
arises from these shall be back charged to the respective
contractor.

Kontraktor akan menyediakan toilet yang layak untuk pekerja sesuai dengan
Permenkes No. 70/ 2016. Semua toilet wajib dipelihara kebersihan dan
sanitasinya./ The contractor shall furnish adequate toilet facilities to contractor
employees (refer to Permenkes No. 70/2016). All portable toilets shall be kept
clean and sanitary and shall be located in an easily accessible area. If they
are to be used at night, the area shall be adequately illuminated.

IJIN KERJA/ PERMIT TO WORK - PTW
Employer mewajibkan sistem “ïjin kerja” untuk memastikan semua
aktivitas di projek tercatat. Ijin kerja disetujui oleh Employer kepada
Kontraktor./ Employer requires a "permit to work" system to keep track
of all construction work in an operating area. The permits are issued
by the Employer representative to contractors/workers.

Pelanggaran terhadap sistem ijin kerja ini akan menyebabkan

SCAFFOLDS
- Kontraktor wajib menunjuk pekerja yang kompeten (certified scafolder) sesuai
ketentuan pemerintah./ The contractor shall designate a competent person

determine the feasibility, appropriate training, and fall protection required during
erection, use, and dismantling of a scaffold.
- Handrails wajib dilengkapi sebelum digunakan./ Appropriate handrails, middle rails,
toe boards, and cleats are required. When timber planks are used for decking, they
shall be tied down or secured to the scaffold structure at both ends.
- Semua scaffolds akan diinspeksi oleh inspektor scaffolder sebelum mulai digunakan,

Judul

1. Pagar pengaman (perintang) sebagai pelindung diperlukan sekitar galian-galian, lobang di


lantai atau atap, pinggiran atap dan platform pada ketinggian tertentu, serta sekitar pekerjaan
pada ketinggian tertentu, untuk mengingatkan orang dari bahaya jatuh. Pagar pengaman harus
dipasang sedemikian sehingga terlihat jelas/ The protective fence (barrier) as a protector is

- Lebih dari 25 dan kurang dari 50 pekerja, maka 2 Safety Officer mengawasi./
More than 25 workers and less than 50 workers two dedicated safety officers
are required – full time
- Jika ada pekerjaan berisiko tinggi, maka dibutuhkan lebih banyak petugas
safety./ More safety officers are required if they have more than one workplace/
high risk activity
- Petugas K3 yang bersertifikasi AK3 Umum./ A certified of “Ahli K3-Umum”
safety officer is a mandatory
