Spot Report
It is done after an important incident took place in a certain area at a
given time. Verbal or written, it must be acted upon within 24 hours.

The idea is to inform an immediate chief (as SOP, considering that


whatever happens in said area is a command responsibility), or that
one from a higher headquarters or office, regarding the details
relative to a particular occurrence. When written, a spot report may
use the PNP message form, especially if the reporting officer is far
from the receiver.
SHOOTING INCIDENT
An alleged shooting incident transpired on
January 16, 2018 at about 4:30 PM at IC 166,
Betag, La Trinidad, Benguet.

It was reported though telephone call by


Armelyn B. Depolio, college level, housewife,
Filipino citizen, female, married, born on
January 10, 1975, 43 years of age, and currently
residing at IC 160, Betag, La Trinidad, Benguet.
SHOOTING INCIDENT
The victim was identified as Delbert C.
Mintas, college level, driver, Filipino
citizen, male, married, born on January 5,
1970, 48 years old, a native of Bontoc,
Mountain Province and residing at IC
166, Betag, La Trinidad, Benguet.
SHOOTING INCIDENT
It was revealed that an unidentified suspect
who is riding a motorcycle without a plate
number shot the victim while the latter is
washing his car in front of his house. The
suspect is believed to be 5’6’’ in height.

The victim was brought at Benguet General


Hospital.

2. An alleged shooting incident


transpired at IC-166 Betag, La
Trinidad, Benguet at around 4:30 P.M.
on January 16, 2018.
3. The said incident was reported at
around 5:00 P.M. on the same date by a
concerned citizen who was identified as
Armelyn Buhay Depolio, college level,
housewife, a Filipino citizen, female,
married, born on January 10, 1975, 43
years old, a native of Benguet, and
currently residing at IC 160 Betag, La
Trinidad, Benguet
4. Investigation disclosed that the
victim is Delbert Cuaban Mintas,
college level, driver, Filipino, male,
married, born on January 5, 1970, 48
years old, a native of Mt. Province,
and currently residing at IC 166 Betag,
La Trinidad, Benguet.
5. The first responder arrived at the
crime scene at about 5:10 P.M./ He
conducted an interview on the
reporting person/and subsequently
arrested the suspect/while the latter is
attempting to escape from the crime
scene./
A caliber 45 with an unknown serial number
which is in the possession of the suspect
was confiscated. The suspect’s name is
Virgel Gapos Palomar, aka “Boy Negro,”
high school graduate, driver, co-worker of
the victim, Filipino, married, born on June
10, 1980, 38 years old, a native of Mt.
Province, and currently residing at FA 163,
Tebteb, Lubas, La Trinidad, Benguet.
He is about 5’6’’ in height and weighs
75 kgs. He was brought to the police
station for questioning.
6. At about 5:20 P.M., I arrived at the
crime scene and conducted further
investigation. I requested for SOCO
assistance. The SOCO Team arrived at
about 5:30 P.M. and conducted crime
scene processing and were able to
gather pieces of evidence.
7. The victim was declared dead on
the spot. He was brought at the La Paz
Funeraria for autopsy. It was revealed
that he sustained five gunshot wounds
to his chest.
8. At the crime scene, five cartridge
cases of cal. 45 was recovered together
with shoeprints at the kitchen, and
fingerprints at the doorknob entrance.
9. The SOCO Team Leader turned-
over the crime scene at around 6:30
P.M.
