In 15 May 2021 the quality department notified from


line inspector with

“The presence of foreign particles in water”

What is the nature of these particles

• Particles was filtrated on Millipore filtration system on 0.45 micron


cellulose nitrate filter
• Particles examined were black solid particles

What is the frequency of appearance of the problem

• Inspection data was collected by the quality supervisor


• The reports of line inspection indicates 9 bottle every 100 bottle was
excluded from production line by quality inspector due to presence of
foreign black particles
What is the percentage of problem in batch

• Batch size = 10000 bottle


• Line speed = 2000 bottle/hour
• Defected bottles = 900 bottle
• percentage of defected bottles per batch = 9%
What is the possible source of the problem

• Root cause analysis is a structured team process that assists in identifying


underlying factors or causes of an adverse event or near-miss.
• Understanding the contributing factors or causes of a system failure can
help develop actions that sustain the correction.
• To identify the root cause of the problem we used the fish – Bone diagram.
What is the possible source of the problem

• A cause and effect diagram, often called a “fishbone” diagram, can help
in brainstorming to identify possible causes of a problem and in sorting
ideas into useful categories.
• A fishbone diagram is a visual way to look at cause and effect. It is a
more structured approach than some other tools available for
brainstorming causes of a problem (e.g., the Five Whys tool).
What is the possible source of the problem

• The problem or effect is displayed at the head or mouth of the fish.


• Possible contributing causes are listed on the smaller “bones” under
various cause categories.

• Further investigation was carried out by sampling 500 empty bottles and
the result was 55 with black spots (11% of inspected bottles).
How to reduce or eliminate the presence of these
particles

• Increase sample size of inspected glass bottles when receiving.


• Reviewing AQL (Acceptance Quality Limits) to discuss the risk and the
deviation of black spots with the supplier.
• Inspect empty glass bottles before filling process.

• Increase sample size of inspected glass bottles when receiving.


• Reviewing AQL (Acceptance Quality Limits) to discuss the risk and the
deviation of black spots with the supplier.
• Change in Black spots AQL from 0.04 (as a minor defect) to 0.001( as
acritical defect )
• Inspect empty glass bottles before filling process.
CONCLUSION

• After investigation the most probable reason for occurring


the problem was due to Primary Packaging Material.
• Empty glass bottles shows the presence of moving black
particles inside the bottles
