Individual Learning Plans
3.1 Learner’s gender, needs, strengths, interests, and
experiences

Annotation:
The samples above are individual learner monitoring plan for learners with special needs. For instance, one student is
submitting modules late because of a medical condition while the other has travelled to Manila to attend to her ailing
father in the hospital. Both has considerable reasons thus the school decided to give interventions such as sending digital
copies of module and remedial activity. The student had slow progress but were able to comply to the needed outputs to
pass the subject. The Individual Learning Plans helped me monitor this learners and help them with their needs.

3.4 Learners in difficult circumstances
1

Annotation:
Photos shown above are the Weekly Home Learning Plan for Oral Communication subject
offered on the 1st semester in RRNHS. The parts highlighted here are the changes made in
the WHLP, specifically, on the length of instruction or direction under Learning Task. At
first, it was very detailed and lengthy, which is too overwhelming for students. After the
Learning Delivery Modality Course was conducted, it was realized by everyone that in a
modular print learning modality, it should be concise and include only module name/
number and few instructions for additional activities as prescribed by the teacher. You will
also notice between Picture 2 and 3 that there is a separate WHLP made for both GAS and
TVL track. It was an undivided decision made by the school head and SHS grade level
chairman for students ease in identifying instructions only for their track. Also on my
subject, I also included differentiated activities for students in respect of their unique
talents and multiple intelligences.

4.1 Planning and Management of Teaching and Learning Process

Annotation:
In Rapu-Rapu National High School, preparation of Weekly Home Learning Plan is
the responsibility of all subject teachers while consolidation is done by the Grade
level chairman. As the grade level chairman of Grade 11, I prepared Week 1 to Week
14 WHLP. Later on it was agreed upon by all personnel including our head to also let
other teachers, aside from the chairman, prepare the WHLP. In this manner,
responsibility is shared and improvements were made. It also noteworthy in the
photos above that formatting was changed to lessen consumption of coupon bonds
for printing. Hence, instruction in the WHLP were also made concise. Moreover, I
also prepared schedule for summative test a time table guide for students to have
ample time for each subject, aside from the usual weekly home learning plan
prepared for the weekly instruction.

4.2 Learning Outcomes aligned with Learning Competencies

Annotation:
Photo 1 shown above is from the Most Essential Learning Competencies
Matrix from the Department of Education for the topic Models of
Communication with learning competency: a. differentiates various
models of communication. Photo 2 is still from MELCS Matrix under the
topic Communicative strategies with learning competency: a. employs
various communicative strategies in different situations. Having this as a
guide I prepared and designed evaluation tools that is aligned and will
meet the learning competencies as prescribed by the MELCS of the
Department of Education.

5.1 Design, selection, organization and utilization of assessment
strategies
1

Annotation:
The photos shown above are evidences of the process of design, selection,
utilization and organization of assessment strategies in the subjects I handled this
school year. Photo no.1 is an adapted worksheet in identifying parts of a paragraph
and recognizing the main idea. These are basic knowledge that is expected of the
learner to evaluate written text based on its properties. This served as their review
material. Photo no, 2 is an additional activity sheet for the topic “Language and
Mechanics”. Photo no, 3 is the summative Test I prepared for the 2 nd quarter of
Reading and Writing. While Photo no, 4 is a sample activity sheet as well as rubric
and direction for the required output/ performance task in the subject Personal
Development which I also handled during the 1st semester.

Teacher-made Learning Resources
4.5 Teaching and Learning Resources including ICT

Annotation:
Shown in the photos are some of the Learning Activity Sheets in Oral
Communication. The modules provided by the Department of Education Division
Office was used in delivering learners lesson and instruction. Since all activities in
the module have their corresponding key to correction, where students can check
their answers, I decided to add activities to reinforce and assess students learning.
Although the aim of modules including answer keys is to develop independent
learning and honesty among students, there is still tendency to manipulate answers.
It is also evident that I usually utilize graphic organizers for students ease of
memorizing and summarizing lessons. To assess them properly I added one or two
activities that comprehensively assesses the contents of the module. In this manner,
assessment can be made more valid and reliable.

The certificates above were evidences of some of the webinars and trainings I attended before the opening of classes (Pictures 1, 2 and 3).
It was the time were everyone was still having a tough time because of the pandemic situation. In the same manner, the teachers are asked
to work from home. To continue learning and professional development I immersed to different trainings sponsored by DepEd and other
respected institutions. Picture 4 is a certificate from DepEd after having attended the district In-service Training last December. This
webinar capacitates teachers on the new normal education and be more acquainted on the different learning modalities more importantly,
modular distance learning. It was all unplanned, but I attended as opportunities for free webinar arises.

4.2 Professional Development Goals

Annotation:
Photos 1 and 2 are webinars sponsored by Department of Education while Photo 3 is fa free webinar conducted by Philippine
Association of Graduate Students in Education (PAGSTE), a private organization. Truly, teachers need upskilling especially on the Use
of Technology as well as awareness on Mental Health Issues. These webinars helped me to be more prepared as I handle students in the
new normal. As a teacher, we must be well-informed and adaptable of the changes that happens around us. Being facilitators of learning
and the prime person who delivers education, there is so much expected of us. As a teacher, I aim to be more innovative and find
creative ways in assessing students learning especially this time when we are physically apart from them. These webinars and training
has helped me to achieve my goals as a teacher. And in the future, I still hope to have more of these as teachers and students became
more adept to the new normal in education.

Reflective Summary 12

“When you can’t change the direction of the wind – adjust

your sails.”

In this time of pandemic, teaching became more arduous

especially to rural and island schools. Transportation and
communication is challenged by distance, weather
conditions, and stable connectivity. It is unknown to
everyone when will the pandemic end but it is clear to
educators that education must continue. Hence, the
leaders and heads of educational institutions are
continuously devising ways to address this crisis. One of
these are the different learning modalities.

In Rapu-Rapu National High School,

the learning modality is print modular
distance learning.

The Department of Education knew

the need to capacitate the teachers.
Thus, the conduct of LDM Course 2.
This collection of work is a tangible
reflection of my learnings and
achievement throughout the course.
It also shows how this endeavor has
helped me in my journey as a teacher
facilitating LDM. It capacitated me not
only professionally but also developed
my character (adaptability) in
responding to the needs of my
students.

5.2 Monitoring and evaluation of learner progress and achievement

Annotation:
To monitor students progress and achievement I created a FB Messenger Group chat for each section. We also have a
separate GC for all G11 students. Using this platform I am able to send students reminders on the following: a.
submission and distribution of modules b. updates on their records such as unsubmitted test or notebook c. unnamed test
papers, activity sheets and notebook.
The photo on the bottom right is taken during the home visitation to students with low grades and poor performance.

Annotation:
The pictures above are screenshots of conversations with students who wanted
updates on their grades thus I provide them feedback on how they can improve
their rating or grades. When students have missing activity sheets or test I gave
them chance to resubmit by giving them digital copies thru messenger. Photo
no. 4 is a conversation with a student whom I send digital copies of modules
and activity sheets because of personal circumstances. I make it a point to
monitor and evaluate her properly despite the situation.

5.3 Feedback to improve learning

Annotation:
Above are some screenshots of personal and group conversations with my students. I make sure to appreciate and
commend their efforts. We may be physically apart but students must still feel the presence of their teachers. Itis also my
way of building rapport with my students thereby they wont be afraid to ask questions or raise concerns whenever arises.

The photo on the bottom right is a certificate given to a student with Honors. His parents gladly received the certificate
during distribution of modules.
