This form used based from the submitted outputs of the learners just to
know who needs an assistance when it comes to understanding and
who cope up the lessons.The intervention that I used is
homevisitation,chat and call.
Individual Monitoring Plans

ANNOTATION:
A learner got 75% mastery in ESP.
To help him/her develop his/her skills, I
designed a doable intervention
appropriate to the distance learning
modality such as conducting tutorial and
home visitation. These strategies helped
the learner to improve his academic
performance .These are my artifacts
showing my parents who are having
trouble accomplishing their outputs. This
conference involves setting up
strategies and plan so that learners will
finish their modules on time. Techniques
such as giving them schedules when to
answer a specific subject and monitoring
to be made by the teachers and parents.
This allows tracking of progress and
incorporating intervention program to
avoid learning obstacles.

Annotation
 During the first grading period Weekly Home
Learning Plan was used as per DepEd Order. Weekly Home
Learning Plan shall aid teachers and parents in keeping
track of the day-to-day in school and off school general
learning process as they implement the most suitable and
feasible alternative learning modality based on their
school context. Our 1 st quarter WHLP was a bit
confusing to our learner and parents, and some of them
are complaining that there are many learning tasks every
day. According to them, not only one they need to assist
and facilitate in just one day, and the teacher sends a lot
of learning lesson. To address this problem, we revise our
WHLP to understand and follow the everyday task easily.
We decrease the learning task to their modules. We only
get the essential learning competency they need to learn.
WHLP consist of the most essential learning
competencies, learning task, mode of delivery and the
duration.

ANNOTATION:
In order for the students to have further understanding of
the lesson,I together with my co-teacher,we created or produce
another activities to catch up the lesson specially who's lagging
behind. We make also a syllabus inorder to have a guide of the
parents for everyday lesson.

LDM or learning delivery modality course , which


tasked me to collect various verification artifacts, truly
reflect my abilities and achievements as a public –
school teacher .LDM helps me tract, assess and
evaluate my performance as a modular distance
learning teacher.LDM allows me to follow all the
learning resources, discover how to use the new
platforms that alleviate the quality of education despite
of pandemic. It reveals the development of my skills in
preparing educational resources as well as responding
and addressing the needs of my learners, their parents ,
my co worker and my self.LDM allowed me to relate
my personal growth and professional development.
