CASE SCENARIO

Bianca, a 25-year-old single mother is living in Brgy. Santolan with her 4-year-old
Toddler Markus and 9 months-old baby girl, Zoe. Ceres, a community health nurse
at Santolan Health Center had a home visit to Bianca because she missed two
immunization schedules of Zoe.

Upon checking the immunization of both children, Ceres found out that Markus has

Importance of
immunization to
her child
What is immunization?
Immunization is the process whereby a person is made immune or
resistant to an infectious disease, typically by the administration of a
vaccine. Vaccines stimulate the body’s own immune system to protect
the person against subsequent infections or disease.

Why is it important?
Immunization helps her child to become immune from diseases caused
by bacteria or viruses. It is a simple and effective way of protecting
children from serious diseases.
It not only helps protect individuals but also protects the wider
community by minimizing the spread of disease.

Contraindication to immunization

While vaccines benefit most individuals, there are rare situations


where vaccination is not recommended. Contraindications and
precautions identify situations when vaccines should not be given.

Sometimes there are reasons why a specific vaccine should never be


administered (also called an absolute contraindication) and there are
times when you should delay giving the vaccine for a short period of
time (also called temporary contraindication).
• Reason for NEVER administering a specific vaccine:
- an infant or person who has a history of anaphylaxis or severe
hypersensitivity reaction during previous immunization to subsequent
doses of a vaccine.
- person with a known allergy to a vaccine component should
not be vaccinated.
• Reason for DELAYING administering a specific vaccine:
- if a mother/parent strongly objects to immunization of a sick
infant. Ask the mother to come back when the infant is well.
- if a child is suffering from some chronic condition and is
under medication. Consultation with the primary physician could
result in postponed immunization until after the medication or
treatment is completed.
Additional information for containdication:
There are a few absolute contraindications to the EPI vaccines. Do not give:
• Pentavalent vaccine/DPT to children over years of age.
• Pentavalent vaccine/DPT to a child with recurrent convulsions or
another active neurological disease of the central nervous system.
• Pentavalent vaccine 2 or 3/DPT 2 or 3 to a child who has had
convulsions or shock within 3 days of the most recent dose.
• Rotavirus vaccine when the child has a history of hypersensitivity to a
previous dose of vaccine, intussusceptions or intestinal malformation, or
acute gastroenteritis; and
• BCG to a child who has signs and symptoms of AIDS or other immune
deficiency conditions or who are immunosuppressed.

• Is redness and/or a small lump at the injection site
• An acute inflammatory reaction within 2-4 days after vaccination
BCG • Deep abscess at vaccination site
• Indolent ulceration
• Glandular enlargement
• Local soreness at the injection site
• Severe skin reaction
• Fever
HEPA B • Sore throat
• Swelling in your face or tongue
• Burning in your eyes
• Skin pain followed by a red or purple skin rash that spreads
(especially in the face or upper body)
• Raised temperature (fever)

Why do we have side effects from vaccines?
By allowing an external agent to enter the body, there is always the
possibility that the body will respond, as the cells are made to detect any
infectious agent and fight it.

Most common side effects of immunization to her child are:


• Fever (that is, a temperature over 38.5°C)
• Redness, swelling, and tenderness around the area where the needle
went in
• Babies may be unsettled or sleepy
• Sometimes, a small, hard lump (nodule) at the injection site may
persist for some weeks or months. This should not be concern and
requires no treatment.

Home remedies
after
immunization
After vacccination, children may be fussy because of pain or
fever. To reduce discomfort, you may want to give your child a medicine
such as acetamin- ophen or ibuprofen. If the child is fussy for more than
24 hours, call your healthcare provider.
• Feed your baby to relieve vaccination pain.
• A little sugar water may help shots under 6 months.
• Apply a cool wet cloth if the child experience discomfort at the
injection site after vaccination, it can help reduce tenderness and
swelling.
• If fever persist give child a cold sponge bath.
• Use the 5 S’s approach.

• Swaddle him immediately after vaccination.
• Place him on the side or the stomach.
• Make shushing sounds.
• Swing him in your arms or an infant swing
• Give him something to suck, for example, your breast, a milk bottle
or a pacifier
