Oblique radiograph of the foot shows a transverse avulsion fracture
(arrow) of the 5th metatarsal bone with displacement and extension
into the cuboid-metatarsal joint. Note a nearby ‘os peroneum’

(a) Axial loading forces (1) are exerted down the length of any given bone. The result is a cortical buckle
fracture (fx). (b) Typical type A buckle fractures of the radius and ulna (arrows)
Buckle fracture type B. (a) Axial loading forces (1) are exerted down the length of the bone. However,
other forces such as valgus also are at play. (b) This results in shifting of the axial load axis (3) and
compression of the underlying metaphysis (2). (c) The same laterally placed axial loading forces (3)
eventually result in an angled buckle fracture (fx). (d) Typical angled buckle fracture of the distal radius
(arrow). Compare with the smooth cortex over the anterior aspect of the radius

Periarticular osteopenia involving 2nd to 5th MTP Note the sinus tracts it extends from the cloaca to
joints. Lytic area in 5th metatarsal head – neck with the skin. Two sinus tracts (yellow arrow) extend
cortical breach. Soft tissue swelling with air foci in from the biceps femoris abscess (red arrow) to
mid foot laterally. NO arterial calcification seen. reach the skin.
