Hazard recognition is one of the most critical aspects of occupational safety. In fact, “One of the ‘root causes’ of workplace injuries, illnesses, and
incidents is the failure to identify or recognize hazards that are present, or that could have been anticipated.”

• Using unguarded equipment or tools inappropriate for the task
• Poor housekeeping within the work area
Task Based • Inadequate work area protection (fire, electric shock, exposure)
Hazards • Poor ergonomic conditions or insufficient space for safe work
• Insufficient containment and protection in case of an emergency
• Lack of task-related safety training and supervision

2 Behavioral Hazards

Not maintaining focus, acting distracted and distracting others
Rushing ahead, attempting to skip steps to finish the task sooner
• Not wearing personal protective equipment
• Acting complacent due to high familiarity with the task

3 Environmental
Hazards
•For example:
• Physical workplace set-up (furniture, lay-out, landscape)
• Surrounding environment (air quality, light levels, noise in area)
• Contamination from chemical products or waste

What to Report?

Incidents
• Accident: Unexpected event or attempt that
leads to an HSSE consequence
• Near Miss: An undesired event that under
slightly different circumstances could have
been an incident.

Observations
• Unsafe Acts: Performing a task or an activity
in a manner that may threaten the health
and safety of workers.
• Unsafe Condition: A condition in the
workplace that is likely to cause property
damage or injury.
