• Permit for hospitals (done) • Assist subject to download 14-days report
• Case Report Form for Baseline Study • Collect, summarize, and report subjects’
• data monthly
Collecting baseline data
• Archive all subject data to Research Team
• Recap and report list of subjects

• Facilitate training for healthcare providers
• 1st care contact for study subject
Inviting the study subjects
• Reporting any adverse events
• Explaining informed consent and ADMS
