Rationale/ • To assess the risk of local transmission and conduct

transmission where the case was diagnosed or the
case resides.

• The area can either be classified as a focus or non-


focus depending on the result of the thorough
investigation

• For confirmed malaria patients (since elimination


phase – imported*)

Data collection On the 3rd to 5th day, base on the 1-3 -5 strategy
frequency

Facilities recording and This has to be filled-up by the responsible


reporting; Responsible investigating team (Rural/Provincial SI, Provincial
person/staff vector control officer(if available), Malaria point
person in province, regional entomologist)
