ISSUES & CONCERNS:
For Tulunan:
- Decreased/ Low Number of Vaccinees;
- No Available/ Lack of Thermometer/ Dial Thermometer;
- Inactivated Polio Vaccine is VVM 2 Already (please see attached
photo);
- Some ice packs were not conditioned.
For Matalam:
- Decreased/ Low Number of Vaccinees;
- Error in the encoding/uploading using DICT-VAS system;
- Insufficient fund for internet load;
- Lack of supplies for AEFI Kit.
For M’lang:
• Inadequate supply of Logistics (Alcohol, Cotton,
Syringes, etc.);
• Quarantined Health Care Personnel of the
Vaccination Team.
RECOMMENDATIONS/ ACTION/S TAKEN:
For Tulunan (Recommendations):

- Increase the number of vaccination sites and “barangayan” and intensify Information Education Campaign
to barangays with low number of vaccinees; Invite a speaker who has a good speaking skill to entice a high
number of vaccinees;
- Additional Supply/ Purchase of Thermometer/ Dial Thermometer;
- NIP Coordinators and Covid-19 Pharmacists be reminded on how prepare and condition ice packs to avoid
a high temperature inside the vaccine carrier (as some ice packs were not well conditioned) and conduct
refresher training on proper conditioning of ice packs and preparing of vaccine carriers; provide
temperature monitoring on each vaccine carrier to see if there is a decreased or increased temperature inside
the carrier;
- NIP Coordinators/ Midwife/ Nurse assigned at routine vaccines must, from time to time, check the VVM of
routine vaccines especially before dispensing to the barangay (especially heat sensitive vaccines and
vaccines with VVM and multi-dose vaccines).
- Actions Taken: Conducted a short overview lecture on Cold Chain Management; IPV discarded; IPV
discarded.
For Matalam (Actions Taken):
- Increase the number of vaccination sites and “barangayan” and intensify
Information Education Campaign to barangays with low number of
vaccinees; Invite a speaker who has a good speaking skill to persuade a
high number of vaccinees;
- Call the Program Manager;
- Willing Health Care Workers Personnel Uses Mobile Data;
- Requested AEFI Kits to LGU.
For M’lang (Actions Taken)
- Tap the LGU to purchase the needed supplies; and
- Tap volunteers (BFP, PNO, BHW’s, ETC).

Booster Shots;
- “Certificate of Exemption” was Sold in Some
Barangays Amounting to PHP 150.00 to 200.00;
- Inactivated Polio Vaccine is VVM 2 Already (please

For Pikit:
- Decreased/ Low Number of Vaccinees.

For Alamada:
- Decreased/ low number of vaccinees;
- Circulation of certificate of exemption.
RECOMMENDATIONS/ ACTION/S TAKEN:
For Aleosan:
- Increase the number of vaccination sites and “barangayan” and intensify Information
Education Campaign to barangays with low number of vaccinees; Invite a speaker who has a
good speaking skill to persuade a high number of vaccinees;
- Action Taken: Called Regional Office for Public Attorney’s Office for verification. Said
“Certificate of Exemption” is issued by the said office for “FREE” and “NOT FOR SALE”;
- NIP Coordinators/ Midwife/ Nurse assigned at routine vaccines must, from time to time,
check the VVM of routine vaccines especially before dispensing to the barangay (especially
heat sensitive vaccines and vaccines with VVM and multi-dose vaccines).
- Action Taken: IPV discarded.
For Pikit:
- Actions Taken: Conducted actual master listing of all targets (fully vaccinated, partially
vaccinated, unvaccinated and vaccinated from other municipalities) to determine actual
number of targets and coverage; Intensify Information Education Campaign activities
especially on low coverage barangays by tapping various Community and Sectoral
Leaders; Incentivize fully vaccinated Senior Citizens to increase demand among A2
priority groups.
- Good Practice: Conducted/ Organized Mobile Vaccination Hub to reach far-flung
barangays with low vaccination coverage.
For Alamada:
- Increase the number of vaccination sites and “barangayan” and intensify
Information Education Campaign to barangays with low number of
vaccinees; Invite a speaker who has a good speaking skill to persuade a
high number of vaccinees;
- Actions Taken: Reported to authorized personnel.

- Circulation of fake vaccination card (please see attached photo) sold for
PHP 1,500.00 to 2,500.00;
- No Available/ Lack of Thermometer/ Dial Thermometer;
- No Vaccine Carrier Foam was Present During Inspection;
- Ice packs were not conditioned (especially on main storage of vaccines).
For President Roxas:
- Decreased/ Low Number of Vaccinees;
- No Available/ Lack of Thermometer/ Dial Thermometer.

For Matalam:
- Decreased/ Low Number of Vaccinees;
- Error in the encoding/uploading using DICT-VAS system;
- Insufficient fund for internet load;
- Lack of supplies for AEFI Kit.
RECOMMENDATIONS/ ACTION/S TAKEN:
For Arakan:
- Increase the number of vaccination sites and “barangayan” and intensify Information Education Campaign to barangays
with low number of vaccinees; Invite a speaker who has a good speaking skill to persuade a high number of vaccinees;
- Called through mobile phone, CHO Cotabato City to verify if the vaccinator reflected on the vaccination card was under
the CHO. The said personnel was currently employed under CHO Cotabato City but according to the concerned person, said
vaccination card was fake;
- Additional Supply/ Purchase of Thermometer/ Dial Thermometer;
- The RHU must provide an improvised foam (if the foam is missing) that can be used as a substitute to the original foam;
- NIP Coordinators and Covid-19 Pharmacists be reminded on how prepare and condition ice packs to avoid a high
temperature inside the vaccine carrier (as some ice packs were not well conditioned) and conduct refresher training on proper
conditioning of ice packs and preparing of vaccine carriers; provide temperature monitoring on each vaccine carrier to see if
there is a decreased or increased temperature inside the carrier.
