■ Connectivity of PHC to DH/GH is facilities through Ambulance service but it need to

be atomize as it will improve response time in critical stage of patient.
■ Follow up of treatment given is important and at PHC it is difficult to manage because
of lack of proper patient data.
 Why Smart Health Centre Needed

■ District has District Hospital & Government Medical College along with 150 PHCs across district. Through
which we are having both OPD as well as Inpatient department.
■ Currently there is a lag in PHCs due lack of manpower, equipment's & patient referral connectivity to DH &
GH.

■ There is a need of specialized centers which can be 1 step ahead than PHC as it will have OPD, X-Ray,
Pathology, Medical Store, Ambulance service, 24 x 7 connectivity to DH/GH & IT connectivity to DH/GH for
comprehensive health care data management by this way we can offer 360 degree healthcare for citizens.
■ Smart health Centre can reduce the OPD & IPD Burdon of DH/GH by referring only patient which require only
critical & tertiary care.
■ Smart Health Centre can provide door step services of Emergency Medical response in which dedicated toll free
number can be given to citizen & Critical care ambulance with paramedical staff will be available round the
clock.

■ We will add 1 Health Kiosk Machine per PHC, 1 X-Ray Machine for 4-5 PHC, Pathology
Sample collection can be done in all PHC through trained Phlebotomist. Which can be
processed at Centralized Pathology Labs 4-5 in Tumkuru District.
■ All PHCs will be connected two way patient to PHC & PHC to DH/GH through Hospital
Integrated Management System(HIMS) for patient data transfer & management from 1 hospital
to another hospital.
