NORMAL PHYSIOLOGY OF
MICTURITION

Introduction
• The bladder and urethra comprise the lower
urinary tract ( LUT) and act as a single ( vesico-
urethral) unit during normal lower urinary
tract function.
• The role of this unit is to:
– Adequately store urine ( storage)
– Efficiency empty urine ( voiding).

Normal Micturition
• Result of
– The Contraction of Detrusor musle
– The Opening the bladder neck
– The Opening the striate sphincter
– The free drainage of distal canal
Normal Micturition
• Must be:
– Voluntary, it means the starting and retaining is
under voluntary control.
– Nocturia ( > 50 y, one nocturia is considered
normal).
– Easy and painless, no need abdominal pressure.
– Complete ( bladder must be empty after every
micturition).
Normal Micturition
• Normal micturition last:
– Less than 30 seconds
– Occur 3-4 times a days ( every 3-4 h)
– Eliminate between 300-400 ml ( daily urine: 1500
ml).
Lower Urinary tract Symptoms ( LUTS)

• If the function of this vesico-urethral unit is


disturbed then urinary dysfunction along with
associated LUTS may occur.

Storage Voiding Post-micturition
- Urgency ( impériosités) - Hesitancy - Feeling of incomplete
- Increased daytime - Intermittency emptying
frequency ( pollakisuria) - Slow steam - Post-micturition dribble
- Nocturia - Splitting or spraying
- Urinary incontinence - Straining
- Altered bladder sensation - Terminal driblble
