How to prepare a Check Sheet

1
• Determine what event you want to count

2
• Define what warrants a check mark

3
• Decide when you will collect the data and for how long

4
• Design the check sheet to collect the data

5
• Record the data by marking simple check marks
(Larry Webber and Michael Wallace, 2021, p. 188-119)
When to use it?
Check sheets are used for data collection. For
example, they can help you in: Which tools can we use to do a Check
- Recording the number of defects by type
- Recording the locations of defects Sheet?
- Recording the number of safety incidences It does not require software to use this tool.

• When data can be observed and collected • Manual: Using a pen and paper: In most cases,
repeatedly by the same person or at the same this method will work well unless the data
location volume is high.

• When collecting data on the frequency or •Microsoft Excel: If the data volume is high and it
patterns of events, problems, defects, defect is not practical to collect it manually, you need
location, defect causes, or similar issues some software support.

• When collecting data from a production process

(Nancy Tague, The quality Toolbox, 2005, Check Sheet)

Check Sheet Types
• Classification check sheet: • Frequency check sheet:

• Defect location check sheet: • Measurement scale check sheet:

Tips
• Ensure that all the operators involved in the process are properly
trained and clearly understand every section of the check sheet.
• Run a short pilot exercise to validate that the sheet captures the
required data appropriately and, if required, make the necessary
improvements.
• As far as possible, try to make use of quantitative data instead of
qualitative data, as the former results in more conclusive evidence.

Questions
1. How the check sheets can be use in healthcare?
Check Sheets allow healthcare teams to record historical data from patients, and display patterns or trends.
It also helps to understand data in an easily way, to have an efficient process and to apply any key
performance indicator to look for improvement. In healthcare, check sheets can also be used to record all
the vita signs, health sheet for each patient in ERs and much more.
2. What are the benefits to use Check Sheets?
• Provide a document to collect data
• Helps to create histograms, bar graphs, and pareto charts to analyze data
• It is used to monitor a process performance
• Have a tool that allows to identify easily quality defect types, location and causes.
• Identify the frequency of the problem that is been studied.
3. What is check sheet in Six Sigma?
The check sheet is a simple document that collects data in a real time at a specific location. This
document helps people to analyze data and to take actions about the issue presented in the
process.
